        Jain's hand retractor system and stand: an innovative device for hand surgery

        Solo hand surgery is becoming increasingly common, mostly due to a lack of assistants and dedicated operating theatres. The solo hand surgeon faces challenges of proper hand positioning, elevation, and skin flap/tissue retraction. Most commercially available hand retractors or stands do not address all the relevant problems and are not economically feasible for trainee surgeons. The Jain's hand retractor system and stand, described herein, provides solutions to all these problems, and its simple design helps surgeons to reproduce it easily.

        Development and evaluation of film coated aceclofenac and chlorzoxazone tablet with enhanced dissolution rate

        Out of many complications two major problems facing in formulation industry are poor solubility and short half-life of drugs which results into poor bioavailability after oral administration. Solid dosage forms are coated for a number of reasons, the most important of which is controlling the release profiles and bioavailability of the active ingredient. Thus the development of a significant dissolution procedure for drug products with limited water solubility has been a challenge to the pharmaceutical industry. Aceclofenac (Biopharmaceutical classification Class II drug) is a novel non-steroidal antiinflammatory drugs (NSAIDs) having anti-inflammatory and analgesic properties, and is widely used in the treatment of rheumatoid arthritis, osteoarthritis, and ankylosing spondylitis. The investigation revealed that there is no official dissolution medium available in the literature. The objective of present study is to formulate film coated tablet of Aceclofenac and Chlorzoxazone having short half-life by coating with hydroxyl propyl methyl cellulose (E5 LV). Then the formulated tablets were evaluated for its physicochemical properties and in vitro release studies. The incorporation of drugs into polymer matrices is considered a valid tool in order to optimize insufficient features of the drug molecule, like solubility, stability or toxic effects. In the present work, the incorporation of aceclofenac was performed in inert HPMC and there was no chemical interaction between the drug and polymers as concluded from the FTIR studies. In the present study, parameters such as solubility, medium pH, surfactant type, dissolution behavior of formulations, stability, and discriminatory effect of dissolution testing in different dissolution mediums were studied for the selection of a proper dissolution medium. The drug showed an enhanced release rate in the dissolution media containing pH 6.8 phosphate buffer, 900 ml with 0.5 % sodium lauryl sulphate at 75 rpm for 60 min and thus was chosen as the discriminating dissolution method for film coated aceclofenac formulation. It was found that greater than 80 % of the label amount is released over 60 min.

        COMMON FIXED POINTS OF A LIMIT WEAK COMPATIBLE PAIR OF A MULTIVALUED MAPPING AND A SINGLE VALUED MAPPING IN D-METRIC SPACES

        This paper deals with a general contraction. Two fixed-point theorems for a limit weak-compatible pair of a multi-valued map and a self map on a D-metric space have been established. These results improve significantly, the main results of Dhage, Jennifer and Kang [5] by reducing its assumption and generalizing its contraction simultaneously. At the same time some results of Singh, Jain and Jain [12] are generalized from a self map to a pair of a set-valued and a self map. Theorems of Veerapandi and Rao [16] get generalized and improved by these results. All the results of this paper are new.

      • Studies of hypro-mellose (HPMC) functionalized ZnS:Mn fluorescent quantum dots

        <P>Fluorescent ZnS:Mn quantum dots coated with hypro-mellose (HPMC) were synthesized by using simple and facile chemical precipitation technique in which HPMC acts as a novel organic surfactant to passivate the surface, to tune the optical properties and also to functionalize the surface of quantum dot for a futuristic controlled drug release applications. Morphological and optical characterizations were done to see the effect of HPMC coating on ZnS: Mn quantum dots. Structural and morphological studies was done by using XRD and TEM respectively. XRD studies confirms the pure zinc blende phase for all the samples. TEM studies shows that HPMC is efficiently doing the surface passivation in doped quantum dots. Optical studies were done by using UV-visible, FTIR and time resolved photoluminescence. UV-visible studies shows that quantum confinement effect is more prominent and also tunable in case of HPMC coated manganese doped ZnS quantum dots. FTIR studies confirms the functionalization of HPMC on the surface of doped ZnS quantum dots. Laser induced time resolved photoluminescence studies were also done to see the effect of HPMC on the photoluminescence intensity of doped quantum dots. Time resolved studies clearly shows the much desired luminescence stability of the HPMC coated quantum dots. It is observed that both the structural and optical characteristics have strong dependence on capping concentration. As HPMC is a very important viscoelastic polymer and organic surfactant used in various types of drug formulations, these studies opens a new way in the area of efficient drug delivery, controlled drug release and targeting.</P>

        LARGE TIME ASYMPTOTICS OF LEVY PROCESSES AND RANDOM WALKS

        We consider a general class of real-valued Levy processes {X(t), $t\geq0$}, and obtain suitable large deviation results for the empiricals L(t, A) defined by $t^{-1}{\int^t}_01_A$(X(s)ds for t > 0 and a Borel subset A of R. These results are used to obtain the asymptotic behavior of P{Z(t) < a}, where Z(t) = $sup_{u\leqt}\midx(u)\mid$ as $t\longrightarrow\infty$, in terms of the rate function in the large deviation principle. A subclass of these processes is the Feller class: there exist nonrandom functions b(t) and a(t) > 0 such that {(X(t) - b(t))/a(t) : t > 0} is stochastically compact, i.e., each sequence has a weakly convergent subsequence with a nondegenerate limit. The stable processes are in this class, but it is much larger. We consider processes in this class for which b(t) may be taken to be zero. For any t > 0, we consider the renormalized process ${X(u\psi(t))/a(\psi(t)),u\geq0}$, where $\psi$(t) = $t(log log t)^{-1}$, and obtain large deviation probability estimates for $L_{t}(A)$ := $(log log t)^{-1}$${\int_{0}}^{loglogt}1_A$$(X(u\psi(t))/a(\psi(t)))dv$. It turns out that the upper and lower bounds are sharp and depend on the entire compact set of limit laws of {X(t)/a(t)}. The results extend to random walks in the Feller class as well. Earlier results of this nature were obtained by Donsker and Varadhan for symmetric stable processes and by Jain for random walks in the domain of attraction of a stable law.

        A COMMON FIXED POINT THEOREM FOR A SEQUENCE OF MAPS IN A GENERALIZED MENGER SPACE

        The object of this paper is to establish a unique common fixed point theorem through weak compatibility for a sequence of self-maps satisfying a generalized contractive condition in a generalized Menger space. It improves and generalizes the result of Milovanovic-Arandelovic [2], Vasuki [10] and Sehgal and Bharucha-Reid [8]. All the results presented in this paper are new.

        Selective Unilateral Inner Hair Cell Injury: A Case Report

        Sensorineural hearing loss (SNHL) is known to be associated with lesions at various sites, including outer hair cell (OHC) and inner hair cell (IHC) injury, as well as auditory dyssynchrony or vestibulocochlear nerve tumors. Therefore, it is important to establish the differential diagnosis of SNHL, which however is particularly challenging in patients with unilateral SNHL. The test battery approach is effective for accurate diagnosis in such cases. In this report, we discuss the usefulness of the test battery approach to accurately detect the site of the lesion in a patient with unilateral hearing loss. A 14-year-9-month-old adolescent who observed decreased hearing sensitivity in the left ear accompanied by difficulty with understanding speech at a distance was diagnosed with unilateral hearing loss in the left ear. In this report, we describe a rare and diagnostically challenging case of unilateral SNHL in a patient with normal OHC function and selective injury to IHCs, which was diagnosed using a test battery approach.

        Bertolotti Syndrome: A Diagnostic and Management Dilemma for Pain Physicians

        Background: Bertolotti's syndrome (BS), a form of lumbago in lumbosacral transitional vertebrae, is an important cause of low back pain in young patients. The purpose of this study was to assess the etiology of low back pain and the efficacy of treatment offered to patients with BS. Methods: All patients of BS Castellvi type1a during a period of 6 months were enrolled in the study. The patients underwent interventional pain procedures for diagnosis and pain relief. Response to the therapy was assessed based on VAS and ODI scores. A 50% decrease in VAS score or a VAS score less than 3 would be considered adequate pain relief. Results: All 20 patients diagnosed with BS during the 6-month observation period had scoliosis. Common causes of back pain were the ipsilateral L5-S1 facet joint, neoarticulation, the SI joint, and disc degeneration. Responses to various interventions for pain relief were different and inconsistent from patient to patient. In particular, responses to interventions for neoarticular pain were generally poor. Conclusions: Pain in patients with BS does not usually respond to interventional pain treatment. A very dynamic treatment approach must be pursued while managing BS patients, and the treatment plan must be individualized at various stages in order to obtain satisfactory pain relief.

        Bioactivities of Polyphenolics from the Roots of Bauhinia racemosa

        Various extracts of Bauhinia racemosa Lamk. (roots) and the isolated polyphenolics, viz. l,7,8,12b-tetrahydro-2,2,4-trimethyl-10-methoxy-2H-benzo [6,7] cyclohepta[1,2,3-de][1]benzopyran-5,9-diol (racemosol) (1), 1,7,8,12b-tetrahydro-2,2,4-trimethyl-2H-benzo[6,7]cyclohepta[l,2,3-de][l]benzopyran-5,10,11-triol (2), and 1,7,8,12b-tetrahydro-2,2,4-trimethyl-2H-benzo[6,7] cyclohepta [1,2,3-de][1]benzopyran-5,9,10-triol (de-O-methyl racemosol) (3) were screened for antibacterial, antifungal, and antiviral activities. The isolated compounds exhibited profound antibacterial and antifungal activities while methanol extract exhibited potential efficacy against Herpes simplex virus.
