      • Congregate Versus Community Supports: U.S. Scientific Evidence and One Case Stud

        Deinstitutionalization in the United States was extremely successful for citizens with developmental disabilities in the late 20th century. Positive outcomes, and lower costs, were demonstrated in more than a dozen large scale quantitative studies. Community infrastructure was created before people moved out of institutions, and they moved into secure small homes with the supports they needed, plus well planned work and daytime engagement services already in place for them. However, deinstitutionalization for citizens with mental illness was unsuccessful, because it was done purely to save money, without creation of community supports necessary for secure living and working situations. This article describes the successful outcomes of the developmental disabilities movement, and calls for other nations to avoid the mistakes made in the United States, such as large group homes. It also calls for avoiding the failures of deinstitutionalization among citizens with mental illness.

      • Physical Activity: A Comparison of People with Intellectual Disability in Supported Living and Group Homes

        Purpose : Supported living, characterized by very small, family-like structures, are known to be different in many ways from traditional group homes for people with intellectual and developmental disabilities. It is also known that people with intellectual and developmental disabilities in residential settings fall far below population norms in terms of physical activity and exercise. We tested for differences in physical activity between people in supported living versus group home settings. Our hypothesis was that the more individualized supported living settings might be associated with greater opportunity for people to be involved in active physical movement. Method : Large individual quality of life data sets from three states in the U.S. were available to examine. Each data set had the same item on frequency of events described as“Go to a health or exercise club, spa, or center.” Results : One large state data set permitted division of people into large (5 and 6 person) and small (1 to 4 person) settings, and the difference in physical activity / exercise events was quite different both practically and statistically. In two other states, one system was based almost entirely on a 6 person group home model while the other state’s system was based on small supported living settings. The difference in physical activity / exercise between those two systems was large and significant. Conclusions : There are many reasons to think that small family-like supported living models are superior in quality to the older group home model. These analyses provided one more aspect of evidence that people tend to have better, healthier lives in the newer model of residential support. Physical activity / exercise is crucial for quality of life, both in terms of physical and mental well-being. It is suggested that nations embarking on deinstitutionalization efforts, following the CRPD’s Article 19 dictum, will be well advised to avoid or skip the group home model and proceed directly to supported living models.         중증.중복 발달장애인의 탈시설 이후 삶의 변화

        이 연구는 인권침해로 인해서 시설이 폐쇄된 대구의 희망원에서 지역사회로 이주한 중중‧중복 발달장애인 9인의 삶과 행동이 탈시설 이후 어떻게 달라졌는지를 추적한 연구다. 이를 위해 탈시설 이후 발달장애인의 삶의 질과 행동의 변화를 추적하기 위해 미국의 성과연구소(COA)에서 개발된 PLQ(Personal Life Quality Protocol)을 사용하여 시설에서 지역사회로 이주한지 6개월이 경과된 시점의 삶의 변화를 측정하였다. 자료 수집은 당사자 또는 당사자의 상황을 가장 잘 아는 지원자를 대상으로 시설퇴소직후와 조사시점을 비교하여 각 항목에 대한 최근 한달간의 평균 수치를 기록하도록 하였다. 분석은 SPSS 18을 활용하여 T-Test를 실시하였다. 연구 결과 사회통합, 생산성, 자기결정, 삶의 질, 행동의 개선이 크게 이뤄진 것으로 나타났다. This study was an evaluation of a supportive community living demonstration project. Nine persons with significant developmental disabilities moved from the Korean institution Hope Village, where major human rights violations had been documented. They moved into one or two person apartments with full 24 hour supports in Daegu City. Research using PLQ(Personal Life Quality Protocol). PLQ is protocol for measuring developmental disabled person s quality of life after deinstitutionalization. Analysis revealed significant changes in every measured dimension of life and service quality: Behavior, Perceived Quality of Life, Inclusive Activities, Choice Making and Autonomy in Daily Life, Productivity and Meaningful Day Activities and Close Relationships.

        중증‧중복 발달장애인의 탈시설 이후 삶의 질 종단연구 -대구시립희망원 사례를 중심으로-

        This study examines the quality of life before and after deinstitutionalization of adults with profound and multiple learning disabilities(PMLD) from a longitudinal perspective. With the closure of Daegu Municipal Institution ‘Hope’, the quality of life was measured using the Personal Life Quality Protocol from 2019 to 2022 for eight people who participated in the first project to support independent living for adults with PMLD in Korea. It was analyzed through descriptive statistics and linear mixed effect models. The results are as follows. First, social integration activities, choice and autonomy in daily life, and satisfaction with quality of life all improved significantly over time, which was statistically significant. Second, productivity and daytime activities at the time of facility residence could not be measured, but tended to increase over time after deinstitutionalization. However, it was not statistically significant. Third, in terms of behavior, challenge behavior tended to decrease over time and was statistically significant, while adaptive behavior tended to decrease, but was not statistically significant. Through this, it was confirmed that the overall quality of life after deinstitutionalization was improved, and that tasks for inclusion such as jobs and significant activities were still important. 이 연구는 인권침해 문제가 제기되었던 대구시립희망원에서 2019년 지역사회로 탈시설하여 생활하고 있는 중증․중복 발달장애인 8명의 탈시설 이후 시간의 흐름에 따른 삶의 질 변화를 종단적 관점에서 살펴본 것이다. 먼저 대구시 중증․중복 발달장애인 탈시설 자립지원 시범사업의 배경과 국내외 선행연구의 흐름을 고찰하였다. 이후 미국의 성과분석연구소(COA)에서 개발되어 전 세계적으로 활용되어온 발달장애인의 거주서비스에서의 삶의 질 추적 도구인 PLQ(Personal Life Quality Protocol)를 사용하여 2019년부터 2022년까지 4년 동안 측정된 데이터를 R version 4.0.5를 사용하여 분석하였다. 분석방법은 사회통합활동, 생산성 및 주간활동, 일상에서의 선택과 자율성, 주관적 삶의 질, 행동(적응 및 도전행동)의 조사시점별 평균과 표준편차를 구하고, 탈시설 이전과 탈시설 이후의 시간별 변화를 살펴보기 위해 선형 혼합 효과 모형(Linear mixed-effects regression model)을 활용하였다. 분석 결과 첫째, 사회통합활동, 일상생활에서의 선택과 자율성, 주관적 삶의 질이 모두 탈시설 이전 대비 탈시설 이후 시간 경과에 따라 크게 향상되었으며 이는 통계적으로 유의하였다. 둘째, 시설 거주 당시의 생산성 및 주간활동은 측정할 수 없었으나, 탈시설 이후 시간의 흐름에 따라 증가하는 경향을 보였다. 그러나 통계적으로 유의하지는 않았다. 셋째, 행동 측면에서 도전행동은 시간의 흐름에 따라 감소하는 경향을 보였으며 통계적으로 유의하게 나타난 반면, 적응행동은 감소하는 경향을 보였으나 통계적으로 유의하지 않았다. 이를 통해 흔히 ‘최중증 장애인’으로 간주되는 중증․중복 발달장애인의 경우에도 탈시설 이후 삶의 질이 전반적으로 향상됨을 확인하였으며, 중증․중복 발달장애인의 탈시설과 삶의 질 향상을 위한 제언과 심리운동적 함의를 제시하였다.
