      • Video Evaluation of Self-Regulation Skills in Preschool Aged Children with Developmental Delays: Can the impact of Tumbling TogetherTM be Measured?

        Objectives: This study evaluated the feasibility of video recording to evaluate an innovative, multi-disciplinary exercise medicine intervention, called Tumbling TogetherTM, on the self-regulation skills required for successful group participation. Methods: A photographer, who did not interact with participants and maintained a consistent position outside of the program area, used a digital video camera to record the program activities. Response to a stop command (attempts after the signal), ability to wait for a turn (yes/no), and ability to line up (correct/incorrect position) were the indicators of self-regulation. Video recordings were analyzed to determine each child’s baseline (first two sessions) and post-program (last two sessions) abilities. Chi-square and t-tests identified differences in performance from beginning to end of the program. Results: Video recordings were analyzed for 27 children (3 (11%) female, 3.1 to 4.8 years of age) with complex communication/developmental challenges. Response to a stop command improved by 38% (Pre: 2.1 ± 0.9; Post: 1.3 ± 0.4; p<.001). Correct response to a line up signal improved from 26% to 89% (p<.001). The ability to wait for a turn when required increased from 7% to 80% (p<.001). Conclusions: Video recordings suitable for analysis were obtained during all sessions. Children’s skills performed during the first two classes were similar. Their skills improved between the start and end of the program. Video recordings accurately reflected the improved performance subjectively reported by therapists and parents. Therefore, video recording appears to be an appropriate method of evaluating the impact of exercise medicine interventions, such as Tumbling TogetherTM, that are designed to enhance the self-regulation skills of preschool children. Research comparing Tumbling participants to children not offered the program, and examining whether the self-regulation skills improved during Tumbling participation would generalize to other settings is required.

        Cold-formed steel channel columns optimization with simulated annealing method

        Cold-formed profiles have been largely used in the building industry because they can be easily produced and because they allow for a wide range of sections and thus can be utilized to meet different project requirements. Attainment of maximum performance by structural elements with low use of material is a challenge for engineering projects. This paper presents a numerical study aimed at minimizing the weight of lipped and unlipped cold-formed channel columns, following the AISI 2007 specification. Flexural, torsional and torsional-flexural buckling of columns was considered as constraints. The simulated annealing method was used for optimization. Several numerical simulations are presented and discussed to validate the proposal, in addition to an experimental example that qualifies its implementation. The ratios between lips, web width, and flange width are analyzed. Finally, it may be concluded that the optimization process yields excellent results in terms of cross-sectional area reduction.

        The role of geophysics in understanding salinisation in Southwestern Queensland

        This study, combining geophysical and environmental approaches, was undertaken to investigate the causes of secondary salinity in the Goondoola basin, in southwestern Queensland. Airborne radiometric, electromagnetic and ground electromagnetic datasets were acquired, along with data on soils and subsurface materials and groundwater. Relationships established between radiometric, elevation data, and measured material properties allowed us to generate predictive maps of surface materials and recharge potential. Greatest recharge to the groundwater is predicted to occur on the weathered bedrock rises surrounding the basin. Electromagnetic data (airborne, ground, and downhote), used in conjunction with soil and drillhole measurements, were used to quantify regolith salt store and to define the subsurface architecture. Conductivity measurements reflect soil salt distribution. However, deeper in the regolith, where the salt content is relatively constant, the AEM signal is influenced by changes in porosity or material type. This allowed the lateral distribution of bedrock weathering zones to be mapped. Salinisation in this area occurs because of local-andintermediate-scale processes, controlled strongly by regolith architecture. The present surface outbreak is the result of evaporative concentration above shallow saline groundwater, discharging at break of slope. The integration of surficial and subsurface datasets allowed the identification of similar landscape settings that are most at risk of developing salinity with groundwater rise. This information is now being used by local land managers to refine management choices that prevent excess recharge and further salt mobilisation.

        Follow-up of Metal-on-Metal Hip Replacements at a Large District Hospital and the Implementation of Medicines and Healthcare Products Regulatory Agency Guidelines: A Review of 297 Patients

        Background: Medicines and Healthcare products Regulatory Agency (MHRA) guidance for patients with metal-on-metal (MoM) hip replacements was provided in 2012 and updated in 2017 to assist in the early detection of soft-tissue reactions due to metal wear debris. A large number of MoM hip replacements were undertaken at our hospital trust. A program of recall for all patients with MoM hip replacements was undertaken and MHRA guidelines were implemented. In this study, we aimed to investigate the effectiveness of the revised MHRA guidelines in the detection of early adverse reactions to metal debris and to re-evaluate the indications for metal artifact reduction sequence magnetic resonance imaging (MARS-MRI) and revision surgery. Methods: Identification and recall of all patients with MoM hip replacements from 2001 were conducted by using theatre logs, patient records, clinical coding information, and consultant logbooks. Two senior arthroplasty consultants reviewed X-rays and patient records. Postal questionnaires were forwarded to patients, together with requests for general practitioners to complete cobalt and chromium blood tests. The two consultant-led review of MOM replacements was undertaken with further radiological investigations (X-rays, MARS-MRI) performed according to the 2017 guidance with support of consultant radiologists. Results: Of 674 identified patients, 297 were available for review: 26 patients did not have MoM implants, 36 were untraceable, 59 refused follow-up, 87 moved out of area, 147 had died, and 22 already had revision. Of 297 patients, 126 were women and 171 were men; age range was 39 to 95 years (mean age, 69 years); 126 had resurfacing and 171 had MoM replacements. Twenty-six patients had elevated metal ions. Thirty-three patients underwent MARS-MRI: MARS-MRI results were positive in 17 and negative in 16. Of 17 patients with positive MARS-MRI, 10 patients were asymptomatic and seven were waiting revision. Conclusions: Positive MARS-MRI can often occur in the absence of elevated metal ion levels; elevated blood metal ion levels do not mean MARS-MRI will be positive. All patients with MoM replacements were at risk. It is imperative to assess patients regularly for symptoms that may raise clinical suspicion and maintain a low threshold to performing MARS-MRI.

        A Pilot Examination of Oxidative Stress in Trichotillomania

        Objective Trichotillomania is a relatively common illness whose neurobiology is poorly understood. One treatment for adult trichotillomania, n-acetyl cysteine (NAC), has antioxidative properties, as well as effects on central glutamatergic transmission. Preclinical models suggest that excessive oxidative stress may be involved in its pathophysiology. Methods Adults with trichotillomania provided a blood sample for analysis of compounds that may be influenced by oxidative stress [glutathione, angiotensin II, ferritin, iron, glucose, insulin and insulin growth factor 1 (IGF1), and hepcidin]. Participants were examined on symptom severity, disability, and impulsivity. The number of participants with out-of-reference range oxidative stress measures were compared against the null distribution. Correlations between oxidative stress markers and clinical measures were examined. Results Of 14 participants (mean age 31.2 years; 92.9% female), 35.7% (n=5) had total glutathione levels below the reference range (p= 0.041). Other oxidative stress measures did not have significant proportions outside the reference ranges. Lower levels of glutathione correlated significantly with higher motor impulsiveness (Barratt Impulsiveness Scale sub-score) (r=0.97, p=0.001). Conclusion A third of patients with trichotillomania had low levels of glutathione, and lower levels of glutathione correlated significantly with higher motor impulsiveness. Because NAC is a precursor for cysteine, and cysteine is a rate limiting step for glutathione production, these results may shed light on the mechanisms through which NAC can have beneficial effects for impulsive symptoms. Confirmation of these results requires a suitable larger follow-up study, including an internal normative control group.

        Optimization of steel-concrete composite beams considering cost and environmental impact

        In the optimized structure sizing, the optimization methods are inserted in this context in order to obtain satisfactory solutions, which can provide more economical structures, besides allowing the consideration of the factors related to the environmental impacts in the structural design. This work proposes a mathematical model for the optimization of steel-concrete composite beams aiming to minimize the monetary cost and the environmental impact, using the Harmonic Search optimization method. Discrete variables were the dimensions of the steel profiles and the thickness of the collaborating slab of the composite steel-concrete beam. The proposed model was implemented in Fortran programming language and based on improvements in the structure of the optimization method proposed by Medeiros and Kripka (2017). To prove the effectiveness and applicability of the model, as well as the Harmonic Search method, analyzes were performed with different configurations of steel-concrete composite beams, in order to provide guidelines that make the use of these systems more streamlined. In general, the Harmonic Search optimization method has proved to be efficient in the search for the optimized solutions, as well as important considerations on the optimization of the monetary and environmental costs of steel-concrete composite beams were obtained from the developed examples.
