        How to Develop Future Internet Medical Care? : A Case Study of China

        Purpose – With the increasing medical demands of the public, the development of future Internet medical care has come to represent a major problem. Therefore, the purpose of this study is to discuss future development strategies for Interne medical care while taking China’s Internet hospitals as an example case. Research design, data, and methodology – This study conducted a case study of China’s Internet hospitals to summarize the fundamental problems faced by Internet hospitals and propose future development strategies to overcome these problems for Internet medical care. Result – Although Internet hospitals have been regarded as the ultimate product of Internet medical care, from the perspective of the government, medical institutions, platforms builders and maintainers, and patients, they still face some basic issues. Conclusion – This study concludes that the government and medical institutions play an important role in the future development of Internet medical care and suggests that the government should make overall plans for the policies and standards and should play the main role in enhancing the public trust in Internet medical care, while medical institutions should take steps such as seizing policy opportunities, driving online and offline collaborations, and constructing suitable evaluation systems to promote the development of Internet medical care.

        The influence of hand grip strength on surgical outcomes after surgery for degenerative lumbar spinal stenosis: a preliminary result

        <P><B>Abstract</B></P> <P><B>Background Context</B></P> <P>Although a number of prognostic factors have been demonstrated to be associated with surgical outcome of degenerative lumbar spinal stenosis (DLSS), no study has investigated the relation between hand grip strength (HGS) and treatment outcome of DLSS.</P> <P><B>Purpose</B></P> <P>The purpose of the present study was to examine the influence of HGS on surgical outcomes after surgery for patients with DLSS.</P> <P><B>Study Design</B></P> <P>This is an observational study.</P> <P><B>Patient Sample</B></P> <P>Patients who underwent spine surgery for DLSS were included in the study.</P> <P><B>Outcome Measures</B></P> <P>Oswestry Disability Index (ODI), EuroQOL (EQ-5D), and visual analog scale (VAS) scores for back or leg pain were assessed.</P> <P><B>Materials and Methods</B></P> <P>A total of 172 consecutive patients who underwent spine surgery for DLSS were included in the present study. Patients were assigned to either high HGS group (≥26 kg for men and ≥18 kg for women, n=124) or low HGS group (<26 kg for men and <18 kg for women, n=48) based on their preoperative HGS performance. Oswestry Disability Index, EQ-5D, and VAS scores for back and leg pain were assessed and compared between two groups preoperatively, 3 and 6 months after surgery. The primary outcome measure was baseline-adjusted ODI scores 6 months after surgery. The secondary outcome measures, including the overall ODI score, EQ-5D score, VAS score for back and leg pain, were assessed at each time point during the 6-month follow-up period.</P> <P><B>Results</B></P> <P>As primary outcome, baseline-adjusted ODI scores were significantly lower in the high HGS group than in the low HGS group 6 months after surgery. In the secondary outcome measurements, the ODI, EQ-5D, and VAS scores for back and leg pain improved significantly with time after surgery in both groups. The effects of HGS group on the overall changes in the ODI and EQ-5D scores during the 6-month period were significantly different between the two groups; however, they were not significantly different on VAS score for back and leg pain. The pattern of change in the ODI during the follow-up period was significantly different between the two groups.</P> <P><B>Conclusions</B></P> <P>Patients with preoperative high HGS display better surgical outcome in terms of disability and health status 6 months after spine surgery. Preoperative HGS can act as a predictor of surgical outcome in patients with DLSS.</P>

      • A Robust Adaptive Beamformer Based on Semidefinite Programming with Quadratic Constraints

        A robust beamforming with quadratic constraints, formulated as a semidefinite programming (SDP) problem, is proposed in this paper. With this formulation, the constraints on magnitude response can be easily imposed on the adaptive beamformer. And the non-convex quadratic constraints can be transformed into linear constraints. Therefore, the proposed method can be robust against the steering direction error. In practice, there are many array imperfections except steering direction error. In order to resist all kinds of array imperfections, the adaptive beamformer based on worst-case optimization technique is proposed by minimizing the array output power with respect to the worst-case array imperfections. Simulation results demonstrate that the proposed method is effective and can achieve a better performance.

        The Screening of Fermented Medicinal Herbs to Identify Those with Anti-inflammatory Properties

        Objectives : Consumption of fermented foods has been known to alleviate some of the symptoms of atopy and may limit allergy development, while there are also many medicinal herbs proved to be effective for immunologically-mediated diseases. In this study, we introduced modern zymology to ferment some herbs to see if fermentation has the possibility of increasing the anti-inflammatory effects of medicinal herbs. Interleukin-4 (IL-4) and interferon-gamma $(INF-\gamma)$ have been demonstrated to be the main factors in the pathology of allergic diseases. Methods : We measured the levels of IL-4 and $INF-\gamma$ on concanavalin A-induced BALB/c mice spleen cells, which were subsequently treated with fermented and unfermented herbs. We then compared the fermented groups with unfermented groups to see if the anti-inflammatory effects of the herbs were influenced by fermentation. Results and Conclusions : Our results showed that fermentation had the potential to increase the anti-inflammatory effects of some medicinal herbs, and Astragalus membranaceus and Salvia miltiorrhiza would be the most suitable medicinal herbs for fermentation among the herbs in this study.

        A remark on QF rings

        This article mainly concentrates on the open question wheth\-er a right self-injective ring $R$ is necessary QF if $R/S_l$ is left Goldie. It is answered affirmatively under the condition $S_l\subseteq S_r$, where $S_l$ and $S_r$ denote the left socle and right socle of $R$ respectively. And the original condition ``right self-injective" can be weakened to ``right CS and right P-injective". It is also proved that a semiperfect, left and right mininjective ring $R$ is QF if $S_r\subseteq^{ess}R_R$ and $R/S_l$ is left Goldie.

        Gas phase dehydration of glycerol catalyzed by rutile TiO_2-supported heteropolyacids

        Gas phase dehydration of glycerol catalyzed by the rutile TiO2-supported heteropolyacids was investigated. The TiO_2-supported heteropolyacid catalysts were prepared by the incipient wetness impregnation method using silicotungstic, phosphotungstic, and phosphomolybdic acids as active compounds. The as-prepared catalysts were characterized by X-ray diffraction, infrared spectroscopy,temperature programmed desorption of ammonia, and surface area measurement. The heteropolyacids supported by rutile TiO_2 were crystallites. The catalytic activity of the catalysts in the gas phase dehydration of glycerol was significantly affected by the type and loading of heteropolyacids. TiO_2-supported silicotungstic acid (20 wt.%) catalyst showed the highest catalytic activity with an acrolein selectivity of 80 mol% at a conversion of glycerol of 99% and a reaction temperature of 280 8C under ambient pressure. The possible reaction route in the gas phase dehydration of glycerol catalyzed by the TiO_2-supported heteropolyacid catalysts was also discussed briefly.

      • Feature fusion via hierarchical supervised local CCA for diagnosis of autism spectrum disorder

        <P>Early diagnosis of autism spectrum disorder (ASD) is critical for timely medical intervention, for improving patient quality of life, and for reducing the financial burden borne by the society. A key issue in neuroimaging-based ASD diagnosis is the identification of discriminating features and then fusing them to produce accurate diagnosis. In this paper, we propose a novel framework for fusing complementary and discriminating features from different imaging modalities. Specifically, we integrate the Fisher discriminant criterion and local correlation information into the canonical correlation analysis (CCA) framework, giving a new feature fusion method, called Supervised Local CCA (SL-CCA), which caters specifically to local and global multimodal features. To alleviate the neighborhood selection problem associated with SL-CCA, we further propose a hierarchical SL-CCA (HSL-CCA), by performing SL-CCA with the gradually varying neighborhood sizes. Extensive experiments on the multimodal ABIDE database show that the proposed method achieves superior performance. In addition, based on feature weight analysis, we found that only a few specific brain regions play active roles in ASD diagnosis. These brain regions include the putamen, precuneus, and orbitofrontal cortex, which are highly associated with human emotional modulation and memory formation. These finding are consistent with the behavioral phenotype of ASD.</P>

        Iron interferes with quorum sensing-mediated cooperation in Pseudomonas aeruginosa by affecting the expression of ppyR and mexT, in addition to rhlR

        The stabilization of quorum sensing (QS) is vital for bacterial survival in various environments. Although the mechanisms of QS stabilization in certain conditions have been well studied, the impact of environmental factors has received much less attention. In this study, we show that the supplementation of 25 μM iron in competition experiments and 50 μM in evolution experiments to casein growth cultures significantly increased the possibility of population collapse by affecting elastase production. However, the expression of lasI and lasR remained constant regardless of iron concentration and hence this effect was not through interference with the LasIR circuit, which mainly regulates the secretion of elastase in Pseudomonas aeruginosa. However, the expression of rhlR was significantly inhibited by iron treatment, which could affect the production of elastase. Further, based on both reverse transcription quantitative polymerase chain reaction and gene knock-out assays, we show that iron inhibits the transcription of ppyR and enhances the expression of mexT, both of which decrease elastase production and correspondingly interfere with QS stabilization. Our findings show that environmental factors can affect the genes of QS circuits, interfering with QS stabilization. These findings are not only beneficial in understanding the mechanistic effect of iron on QS stabilization, but also demonstrate the complexity of QS stabilization by linking non-QS-related genes with QS traits.

        Low-dose diethylhexyl phthalate exposure does not impair the expressive patterns of epigenetics-related genes and DNA methylation of breast cancer-related genes in mouse mammary glands

        Backgrounds: Di-(2-ethylhexyl) phthalate (DEHP), as an endocrine-disrupting chemical (EDC), is widely used in plasticizer and other productions. Ubiquitous human exposure to DEHP has been proposed to be a potential risk to public health. Developmental exposure to DEHP could alter epigenetic programming and result in adult-onset disease. Methods: In this study, we investigated whether DEHP exposure to pregnant mice affected epigenetic changes as a result of increase in breast cancer incidence. Results: Our results showed that the expression of total 143 epigenetics-related genes in mammary gland cells, have no significantly altered after short time and low-dose treated with DEHP from 0.5 days post-coitum (dpc) to 3.5 dpc of pregnant mice. DNA methylation status of some neoplastic development genes, such as EGFr, Esr1, Pgr, Fos and Rassf5 also had no obvious change. Conclusion: These finding showed no impact of DEHP on the expressive patterns of epigenetics-related genes and DNA methylation of breast cancer-related genes in pregnant mouse mammary gland cells.
