Background Free muscle flaps are a mainstay for reconstruction of distal third leg wounds and for large lower extremity wounds with exposed bone. However a major problem is the significant postoperative flap swelling, which may take months to resolve. We studied the efficacy and safety of immediate application of a vacuum assisted closure (VAC) dressing after a free muscle flap to the lower extremity. Methods Over a 19 months period, all consecutive free muscle flaps for lower extremity reconstruction at a Level I trauma center were evaluated prospectively for postoperative flap thickness, complications and flap survival. Immediate application of a VAC dressing was performed in 9 patients, while the flap was left exposed for monitoring in 8 patients. Results There was no statistically significant difference in flap survival between both cohorts. Mean flap thickness at postoperative day 5 for the VAC group was $6.4{\pm}6.4mm$, while flap thickness for the exposed flap group was $29.6{\pm}13.5mm$. Flap thickness was significantly decreased at postoperative day 5 for the VAC dressing group. Conclusions Immediate application of VAC dressing following free muscle flaps to the lower extremity does not compromise flap survival or outcomes and results in decreased flap thickness and a better aesthetic outcome.
SLC35B2 Expression is Associated with a Poor Prognosis of Invasive Ductal Breast Carcinoma

Background: Breast cancer is the most common malignancy in women worldwide, including Thailand, and is a major cause of mortality and morbidity, despite advances in diagnosis and treatment. Novel gene expression in breast cancer is a focus in searches for prognostic biomarkers and new therapeutic targets. Materials and Methods: The mRNA expression of novel B4GALT4, SLC35B2, and WDHD1 genes in breast cancer were examined in invasive ductal breast carcinoma (IDC) patients using quantitative real-time reverse transcription polymerase chain reaction (QRT-PCR). Results: Among these genes, increased expression of SLC35B2 mRNA was significantly associated with TNM stage III + IV of IDC (p<0.001). Hence, up-regulation of SLC35B2 may serve as a prognostic biomarker for poor prognosis, and is also a potential therapeutic target in breast cancer.

The purpose of this essay is to show the reason that how piano teacher Miss Miriam Cornell in Ian McEwan’s Lessons could turn her 11-year-old male pupil Roland willingly into a prisoner who wore pajama and gave up further university education for immersing in her sexual abuse, moreover adult Roland even refused to accuse her after going through a failed marriage and volatile times. Based on Roland’s childhood, and the relationships between him and his parents, this study mainly focuses on the importance of parental love in the formation of Roland’s attitude towards Miriam Cornell and how the piano teacher replace the position of his parents gradually. She with the authority of teacher was more like Roland’s soldier father and the embraces were more like his mother who was left by her husband. Moreover, Roland killed his father mentally as he had Lawrence (his only son with Alissa) and learnt how to be a father step by step.

The effects of stirring speed during filler modification by dual polymers on flocculation and reflocculation of PCC (precipitated calcium carbonate) particles and its effect on handsheet properties were elucidated. PCC surface was modified by adsorbing A-PAM (anionic polyacrylamide) and C-starch (cationic starch) in series at various stirring speeds. It was found that increasing stirring speed during filler modification decreased the initial floc size of PCC. Continuous stirring with the same speed for filler modification resulted in the decrease of a floc size, eventually reached a steady state. The variations in a floc size was influenced by the stirring speed during filler modification: the lower the stirring speed during filler modification, the larger the floc size variations. Conclusively, the stability of PCC floc could be improved by increasing the stirring speed. In addition, the stirring speed influenced the handsheet properties. The smaller the PCC floc, the lower the strength of handseet. However, too much larger floc size also deteriorated paper strength. There exists an optimum floc size in term of paper strength which shall be controlled by stirring speed during filler modification.

Factors influencing penetration of PVAm solution into paper during impregnation were investigated with ultrasonic Penetration Evenness Analyzer (PEA). Paper structure was varied by changing basis weight, freeness of pulp, calendering, and filler addition, and hydrophobicity of paper was varied by adding AKD. In addition, the viscosity of PVAm solution was varied by changing the concentration of PVAm solution. Important factors influencing penetration of PVAm solution into paper were found to be the pore structure and the hydrophobicity of paper, and the viscosity of PVAm solution. Pore structure of paper could be controlled by refining degree and filler addition and hydrophobicity of paper could be controlled by internal sizing. Denser structure of paper, higher hydrophobicity and higher liquid viscosity slowed down the penetration of PVAm solution into paper.
