Background: Regional anesthesia techniques constitute an important part of successful analgesia strategies in the perioperative care of patients undergoing breast surgery. The advent of ultrasound-guided regional anesthesia has led to the development of fascial plane blocks. The large array of blocks available for postoperative analgesia in breast surgery has increased the accessibility of regional anesthesia but has also created a dilemma of choice. This study compared the analgesic efficacy of the ultrasound-guided modified pectoral nerve (PECS) block and erector spinae plane block (ESPB) in patients undergoing radical mastectomy.Methods: Seventy women were enrolled in this prospective, double-blind, randomized control trial. After exclusion, 67 female patients who underwent radical mastectomy were finally analyzed. Ultrasound-guided PECS blocks and ESPBs were performed with 30 ml 0.25% bupivacaine. Postoperative morphine and pain scores were compared between the groups.Results: Postoperative total morphine consumption in the first 24 h was significantly higher in the PECS group (P < 0.001). The ESPB group exhibited significantly reduced morphine consumption at all postoperative time points. Numeric rating scale scores were lower in the ESPB group at 6, 12, and 24 h postoperatively at rest and when coughing.Conclusions: Ultrasound-guided bi-level ESPBs provided better postoperative analgesia than PECS blocks after radical mastectomy surgery.

The use of biodiesel-blended fuels in diesel engines improves the engine performance parameters and the partial recovery of incomplete combustion products, while also increasing the level of NOx emissions. In this study; biodiesel obtained through the transesterification of waste chicken frying oil was mixed with diesel fuel (90% diesel + 10% biodiesel-B10), and was then used as fuel in a direct injection diesel engine. To reduce the increased NOx emissions caused by the use of B10 fuel, the steam injection method (which is one of the NOx reduction methods) was applied. Steam was injected into the intake manifold at different ratios (5%-S5, 10%-S10 and 15%-S15) and at the time of the induction stroke with the aid of an electronically controlled system. Based on the study results, it was observed that steam injection into the engine using B10 fuel improved both the engine performance and the exhaust emission parameters. It was determined that the S15 steam injection ratio resulted in the best engine performance and emissions parameters. In comparison to STD fuel;the highest increase observed at the S15 steam injection ratio in the effective engine power was 2.2%, while the highest decrease in the specific fuel consumption was 3.4%, the highest increase in the effective efficiency was 3.5%, and the highest decrease in NOx emissions was 13.7%.

Aneurysmal bone cysts (ABCs) are benign and rapidly expanding bone destructive lesions of any bone. They are commonly localized in the metaphysis of long bones, whereas skull base ABCs are rare. We report a case of a 21-year-old man who had been misdiagnosed as chordoma and undergone surgery. However, histopathological examination revealed it to be an ABC.
A deep learning approach to permanent tooth germ detection on pediatric panoramic radiographs

Purpose: The aim of this study was to assess the performance of a deep learning system for permanent tooth germ detection on pediatric panoramic radiographs. Materials and Methods: In total, 4518 anonymized panoramic radiographs of children between 5 and 13 years of age were collected. YOLOv4, a convolutional neural network (CNN)-based object detection model, was used to automatically detect permanent tooth germs. Panoramic images of children processed in LabelImg were trained and tested in the YOLOv4 algorithm. True-positive, false-positive, and false-negative rates were calculated. A confusion matrix was used to evaluate the performance of the model. Results: The YOLOv4 model, which detected permanent tooth germs on pediatric panoramic radiographs, provided an average precision value of 94.16% and an F1 value of 0.90, indicating a high level of significance. The average YOLOv4 inference time was 90 ms. Conclusion: The detection of permanent tooth germs on pediatric panoramic X-rays using a deep learning-based approach may facilitate the early diagnosis of tooth deficiency or supernumerary teeth and help dental practitioners find more accurate treatment options while saving time and effort

Fibroadenoma is the most common breast tumor in women. Malignant transformation occurs rarely within fibroadenoma atolder ages. Clinicians, radiologists, and pathologists need to beaware of malignant transformation within fibroadenomas. Radiologicstudies play an important role in the diagnosis of fibroadenoma;however, radiologic findings are often nonspecific formalignancy and may appear completely benign. We detected anoccult ductal carcinoma in situ that originated inside a fibroadenomaby using shear wave elastography. We report shearwave elastography findings of ductal carcinoma in situ withinfibroadenoma and discuss the diagnostic role of this modality.

Aims: Chronic hepatitis B (CHB) and C (CHC) are leading causes of liver cirrhosis and hepatocellular cancer. Defining of inflammatory activity and severity of disease is important for commencing antiviral treatment and monitoring the patient. Currently noninvasive serological markers to define disease activity are of great interest. In this study we aimed to determine soluble urokinase plasminogen activator receptor (suPAR) levels of patients with CHB and CHC, compare its levels with those of controls and to find if suPAR levels correlated with some laboratory parameters, histologic activity index (HAI) and fibrosis stage. Methods: Twenty-eight patients with CHB, 11 patients with CHC and 41 healthy subjects, eighty subjects in total, were included in the study. suPAR levels of all subjects were determined using commercial ELISA kits according to manufacturer’s instructions. Correlation between suPAR levels and parameters including AST, ALT, leukocyte count, thrombocyte count and liver histology scores were statistically analyzed. Results: Fifty percent of CHB patients were on oral antiviral treatment. Serum suPAR levels of both CHB and CHC patients were higher than controls (p< 0,001). Age, suPAR, HAI, AST, ALT, leukocyte and thrombocyte counts of subjects in different groups (CHB, CHC patients and controls) were shown in table 1. When we divided patients into two groups according to HBV DNA, suPAR levels were not different between patients with HBV DNA >1000 IU/ml and those with HBV DNA<1000 IU/ml. suPAR levels were not found to be correlated with AST, ALT levels, thrombocyte and leukocyte counts or histologic findings. Conclusions: In both CHB and CHC patients, suPAR levels were determined to be higher than healthy controls although suPAR was not found to be correlated with diseae activity or severity related parameters. However new wide scale studies are needed to investigate the relation between suPAR and disease activity or fibrosis stage.
