관절경적 해면골 이식술 및 경피적 K-강선 고정을 이용한 주상골 불유합의 치료

목적: 주상골 불유합의 치료를 위하여 관절경적 골이식술 및 경피적 K-강선 고정을 시행한 환자를 대상으로 치료 결과를 분석하였다. 방법: 주상골 불유합으로 관절경적 골이식술 및 경피적 K-강선 고정술을 시행받은 환자 20명을 대상으로 하였다. 손상 후 불유합 수술까지의 기간은 평균 74개월(범위, 3-480개월)이었고, 기능적 결과는 수술 전과 최종 추시 시에 측정한 modified Mayo wrist score와 visual analogue scale (VAS)를 이용하여 평가하였다. 결과: 20명 모두에서 골유합을 얻을 수 있었고, 골유합까지 소요된 평균 시간은 9.7주(범위, 7-14주)였다. VAS score는 수술 전 평균 6.3점(범위, 4-8점)에서 최종 추시 시 평균 1.62점(범위, 0-3점)으로 향상되었으며, modified Mayowrist score는 수술 전 평균 62.5점에서 최종 추시 시 평균 85.7점으로 향상되었다. 결론: 주상골 불유합 치료에 대한 관절경적 골이식술 및 경피적 K-강선 고정술은 최소 침습적인 방법으로 보다 생물학적인 상태에서 불유합을 치료할 수 있는 기회를 제공해 주는 장점을 가진 좋은 치료 방법이라고 생각된다. Purpose: The purpose of this study was to analyze the clinical results of patientswith scaphoid nonunions treated with arthroscopically assisted bone graftingand percutaneous K-wires fixation. Methods: We retrospectively reviewed 20 patients with a scaphoid nonunionswhich was treated with arthroscopically assisted bone grafting and percutaneousK-wires fixation from November 2008 to July 2012. Time from injury totreatment was 74 months (range, 3-480 months) in average. Functional outcomewas evaluated using the modified Mayo wrist score and visual analoguescale (VAS) for pain, which were measured before operation and at the last followup. Results: All nonunions were healed successfully. The average radiologic uniontime was 9.7 weeks (range, 7-14 weeks). The average VAS score improved from6.3 (range, 4-8) preoperatively to 1.6 (range, 0-3) at the last follow up. The averagemodified Mayo wrist score increased from 62.5 preoperatively to 85.7 at thelast follow-up. Conclusion: Arthroscopically assisted bone grafting and percutaneous K-wiresfixation is an effective treatment method for a scaphoid nonunion. It may providemore biological environment than open surgery as a minimally invasiveprocedure.
