        Extravascular lung water index, pulmonary vascular permeability index, and global end-diastolic volume index in mechanically ventilated COVID-19 patients requiring prone position ventilation: a preliminary retrospective study

        Background: There is a lack of data on extravascular lung water index (EVLWi), pulmonary vascular permeability index (PVPi), and global end-diastolic volume index (GEDVi) during prone position ventilation (PPV) in coronavirus disease 2019 (COVID-19) patients. The objectives of this study were to analyze trends in EVLWi, PVPi, and GEDVi during PPV and the relationships between these parameters and PaO2/FiO2.Methods: In this preliminary retrospective observational study, we performed transpulmonary thermodilution (TPTD) in seven mechanically ventilated COVID-19 patients without cardiac and pulmonary comorbidities requiring PPV for 18 hours, at specific times (30 minutes pre-PPV, 18 hours after PPV, and 3 hours after supination). EVLWi, PVPi and GEDVi were measured. The relationships between PaO2/FiO2 and EVLWi, and PVPi and GEDVi values, in the supine position were analyzed by linear regression. Correlation and determination coefficients were calculated.Results: EVLWi was significantly different between three time points (analysis of variance, P=0.004). After 18 hours in PPV, EVLWi was lower compared with values before PPV (12.70.9 ml kg–1 vs. 15.31.5 ml kg–1, P=0.002). Linear regression showed that only EVLWi was correlated with PaO2/FiO2 (β =–5.757; 95% confidence interval, –10.835 to –0.679; r=–0.58; R2=0.34; F-test P=0.029).Conclusions: EVLWi was significantly reduced after 18 hours in PPV and values measured in supine positions were correlated with PaO2/FiO2. This relationship can help clinicians discriminate whether deterioration in gas exchange is related to fluid overload or disease progression. Further clinical research should evaluate the role of TPTD parameters as markers to stratify disease severity and guide clinical management.

        Numerical investigations on the turbulence driven responses of a plate in the subcritical frequency range

        Some numerical investigations are presented concerning the response of a given plate under turbulence driven excitations. Three different input loads are simulated according to the wall pressure distributions derived from the models proposed by Corcos, Efimtsov and Chase, respectively. Modal solutions (finite element based) are used for building the modal stochastic responses in the sub-critical aerodynamic frequency range. The parametric investigations concern two different values of the structural damping and three values of the boundary layer thickness. A final comparison with available experimental data is also discussed. The results demonstrate that the selection of the adequate TBL input model is still the most critical step in order to get a good prediction.

        The random structural response due to a turbulent boundary layer excitation

        In this paper, the structural random response due to the turbulent boundary layer excitation is investigated. Using the mode shapes and natural frequencies of an undamped structural operator, a fully analytical model has been assembled. The auto and cross-spectral densities of kinematic quantities are so determined through exact analytical expansions. In order to reduce the computational costs associated with the needed number of modes, it has been tested an innovative methodology based on a scaling procedure. In fact, by using a reduced spatial domain and defining accordingly an augmented artificial damping, it is possible to get the same energy response with reduced computational costs. The item to be checked was the power spectral density of the displacement response for a flexural simply supported beam; the very simple structure was selected just to highlight the main characteristics of the technique. In principle, it can be applied successfully to any quantity derived from the modal operators. The criterion and the rule of scaling the domain are also presented, investigated and discussed. The obtained results are encouraging and they allow thinking successfully to the definition of procedure that could represent a bridge between modal and energy methods.

        Eurasian Extinction: Miscegenation and the Question of the Native in Macau Literature

        For the multiethnic city of Macau, the era of political transition from Portuguese to Chinese rule (1987–1999) placed renewed importance on definitions of belonging and nativity. This paper argues that a narrative of Eurasian extinction has been a crucial mode in which to articulate Sinitic belonging in the literary space of pre-postcolonial Macau. Tracing the figure of the Macanese Eurasian protagonists in the work of Sinophone authors Li Guanding and Liao Zixin, I read these narratives as being characterized by the troubled presence of the non-procreative mestiço who threatens to condemn the entire Eurasian community to extinction. The looming theme of Eurasian demise is not to be understood as a lament over the city’s lost colonial era but, I argue, should be read as an anxious reflection on the difficulties of Sinitic native access in a city soon to be subsumed by a hegemonizing and homogenizing Chinese nation.

      • Analytical similitudes applied to thin cylindrical shells

        This work is focused on the definition and the analysis of both complete and incomplete similitudes for the dynamic responses of thin shells. Previous numerical and experimental investigations on both structural and structural-acoustic systems motivated this further analysis, mainly centred on the incomplete (distorted) similitudes. These similitudes and the associated scaling laws are defined by using the classical modal approach (CMA) and by invoking also the Energy Distribution Approach (EDA) in order to take into account both the cinematic and energetic items. The whole procedure is named SAMSARA: Similitude and Asymptotic Models for Structural-Acoustic Research and Applications. A brief summary of the procedure is herein given and the attention is paid to the analytical models of thin stiffened and unstiffened cylindrical shells. By using the well-known smeared model, the stiffened cylinder equations are used as general framework to analyse the possibility to define exact (replicas) or distorted similitudes (avatars). Despite the extreme simplicity of the proposed models, the results are really encouraging. The final aim is to define equivalent models to be used in laboratory measurements.

      • Identification and Characterization of a Small-Molecule Inhibitor of Wnt Signaling in Glioblastoma Cells

        <P>Glioblastoma multiforme (GBM) is the most common and prognostically unfavorable form of brain tumor. The aggressive and highly invasive phenotype of these tumors makes them among the most anatomically damaging human cancers with a median survival of less than 1 year. Although canonical Wnt pathway activation in cancers has been historically linked to the presence of mutations involving key components of the pathway (APC, β-catenin, or Axin proteins), an increasing number of studies suggest that elevated Wnt signaling in GBM is initiated by several alternative mechanisms that are involved in different steps of the disease. Therefore, inhibition of Wnt signaling may represent a therapeutically relevant approach for GBM treatment. After the selection of a GBM cell model responsive to Wnt inhibition, we set out to develop a screening approach for the identification of compounds capable of modulating canonical Wnt signaling and associated proliferative responses in GBM cells. Here, we show that the small molecule SEN461 inhibits the canonical Wnt signaling pathway in GBM cells, with relevant effects at both molecular and phenotypic levels <I>in vitro</I> and <I>in vivo</I>. These include SEN461-induced Axin stabilization, increased β-catenin phosphorylation/degradation, and inhibition of anchorage-independent growth of human GBM cell lines and patient-derived primary tumor cells <I>in vitro</I>. Moreover, <I>in vivo</I> administration of SEN461 antagonized Wnt signaling in <I>Xenopus</I> embryos and reduced tumor growth in a GBM xenograft model.       • The Sustainable Development Goals And The Dangers of Policy Reductionism

        On September 25, 2016, world leaders participated in the United Nations Development Summit to forge the 2030 Agenda for Sustainable Development. The Summit adopted 17 sustainable development goals and 169 targets that are envisioned "to end poverty, fight inequality and injustice, and tackle climate change by 2030." The Sustainble Development Goals (SDGs) perceived to build upon the gains of the eight Millennium Development Goals (MDGs) targets launched in 2000 and expired in 2015. This paper provides a cursory assessment and critique of the adoption of the newly-minted SDGs which, as it is, are conceived to address the vexing and complex problematic of poverty, inequalities, health and well-being, and climate change, and several anomalies and issues that are projected to confront nation states and the world order in the coming decades. The paper examines these within the context and framework of the dangers of policy reductionism, which, in philosophical terms, represent a theory, or practice of simplifying a complex phenomenon, idea, or condition to the point of miniming, obscuring or even distorting it.

        Single coronary artery originating from the right sinus Valsalva and ability to work

        We present a case of a 56-year-old male electrician who was admitted to the hospital with atrial fibrillation, atypical chest pain and dyspnea. He gave a history that on the morning he had working for almost 4 hours carrying out various activities with considerable physical effort. After cardioversion, conventional coronary angiography revealed a suspect of single coronary vessel (SCA) arising from the right sinus of Valsalva. The patient underwent multislice computed tomography that showed a SCA arising from the right sinus Valsalva and dividing in Right Coronary Artery (RCA) and Left Main coronary artery (LM). The finding of posterior course of the LM without atherosclerotic has proved crucial for the expression of an opinion of working capacity even with limitation.

        Effect of Systemic and Auricular Acupuncture with a 2/100 Hz Frequency and Nogier Frequency in Fibromyalgia: a Randomized Clinical Trial, Pilot Study

        Background: Fibromyalgia is a syndrome of chronic, generalized muscular pain, accompanied by sleep disturbances, fatigue and cardic autonomic dysfunction that will affect the quality of life. There is currently no gold standard treatment. There are limitations of studies with electroacupuncture in auricular acupuncture. Objectives: We evaluate the effects of systemic electroacupuncture (EA) with frequencies of 2/100 Hz associated of auricular acupuncture with a Nogier frequency (2.28, 4.56 and 9.12 Hz) for pain intensity, heart rate variability (HRV), and quality of life in fibromyalgia. Methods: Randomized clinical trial, a pilot study. Eighteen volunteers were randomized into a control group (CG, n = 9) and an experimental group (EG, n = 9). Six systemic EA sessions systemic and auricular were applied in the EG for 20 min, twice a week, for six weeks consecutive. The Numerical Pain Assessment Scale (NPRS), 2010 diagnostic criteria of the American College of Rheumatology (FDC 2010), Fibromyalgia Impact Questionnaire (FIQ) and analysis of HRV were the instruments used. The independent t-test compared to the groups was applied. Results: There was no statistically significant difference for the primary outcome for NPRS (p > 0.05). In the secondary outcome there was a significant difference in the total score and in some FIQ domains (p = 0.008) and some variables such as pain (p = 0.02) and anxiety (p = 0.006). There was no significant difference for the FDC 2010 and HRV variables (p > 0.05). Conclusion: 2/100 Hz systemic EA associated with the Nogier frequency positively influenced some quality of life variables; however, pain intensity, diagnostic criteria, and HRV variables did not change.
