        Cohort profile: National Investigation of Birth Cohort in Korea study 2008 (NICKs-2008)

        Background: An adequate large-scale pediatric cohort based on nationwide administrative data is lacking in Korea. Purpose: This study established the National Investigation of Birth Cohort in Korea study 2008 (NICKs-2008) based on data from a nationwide population-based health screening program and data on healthcare utilization for children. Methods: The NICKs-2008 study consisted of the Korean National Health Insurance System (NHIS) and the National Health Screening Program for Infants and Children (NHSPIC) databases comprising children born in 2008 (n=469,248) and 2009 (n=448,459) in the Republic of Korea. The NHIS database contains data on age, sex, residential area, income, healthcare utilization (International Classification of Diseases10 codes, procedure codes, and drug classification codes), and healthcare providers. The NHSPIC consists of 7 screening rounds. These screening sessions comprised physical examination, developmental screening (rounds 2-7), a general health questionnaire, and age-specific anticipatory guidance. Results: During the 10-year follow-up, 2,718 children (0.3%) died, including more boys than girls (hazard ratio, 1.145; P<0.001). A total of 848,048 children participated in at least 1 of the 7 rounds of the NHSPIC, while 96,046 participated in all 7 screening programs. A total of 823 infants (0.1%) weighed less than 1,000 g, 3,177 (0.4%) weighed 1,000-1,499 g, 37,166 (4.4%) weighed 1,500-2,499 g, 773,081 (91.4%) weighed 2,500-4,000 g, and 32,016 (5.1%) weighed over 4,000 g. There were 23,404 premature babies (5.5%) in 2008 compared to 23,368 (5.6%) in 2009. The developmental screening test indicated appropriate development in 95%-98% of children, follow-up requirements for 1%-4% of children, and recommendations for further evaluation for 1% of children. Conclusion: The NICKs-2008, which integrates data from the NHIS and NHSPIC databases, can be used to analyze disease onset prior to hospitalization based on information such as lifestyle, eating habits, and risk factors.

        Development of Nanoparticulate Formulation of Coenzyme $Q_{10}$ and Comparison of Plasma Coenzyme $Q_{10}$ Response in a Single Supplementation with Regular Coenzyme Q10 Using Rats

        Bioavailability of a nanoparticulate formulation of coenzyme $Q_{10}$ (NQ20), which has high stability in the water phase, was evaluated. The particle size was 188 nm, and the zeta potential value was between -38.8 and -44.8 mV at 4, 25, and $40^{\circ}C$ in distilled water after eight weeks storage. Bioavailability of NQ20 was compared with a commercial coenzyme $Q_{10}$ in oil and water phases as emulsified form using male Sprague-Dawley rats. After single oral administration of each coenzyme $Q_{10}$ solution, the blood of rats was collected at 0, 1, 2, 3, 4, 5, 6, 9, and 12 h, and the concentrations of coenzyme $Q_{10}$ were analyzed by high performance liquid chromatography. The plasma coenzyme $Q_{10}$ levels at 1, 2, and 12 h were significantly higher when the rats were administered NQ20 compared to coenzyme $Q_{10}$ in oil. The maximum plasma concentration ($C_{max}$) and area under the concentration-time curve (AUC) values for NQ20 were $1.10{\pm}0.18$ and $5.92{\pm}1.11mM{\cdot}h/mL$, whereas the $C_{max}$ and AUC of coenzyme $Q_{10}$ in oil were $0.79{\pm}0.07$ and $5.30{\pm}0.62mM{\cdot}h/mL$, respectively(p>0.05). Due to high absorbability and small particle size, NQ20 was found to have similar bioavailability to commercial coenzyme $Q_{10}$ in the oil phase. Results indicate applicability of NQ20 in the food industry, particularly in beverages.

        P-Glycoprotein Inhibitory Activity of Indonesian Medicinal Plants in Human Breast Cancer Cells

        In order to examine their effects on the P-glycoprotein (P-gp) activity in human breast cancer cels,Calotropis gigantea, Curcuma aeruginosa, Merremia mammosa, Sindora sumatrana, and Zingiber casumunar,showed the most potent P-gp inhibitory activity. When each of these extracts was treated together with theanticancer agent, daunomycin, they increased the cytotoxic activity of daunomycin up to IC50 values of les than6.62 M, which is a value with a positive control, verapamil. Also, other 15 plant extracts exhibited significant P-50 values betwen 6.62 and 13.20 M. These prospective samples wil be subjectedto further laboratory phytochemical investigation to find active principles.Keywords . . . . . . .

      • Argyrophilic Nucleolar Organizer Regions in Acute Lymphoblastic Leukemia

        Background : In acute lymphoblastic leukemia (ALL), the importance of argyrophilic nucleolar organizer regions (AgNOR) is not established. Methods : NOR silver staining was carried out in 74 ALL patients. We analyzed the AgNOR parameters; counting parameters including number of AgNOR per cell, percentage of cells with one cluster, and area parameters including mean AgNOR area, total AgNOR area, and its percentage of nuclear area by morphometry. Cyclin A index was evaluated by immunohistochemical stain. We compared the AgNOR parameters with cyclin A index and evaluated the differences of AgNOR parameters in accordance with FAB classification, immunophenotype, a new classification of ALL (ALL with maturation), and response to induction chemotherapy. Results : A positive correlation was found between cyclin a index and AgNOR area parameters and a significant negative correlation was found between mean AgNOR area and number of AgNOR per cell (r=-0.433, P=0.000). AgNOR area parameters revealed the highest value in L3. The number of AgNOR per cell in T cell ALL was higher than non-T cell ALL (P=0.011), and the percentage of cells with one cluster was lower (P=0.002). In cases achieved complete remission(CR) after induction chemotherapy, the number of AgNOR per cell was higher (P=0.005) and the percentage of cells with one cluster was much lower than in cases failed to achieve CR (P=0.013). Conclusions : Our study indicates that the AgNOR area parameters are helpful to predict the proliferating activity of leukemic blasts in ALL. It is suggested that the number of AgNOR per cell and the percentage of cells with one cluster provide a valuable information to estimate the response to chemotherapy in ALL.         A Case of Nephrogenic Diabetes Insipidus with a Rare X-linked Recessive Mutation in an Infant with Developmental and Growth Retardation Tracked by the Korean National Health Screening Program

        Nephrogenic diabetes insipidus (DI) is a rare disease in which the patient cannot concentrate urine despite appropriate or high secretion of antidiuretic hormone. Congenital nephrogenic DI is caused by the arginine vasopressin receptor 2 (AVPR2) or aquaporin 2 (AQP2) gene mutation; the AVPR2 genetic mutation accounts for 90% of the cases. National health screening for infants and children was launched in 2007 in order to prevent accidents and promote public health in infants and children in Korea. The program has been widely used as a primary clinical service in Korea. We treated an infant with faltering growth and delayed development detected by the National health screening program, and diagnosed the problem as nephrogenic DI caused by a rare missense mutation of c.490T>C on the AVPR2 gene. This case can be a good educational nephrogenic DI with a rare AVPR2 mutation, which was well screened and traced by the national health screening program for infants and children in Korea.

      • Eugenol-Induced Apoptosis in Human Gingival Fibroblast Cell and Human Salivary Gland Cell Line

        유지놀은 페놀 계통의 물질이며 치과 치료에 광범위하게 이용되고 있다. 치과 치료의 특성상 유지놀은 치수 및 치아주위 조직에 접촉하게 되고 고사에 의한 세포독성을 유발할 것이라고 알려져 있지만, 이와 관련한 분자 생물학적 기전은 거의 보고 되지 않고 있다. 따라서 본 실험은 유지놀을 치은섬유아세포 및 타액선 세포주에 처리한 후 고사의 분자생물학적 기전을 알아보기 위해 고사 과정에 관련한다고 알려진 Bcl계 물질 및 caspase 활성도를 관찰하였다. 항산화제인 N-acetylcystein (NAC)을 처리한 군과 처리하지 않은 군을 비교하여 MTT reduction assay를 하였고, propidium iodide 염색을 하여 세포 모양을 관찰하였다. 또한 고사 기전을 관찰하기 위해 Bcl-2, Bax, Apaf-1 mRNA 발현과 caspase 활성도를 측정하였다. MTT reduction assay와 NAC을 처리한 군에서 처리하지 않은 군에 비해 세포사가 감소하여 유지놀이 산화제로 작용함을 알 수 있었고, 현미경 소견에서 핵분절 및 염색질 농축이 관찰되어 유지놀은 고사 과정을 통한 세포사를 일으킨다고 생각하였다. 고사 기진을 알아보기 위한 실험에서 치은 섬유아세포에서는 Bcl-2와 Bax mRNA 발현의 변화가 없었고, Apaf-1 발현을 관찰할 수 없었다. 타액선 세포주에서는 Bax의 발현이 증가하고 Bcl-2의 발현은 감소하였으며 Apaf-1 발현은 관찰되기는 하였으나 유의한 증가를 보이지는 않았다. Caspase9과 3의 활성도를 측정한 결과 치은 섬유아세포에서는 변화가 없었고, 타액선 세포주에서는 caspase 9은 감소하고 caspase 3는 증가하였다. 위의 결과로 유지놀은 타액선 세포주에서 Bcl계 및 caspase를 통한 고사를 일으킴을 알 수 있었고 치은섬유아세포는 좀 더 많은 연구가 필요하다고 사료었다.

        Comparison between Kawasaki disease with lymph-node-first presentation and Kawasaki disease without cervical lymphadenopathy

        Purpose: We evaluated the characteristics of patients with Kawasaki disease (KD) who presented with only fever and cervical lymphadenopathy on admission, and compared them with the characteristics of those who presented with typical features but no cervical lymphadenopathy. Methods: We enrolled 98 patients diagnosed with KD. Thirteen patients had only fever and cervical lymphadenopathy on the day of admission (group 1), 31 had typical features with cervical lymphadenopathy (group 2), and 54 had typical features without cervical lymphadenopathy (group 3). Results: The mean age ($4.3{\pm}2.1$ years) and duration of fever ($7.5{\pm}3.6$ days) before the first intravenous immunoglobulin (IVIG) administration were highest in group 1 (P=0.001). Moreover, this group showed higher white blood cell and neutrophil counts, and lower lymphocyte counts after the first IVIG administration as compared to the other groups (P =0.001, P =0.001, and P =0.003, respectively). Group 1 also had a longer duration of hospitalization and higher frequency of second-line treatment as compared to groups 2 and 3 (group 1 vs. group 2, P =0.000 and P =0.024; group 1 vs. group 3, P =0.000 and P =0.007). A coronary artery z score of >2.5 was frequently observed in group 1 than in group 3 (P = 0.008). Conclusion: KD should be suspected in children who are unresponsive to antibiotics and have prolonged fever and cervical lymphadenopathy, which indicates that KD is associated with the likelihood of requiring second-line treatment and risk of developing coronary artery dilatation.

        Effect of Ca addition on selective oxidation of Al<sub>3</sub>Mg<sub>2</sub> phase in Al-5 mass% Mg alloy

        <P><B>Abstract</B></P> <P>This study examined the effect of Ca addition on the selective oxidation of the Al<SUB>3</SUB>Mg<SUB>2</SUB> phase in an Al-5 mass%Mg alloy during oxidation at 400 °C and 500 °C for 72 h. Ca addition led to the formation of the Al<SUB>4</SUB>Ca and Laves_C36 phases besides the primary Al solid solution and Al<SUB>3</SUB>Mg<SUB>2</SUB>. After the oxidation test, there was no significant oxidation in both the Ca-free and Ca-added alloy samples and no visible change in the surfaces between them at 400 °C, which is lower than the melting temperature of Al<SUB>3</SUB>Mg<SUB>2</SUB> (451 °C). At 500 °C, the surface of the Ca-free alloy showed the formation of oxide blisters, which was not observed in the Ca-added alloy. The presence of an Al<SUB>4</SUB>Ca phase at the grain boundaries led to the formation of multi-element oxides containing Al, Mg, and Ca, which can contribute to the formation of a compact oxide layer.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Ca addition in Al-Mg alloy forms two Ca based intermetallic compounds. </LI> <LI> Coexistence of Al<SUB>4</SUB>Ca and Al<SUB>3</SUB>Mg<SUB>2</SUB> phases forms Ca-Mg-Al based oxides after oxidation. </LI> <LI> Ca-Mg-Al based oxides control oxide blister formation from Al<SUB>3</SUB>Mg<SUB>2</SUB> during oxidation. </LI> </UL> </P>
