      • Factored MLLR Adaptation

        <P>One of the most popular approaches to parameter adaptation in hidden Markov model (HMM) based systems is the maximum likelihood linear regression (MLLR) technique. In this letter, we extend MLLR to factored MLLR (FMLLR) in which the MLLR parameters depend on a continuous-valued control vector. Since it is practically impossible to estimate the MLLR parameters for each control vector separately, we propose a compact parametric form of the MLLR parameters. In the proposed approach, each MLLR parameter is represented as an inner product between a regression vector and transformed control vector. We present an algorithm to train the FMLLR parameters based on a general framework of the expectation-maximization (EM) algorithm. The proposed approach is applied to adapt the HMM parameters obtained from a database of reading-style speech to singing-style voices while treating the pitches and durations extracted from the musical notes as the control vectors. This enables to efficiently construct a singing voice synthesizer with only a small amount of singing data.</P>

      • Assessment of cervical cancer screening policy in women aged older than 65 years in Korea

        Updated US guidelines recommend that women aged older than 65 years with evidence of adequate negative prior screening do not require cervical cancer screening. There is no guideline about the appropriate timing and conditions to stop cervical cancer screening in Korea. The purpose of this study was to evaluate the value of cervical cancer screening test in women >65 and to assess the potential age of stopping screening in old women The data of 999 women who underwent cervical cancer screening test followed by coloposcopy at the University hospital from January 2007 to May 2012 were evaluated retrospectively. The mean age was 45.5±13.7 (range: 14-80). Age groups were classified as <21 (n=11), 21-29 (n=128), 30-65 (n=768), and >65 (n=92) and there was no difference in the pathologic results of 65. There was significant higher incidence of cancer compared with ≤CIN3 in women >65 (13.0%) vs. ≤65 (6.6%) (p=0.037). Subjects were further categorized into five age groups (≤55 vs >55, ≤60 vs >60, ≤65 vs >65, ≤70 vs >70, and ≤75 vs >75) to create a cutoff age in terms of the ≤CIN3 vs. cancer, which identified that the cancer incidence was different until the age of 65 but not age over 70. Cervical cancer screening is necessary in women > 65 due to the higher incidence of cervical cancer compared with ≤CIN3. The ratio of higher incidence of cancer was disappeared at the age of 70. New guideline with the appropriate timing and conditions to stop screening should be considered in Korea.

        The Impact of Functional Electrical Stimulus and Proprioceptive Neuromuscular Facilitation to Scapula Adductor on Upper Limb Functions and Gait of the Patients with Stroke

        The present study examined the effects of functional electrical stimulus( group 1), proprioceptive neuromuscular facilitation(group 2) and combined training of functional electrical stimulus and proprioceptive neuromuscular facilitation(group 3) with scapula adductor muscles on scapula movement, upper limb function and gait in fifteen subjects stroke patients. The training was thirty minutes a day, five times a week for six weeks, obtained result as follow, upper limb function was significant difference in the group 2(p<.05) but no significant difference in other groups. The change of weight bearing were significant difference in all the groups(p<.05), and increase of gait velocity were significant difference in all the group(p<.05). In conclusion, when applied with functional electrical stimulus, proprioceptive neuromuscular facilitation and combined training to the scapular adductor muscles, it was oberved in the course of the experiment that proprioceptive neuromuscular facilitation was the most effective treatment among the three methods applied to the scapula adductors.

      • 3′-Deoxy-3′-<sup>18</sup>F-Fluorothymidine PET for the Early Prediction of Response to Leucovorin, 5-Fluorouracil, and Oxaliplatin Therapy in Patients with Metastatic Colorectal Cancer

        <P>The aim of this study was to evaluate 3′-deoxy-3′-<SUP>18</SUP>F-fluorothymidine (<SUP>18</SUP>F-FLT) PET for early prediction of the standard anatomic response and survival outcomes in patients with metastatic colorectal cancer (mCRC) receiving leucovorin, 5-fluorouracil (5-FU), and oxaliplatin (FOLFOX). <B>Methods:</B> The main eligibility criteria included histologically confirmed mCRC, ≥1 extrahepatic measurable lesions, and no prior chemotherapy in a metastatic setting. Chemotherapy consisted of leucovorin on day 1, followed by the continuous infusion of 5-FU on days 1 and 2, and oxaliplatin on day 3. In the second and subsequent cycles of chemotherapy, oxaliplatin was administered simultaneously with leucovorin on day 1. <SUP>18</SUP>F-FLT PET scans were obtained 3 times during the first cycle of chemotherapy: before chemotherapy, 24 h after infusion of 5-FU (day 2), and 48 h after completion of chemotherapy (day 5). The maximum standardized uptake value (SUV<SUB>MAX</SUB>) of <SUP>18</SUP>F-FLT was measured. Treatment responses were assessed by CT after 3 cycles of FOLFOX. <B>Results:</B> Eighteen patients were included in the study. The response rate after 3 cycles of FOLFOX was 27.8% (5/18). The SUV<SUB>MAX</SUB> was increased in responders (<I>P</I> = 0.043) and nonresponders (<I>P</I> < 0.001) on day 2 and was decreased, compared with baseline values, on day 5 in responders only (<I>P</I> = 0.043). Receiver-operating-characteristic curve analysis indicated that the use of a threshold of an SUV<SUB>MAX</SUB> increase on day 2 of ≤45.8% resulted in a sensitivity of 100%, specificity of 69.2%, and relative risk of 2.250 (<I>P</I> = 0.029) for the diagnosis of responders. Use of a threshold of an SUV<SUB>MAX</SUB> decrease on day 5 of ≥10.6% resulted in a sensitivity of 100%, specificity of 76.9%, and relative risk of 2.667 (<I>P</I> = 0.007). Patients with low <SUP>18</SUP>F-FLT flare tended to have longer survivals than patients with high flare (2-y overall survival rate, 77.8% vs. 44.4%; <I>P</I> = 0.051). <B>Conclusion:</B> The <SUP>18</SUP>F-FLT flare observed during 5-FU infusion was associated with poor treatment response in patients with mCRC. The degree of <SUP>18</SUP>F-FLT flare might be used to predict the outcome of patients who receive infusional 5-FU–based chemotherapy.</P>

      • Adoption of a PDA-Based Home Hospice Care System for Cancer Patients :

        <P>This study describes a systematic and efficient point-of-care home hospice information system, which can effectively manage patient information and can easily be incorporated into a home hospice service using wired and/or wireless communications and mobile computing technology. To help with the design of a portable home hospice information system, we analyzed the services provided by visiting nurses and the opinions of users, based on the 2005 Guidelines for Cancer Patient Management Program of the Ministry of Health and Welfare of Korea. We performed a service trial with three experienced visiting nurses. A visiting nurse can use a PDA, implementing the hospice information system, to accomplish tasks that range from registering a new patient to performing nursing services based on the patient's condition, as well as accessing guidelines and searching past information for better nursing performance. The system resulted in a saving of 8.5 minutes in recoding time. We expect the findings of this study to help field workers in community nursing care by reducing unnecessary paperwork and by enabling improvements in the quality of point-of-care services.</P>

        The Use of Health Functional Foods in Gastrointestinal Cancer Patients

        As an adjunct to cancer treatment, the use of health functional foods (HFFs) seems to be increasing. However, little is known for the use of HFFs among cancer patients in Korea. The aims of this study were to investigate the exposure rate of HFF use among gastrointestinal (GI) cancer patients and to examine the relationship of socio-demographic and disease-related characteristics with the use of HFFs. A total of 126 patients diagnosed with GI cancer participated in the study. A cross-sectional survey was conducted using a questionnaire. Over a half of all the patients surveyed (n = 67; 53.2%) used HFFs. Patients who were younger, had higher income, or longer duration of disease showed a trend to use HFFs more frequently, even though the tendency was not statistically significant. The most commonly used HFF was vitamin complex (n = 20; 16%), followed by red ginseng (n = 15; 12%), and sweet wormwood (Artemisia annua) (n = 11; 8.8%). About 26% of all responders expressed concerns for using HFFs. The primary concern was ``going against physician`s recommendations`` (36.8%). About 63% of respondents expressed a desire to consult with their physicians and follow their recommendations. More basic scientific data and educational materials regarding HFFs are required for both health-care professionals and cancer patients. A larger sample and size-controlled groups representing each cancer type will continue to be recruited for participation in this survey.

        Idiopathic pulmonary fibrosis (IPF) is a chronic, progressive fibrosing interstitial pneumonia, which presents with a progressive worsening dyspnea, and thus a poor outcome. The members of the Korean Academy of Tuberculosis and Respiratory Diseases as well as the participating members of the Korea Interstitial Lung Disease Study Group drafted this clinical practice guideline for IPF management. This guideline includes a wide range of topics, including the epidemiology, pathogenesis, risk factors, clinical features, diagnosis, treatment, prognosis, and acute exacerbation of IPF in Korea. Additionally, we suggested the PICO for the use of pirfenidone and nintendanib and for lung transplantation for the treatment of patients with IPF through a systemic literature review using experts’ help in conducting a meta-analysis. We recommend this guideline to physicians, other health care professionals, and government personnel in Korea, to facilitate the treatment of patients with IPF.

      • Development and Effectiveness of Web Based Learning Nursing Process Program on Linked NANDA, NOC and NIC

        This study was done to develop and evaluate comprehensive web based learning program for improving learning ability in the nursing process of nursing students. We evaluated the contents based on the sufficient teaching materials and developed the process for implementation of the educational system. The study was focused on developing guidelines applicable to any clinical situation using a framework of links among all nursing diagnoses, outcomes, and interventions. This study was used a descriptive study and the participants were 62 nursing students. There were significant differences of achievement of learning objectives, application to work, according to the frequency of program use. Therefore, this results indicate that the web based learning nursing process program is effective in improving will help to strengthen learning calculation ability of nursing students in nursing process. Further study should be done develop additional program for improving application.

        Structural evolution of layered Li<sub>1.2</sub>Ni<sub>0.2</sub>Mn<sub>0.6</sub>O<sub>2</sub> upon electrochemical cycling in a Li rechargeable battery

        <P>Recently Li<SUB>1.2</SUB>Ni<SUB>0.2</SUB>Mn<SUB>0.6</SUB>O<SUB>2</SUB>, one of the most promising cathode candidates for next generation Li rechargeable batteries, has been consistently investigated especially because of its high lithium storage capacity, which exceeds beyond the theoretical capacity based on conventional chemical concepts. Yet the mechanism and the origin of the overcapacity have not been clearly understood. Previous reports on simultaneous oxygen evolution during the first delithiation may only explain the high capacity of the first charge process, and not of the subsequent cycles. In this work, we report a clarified interpretation of the structural evolution of Li<SUB>1.2</SUB>Ni<SUB>0.2</SUB>Mn<SUB>0.6</SUB>O<SUB>2</SUB> upon the electrochemical cycling, which is the key element in understanding its anomalously high capacity, through careful study of electrochemical profiles, <I>ex</I><I>situ</I> X-ray diffraction, HR-TEM, Raman spectroscopy, and first principles calculation. Moreover, we successfully resolved the intermediate states of structural evolution upon electrochemical cycles by intentionally synthesizing sample with large particle size. All observations made through various tools lead to the result that spinel-like cation arrangement and lithium environment are gradually created and locally embedded in layered framework during repeated electrochemical cycling. Moreover, through analyzing the intermediate states of the structural transformation, this gradual structural evolution could explain the mechanism of the continuous development of the electrochemical activity below 3.5 V and over 4.25 V.</P> <P>Graphic Abstract</P><P>The original layered Li<SUB>1.2</SUB>Ni<SUB>0.2</SUB>Mn<SUB>0.6</SUB>O<SUB>2</SUB> evolves to a structure where the spinel-like region is locally embedded in the layered framework during cycling. <IMG SRC='http://pubs.rsc.org/services/images/RSCpubs.ePlatform.Service.FreeContent.ImageService.svc/ImageService/image/GA?id=c0jm01971b'> </P>
