      • A Triple Squealer For Axial Flow Turbines

        In this paper a new blade tip shape, triple squealer, has been proposed. This shape is based on the conventional double squealer, and the cavity on the tip surface is divided into two parts by using a third squealer along the blade camber line. Four cases for the ratio of groove depth to span (GDS ratio): 0.75%, 1.5%,2.25%, and 3%, which correspond to 50%, 100%, 150%, and 200% of the tip clearance to span ratio, respectively, were taken up to investigate the effects of the GDS ratio on the flow field and losses. The flat tip case (baseline case) and the double squealer case were also calculated for comparison. The in-house, unstructured, 3D, Navier-Stokes, finite volume, multiblock code with DES (Detached Eddy Simulation) as turbulence model was used to calculate the flow field. It was found from calculated results that reduction in the mass flow rate of leakage flow in the case of a triple squealer with a GDS ratio of 1.5% is 8 times that for the double squealer case.

      • Jak-TGFβ cross-talk links transient adipose tissue inflammation to beige adipogenesis

        <P>The transient activation of inflammatory networks is required for adipose tissue remodeling including the 'browning' of white fat in response to stimuli such as. 3-adrenergic receptor activation. In this process, white adipose tissue acquires thermogenic characteristics through the recruitment of so-called beige adipocytes. We investigated the downstream signaling pathways impinging on adipocyte progenitors that promote de novo formation of adipocytes. We showed that the Jak family of kinases controlled TGF beta signaling in the adipose tissue microenvironment through Stat3 and thereby adipogenic commitment, a function that was required for beige adipocyte differentiation of murine and human progenitors. Jak/Stat3 inhibited TGF beta signaling to the transcription factors Srf and Smad3 by repressing local Tgfb3 and Tgfb1 expression before the core transcriptional adipogenic cascade was activated. This pathway cross-talk was triggered in stromal cells by ATGL-dependent adipocyte lipolysis and a transient wave of IL-6 family cytokines at the onset of adipose tissue remodeling induced by beta 3-adrenergic receptor stimulation. Our results provide insight into the activation of adipocyte progenitors and are relevant for the therapeutic targeting of adipose tissue inflammatory pathways.</P>

        Scoring systems for the management of oncological hepato-pancreato-biliary patients

        Oncological scoring systems in surgery are used as evidence-based decision aids to best support management through assessing prognosis, effectiveness and recurrence. Currently, the use of scoring systems in the hepato-pancreato-biliary (HPB) field is limited as concerns over precision and applicability prevent their widespread clinical implementation. The aim of this review was to discuss clinically useful oncological scoring systems for surgical management of HPB patients. A narrative review was conducted to appraise oncological HPB scoring systems. Original research articles of established and novel scoring systems were searched using Google Scholar, PubMed, Cochrane, and Ovid Medline. Selected models were determined by authors. This review discusses nine scoring systems in cancers of the liver (CLIP, BCLC, ALBI Grade, RETREAT, Fong’s score), pancreas (Genç’s score, mGPS), and biliary tract (TMHSS, MEGNA). Eight models used exclusively objective measurements to compute their scores while one used a mixture of both subjective and objective inputs. Seven models evaluated their scoring performance in external populations, with reported discriminatory c-statistic ranging from 0.58 to 0.82. Selection of model variables was most frequently determined using a combination of univariate and multivariate analysis. Calibration, another determinant of model accuracy, was poorly reported amongst nine scoring systems. A diverse range of HPB surgical scoring systems may facilitate evidence-based decisions on patient management and treatment. Future scoring systems need to be developed using heterogenous patient cohorts with improved stratification, with future trends integrating machine learning and genetics to improve outcome prediction.
