        Trans-sphenoidal approach에 의한 pituitary microadenoma 수술 후의 임신 1례

        We treated a patient with pituitary microadenoma having hyperprolactinemia and irregular menstruation infertility by transsphenoidal microsurgery. Her menstrual cycle become irregular after marriage but previous menstrual cycle was regular. When she visited our hospital due to infertility, initial serum prolactine level was slightly elevated 35.68ng/ml, During bromecriptine therapy, abrupt increase of serum prolactine level up to 126.52 ng/ml and headache and facial flush and galactorrhea developed. Pituitary microadenoma was confirmed by CT and MRI. The patient underwent transsphenoidal microsurgery. Patient was pregnant after surgery, and it was confirmed by USG and she is following up, OPD now.

        The present condition of science education has been investigated b means of questionaires, interviews and referring to literature, and the points at issue gave been extracted on the basis of them, and in consideration of the problems various materials for experiment and practice have been developed to be used for the effective inquiry learning. The present condition and problems of science education in junior high schools are as follows; 1) Science teacher's backgrounds for their teaching profession and majors are good, but they are overburdened and the classes are overcrowded. 2) The textbooks now in use contain too much contents, the levels are high, and the books lack in appropriateness. 3) Especially the contents Presented for experimental activities require too many tasks to be digested in a given time. 4) Experimental facilities or instruments are unsatisfatory in their quantity or quality, and instruments or teaching materials that can be used effectively are far from being sufficient. Futhermore, some experimental instruments are too crude to work successfully. 5) Students' elementary knowledge is not sufficient, and the degrees of their interest or understanding are low. 6) With the teaching methods still conservative and the teachers unable to give satisfactory inquiry leaning. the students are disencouraged. 7) Support of experimental facilities, encouragement of teachers, appropriate school inspection, and administrative or financial aids are desired. In this study, materials for every branch of science have been developed to be available for the effective performance of experiments and practices through inquiry. The materials developed in this study are as follows. 1. The following materials are presented in physics: the relations between the heat quantity of water and the temperature change, and between the heat quantity of the electric heat line and the electric current, for group experiments; the observation of Brown motion, Law of Archimedes, altimeters, comparing the heat capacity of metals, the transmission of radiation, the frictional electricity, the interaction of electric current with magnetic needle, and Fleming's right-hand rule, as the reference materials of demonstration; a program learning materials for diffusion of light with experimental kit utilized in learning the properties of light; a program learning material on the diffraction of light with an experimental kit utilized in learning the diffraction and interference of light. 2. In chenistry, the experimental materials for electrolysis are presented as the teaching-learning material of group experiment. Besides, the errors in the experimental materials and inappropriate expressions in the textbooks now in use are corrected and completed: 3. In biology, problems have been extracted by means of questionaires about the general learning contents and the analisis of Japanese textbooks, and the materials for experiment have been developed. The observation of somatic cell division, that of mold, and that of prothallium are presented as the materials for group experiment. Inquiry learning based on experiment and practice should be settled on the scenes of learning, and the suggestions are made to the effect as follows. 1) The quantity of learning should be reduced, and the levels lowered in the new textbooks. 2) The development and distribution of individual learning materials for the master learning are desirable. 3) The items for group experiment should be reduced to the minimum, and the thorough inquiry learning should be pursued, and the greater part should be replaced by the demonstrative experiments or audio-visual materials. 4) The experimental instruments should be manufactured and distributed on the national scale to ensure their solidity and accuracy. 5) The atmosphere in which inquiry learning is performed by the teacher's creative efforts is greatly needed, To support this conditioning, exhibitions of scientific works should be held continually, science museums operated, the research centers of science education cooperated, new materials for practice developed and studied, and the corporations for research of science education fostered. 6) The measures to lighten the teachers' burden and to encourage them should be contrived.

      • Analysis of Cancer Incidence in Zhejiang Cancer Registry in China during 2000 to 2009

        Objective: The Zhejiang Provincial Cancer Prevention and Control Office collected cancer registration data during 2000 to 2009 from 6 cancer registries in Zhejiang province of China in order to analyze the cancer incidence. Methods: Descriptive analysis included cancer incidence stratified by sex, age and cancer site group. The proportions and cumulative rates of 10 common cancers in different groups were also calculated. Chinese population census in 1982 and Segi's population were used for calculating age-standardized incidence rates. The log-linear model was used for fitting to calculate the incidence trends. Results: The 6 cancer registries in Zhejiang province in China covered a total of 60,087,888 person-years during 2000 to 2009 (males 30,445,904, females 29,641,984). The total number of new cancer cases were 163,104 (males 92,982, females 70,122). The morphology verified cases accounted for 69.7%, and the new cases verified only by information from death certification accounted for 1.23%. The crude incidence rate in Zhejiang cancer registration areas was $271.5/10^5$ during 2000 to 2009 (male $305.41/10^5$, female $236.58/10^5$), age-standardized incidence rates by Chinese standard population (ASIRC) and by world standard population (ASIRW) were $147.1/10^5$ and $188.2/10^5$, the cumulative incidence rate (aged from 0 to 74) being 21.7%. The crude incidence rate was $209.6/10^5$ in 2000, and it increased to $320.20/10^5$ in 2009 (52.8%), with an annual percent change (APC) of 4.51% (95% confidence interval, 3.25%-5.79%). Age-specific incidence rate of 80-84 age group was achieved at the highest point of the incidence curve. Overall with different age groups, the cancer incidences differed, the incidence of liver cancer being highest in 15-44 age group in males; the incidence of breast cancer was the highest in 15-64 age group in females; the incidences of lung cancer were the highest in both males and females over the age of 65 years. Conclusions: Lung cancer, digestive system malignancies and breast cancer are the most common cancers in Zhejiang province in China requiring an especial focus. The incidences of thyroid cancer, prostate cancer, cervical cancer and lymphoma have increased rapidly. Prevention and control measures should be implemented for these cancers.

        Study of oxidation behavior and tensile properties of candidate superalloys in the air ingress simulation scenario

        Air ingress incidents are major safety accidents in very-high-temperature reactors (VHTRs). Air containing a high volume fraction of oxygen may cause severe oxidation of core components at the VHTR, especially for the significantly thin alloy tube wall in the intermediate heat exchanger (IHE). The research objects of this study are Inconel 617 and Incoloy 800H, two candidate alloys for IHE in VHTR. The air ingress accident scenario is simulated with high-temperature air flow at 950 C. A continuous oxide scale was formed on the surfaces of both the alloys after the experiment. Because the oxide scale of Inconel 617 has a loose structure, whereas that of Incoloy 800H is denser, Inconel 617 exhibited significantly more severe internal oxidation than Incoloy 800H. Further, Inconel 617 showed a significant decrease in ultimate tensile strength and plasticity after aging for 200 h, whereas Incoloy 800H maintained its tensile properties satisfactorily. Through control experiment under vacuum, we preliminarily concluded that serious internal oxidation is the primary reason for the decline in the tensile properties of Inconel 617

        Rational Approximation of Fractional Order Systems by Vector Fitting Method

        A novel method for approximating fractional order systems is presented. Vector fitting is involved in thismethod. As the basis of approximation of fractional order systems, approximation of fractional order operators ismostly achieved by curve fitting in frequency domain, such as the well-known Oustaloup’s method. However, thesemethods have several serious defects in principle. A new perspective based on system identification is adoptedto deal with approximation of fractional order operators in this paper. Moreover, nonzero initial condition forapproximating fractional order systems is considered. And the proposed assignment of initial values for the Caputocase offers an effective solution for the simulation with nonzero initial condition. Finally, numerical examples aregiven to verify the efficiency of the proposed method.

        Failure Mode, Effects and Criticality Analysis of Remanufactured Machine Tools in Service

        Failure Mode, Effects and Criticality Analysis (FMECA) of remanufactured machine tools is one of the key issues for improving their quality and reliability. According to the characteristics of remanufactured machine tools and the basic principle of FMECA, the method of failure mode analysis for remanufactured machine tools in service is proposed, with the FMECA procedure being expounded in detail. In addition, failure criticality of each failure mode for remanufactured machine tools is analyzed by risk priority number (RPN) method. Finally, the method is demonstrated in a case study of remanufactured YX3120 gear hobbing machine. The results from the case study are discussed and conclusions are drawn.

      • Risk Factors for Cervical Cancer in Rural Areas of Wuhan China: a Matched Case-control Study

        Cervical cancer is a serious public health problem in developing countries. We investigated possible risk factors for cervical cancer in rural areas of Wuhan China using a matched case-control study with 33 women diagnosed with cervical cancer and 132 healthy women selected from the same area as matched controls. A questionnaire, which included questions about general demography conditions, environmental and genetic factors, the first sexual intercourse, first marriage age, age at first pregnancy, pregnancy first child's age, female personal health history, social psychological factors, dietary habits, smoking and alcohol status and other living habits was presented to all participants. At the same time, HPV infection of every participant was examined in laboratory testing. Results showed HPV infection (P<0.000, OR=23.4) and pregnancy first child's age (P<0.000, OR=13.1) to be risk factors for cervical cancer. Menopause (P=0.003, OR=0.073) was a protective factor against cervical cancer. However, there was no indication of associations of environmental (drinking water, insecticide, disinfectant) genetic (cancer family history), or life-style factors (smoking status, alcohol status, physical training, sleep quality), including dietary habits (intake of fruit and vegetable, meat, fried food, bean products and pickled food) or social psychological factors with cervical cancer. The results suggest that the risk of cervical cancer in Chinese rural women may be associated with HPV infection, menopause and the pregnancy first child's age.

        Reliability Allocation Method for Remanufactured Machine Tools Based on Fuzzy Evaluation Importance and Failure Influence

        Reliability is the key performance indicator for remanufactured machine tools to be approved by customers. Reliability allocation is an important task that needs to be done in the design phase of remanufactured machine tools to ensure that remanufactured products meet the reliability target requirements. A reliability allocation method for remanufactured machine tools is proposed based on fuzzy evaluation importance and failure influence of each component. The importance of each component is evaluated by five indicators such as complexity of structure, maturity of technology, criticality of fault, difficulty of maintenance, and severity of service condition, in which their weights are determined by the method of analytic hierarchy process (AHP). Failure influence is determined by the proportion of downtime caused by each component in total downtime of machine tools. Finally, the proposed method is illustrated in a numerical case study of NC lathe remanufacturing.
