        Phase II study of preoperative chemoradiotherapy (CRT) with irinotecan plus S-1 in locally advanced rectal cancer

        Background and purpose: The aim of this study is to evaluate the efficacy and safety of preoperative radiation therapy combined with S-1 and irinotecan (SI) in LARC. Materials and methods: Patients were considered LARC if they had a T3/T4 lesion or node positive. Weekly doses of 40mg/m<SUP>2</SUP> irinotecan were intravenously administered once per week during weeks 1-5 of radiotherapy. S-1 (70mg/m<SUP>2</SUP>) was given from Monday to Friday in all weeks of radiotherapy. 3-D conformal radiotherapy was given at daily fractions of 1.8Gy for 5days for a total dose of 50.4 (45+5.4)Gy. Surgery was performed 4-6weeks following the completion of chemoradiation. Results: Between June 2006 and November 2007, 43 pts were enrolled. The stage was: cT3 24 patients, cT4 6 patients; 28 patients were cN+. Forty-one patients completed the chemoradiation and 42 patients underwent operation: a low anterior resection was performed in 36 patients, a total colectomy in 1 patient, and an abdominal perineal resection in 5 patients. T downstaging was observed in 50%; 23 N+ patients became N- (55%). The complete pathological response was observed in 9 patients (21%). The 3-year locoregional failure rate, distant failure rate, disease-free survival, and overall survival were 9.5%, 18.6%, 72.1%, and 94.3%, respectively. Only three patients experienced G3 diarrhea; one had G3 sepsis and two had septic shock. Hematological toxicity (G3-G4) was observed in five patients. Conclusions: This study demonstrated the efficacy of preoperative CRT with S-1 and irinotecan with 21% of complete response. However, prompt recognition and management of infection is needed to use it in patients with locally advanced rectal cancer.

      • Streptococcus gordonii induces nitric oxide production through its lipoproteins stimulating Toll-like receptor 2 in murine macrophages

        <P>Streptococcus gordonii, a Gram-positive commensal in the oral cavity, is an opportunistic pathogen that can cause endodontic and systemic infections resulting in infective endocarditis. Lipoteichoic acid (LTA) and lipoprotein are major virulence factors of Gram-positive bacteria that are preferentially recognized by Toll-like receptor 2 (TLR2) on immune cells. In the present study, we investigated the effect of S. gordonii LTA and lipoprotein on the production of the representative inflammatory mediator nitric oxide (NO) by the mouse macrophages. Heat-killed S. gordonii wild-type and an LTA-deficient mutant (Delta ltaS) but not a lipoprotein-deficient mutant (Delta lgt) induced NO production in mouse primary macrophages and the cell line, RAW 264.7.S. gordonii wild-type and Delta ItaS also induced the expression of inducible NO synthase (iNOS) at the mRNA and protein levels. In contrast, the Delta lgt mutant showed little effect under the same condition. Furthermore, S. gordonii wild-type and Delta ItaS induced NF-kappa B activation, STAT1 phosphorylation, and IFN-beta expression, which are important for the induction of iNOS gene expression, with little activation by Delta lgt. S. gordonii wild-type and Delta ltaS showed an increased adherence and internalization to RAW 264.7 cells compared to Delta lgt. In addition, S. gordonii wild-type and AIMS, but not Delta lgt, substantially increased TLR2 activation while none of these induced NO production in TLR2-deficient macrophages. Triton X-114-extracted lipoproteins from S. gordonii were sufficient to induce NO production. Collectively, we suggest that lipoprotein is an essential cell wall component of S. gordonii to induce NO production in macrophages through TLR2 triggering NF-kappa B and STAT1 activation.       • Effects of sevoflurane on collagen production and growth factor expression in rats with an excision wound

        <P>Background</P><P>Sevoflurane is a widely used inhalation anesthetic, but there are no studies on its effect on the wound-healing process. This study was undertaken to evaluate the effect of exposure time to sevoflurane on wound healing.</P><P>Method</P><P>Male Sprague–Dawley rats were used. Two circular full-thickness skin defects 8 mm in diameter were made on the dorsum of the rats. The animals were divided into six groups according to exposed gas type and time: S1 (sevoflurane, 1 h), S4 (sevoflurane, 4 h), S8 (sevoflurane, 8 h), O1 (oxygen, 1 h), O4 (oxygen, 4 h), and O8 (oxygen, 8 h). The surface area of the wounds was measured 0, 1, 3, and 7 days after surgery. Separately, the mean blood pressures (MBP) and arterial oxygen pressures (PaO<SUB>2</SUB>) were monitored during the sevoflurane exposure. Collagen type I production and transforming growth factor-β1 (TGF-β1) and basic fibroblast growth factor (bFGF) expression on the wound surface were analyzed. Routine histological analysis was also performed.</P><P>Result</P><P>Exposure duration to sevoflurane had no influence on MBP and PaO<SUB>2</SUB>. The reduction in wound size and collagen type I production was delayed in S8. The expression of TGF-β1 and bFGF on the wound surface in S8 was significantly attenuated in S8. The histology of the S8 demonstrated a delayed healing status.</P><P>Conclusions</P><P>Prolonged exposure to sevoflurane might alter the inflammatory phase of the wound-healing process by attenuation of growth factor expression such as TGF-β1 and bFGF and subsequently by reduced collagen production.</P>

      • Identification of stathmin 1 expression induced by Epstein–Barr virus in human B lymphocytes

        <P>Abstract. </P><P><I>Introduction</I>: The Epstein–Barr virus transforms resting B cells into proliferating lymphoblastoid cells, the origin of cell lines. <I>Method and results</I>: Our cDNA microarray analyses led to the identification of 232 up-regulated and 112 down-regulated genes with more than a 3-fold difference in lymphoblastoid cell lines compared to resting B cells. The functional classification of these genes exhibited the distinct expression signature for cell proliferation, cell cycle and an immune response. Among them, we verified the differential expression of several oncogenes such as <I>stathmin 1</I> (<I>STMN1</I>), <I>RAB27A</I>, <I>RAB9A</I>, <I>BACH1</I> and <I>BACH2</I> using quantitative real-time reverse transcriptase-polymerase chain reactions or Western blot analysis. Expression of <I>STMN1</I> (which is involved in regulation of the microtubule filament system, cell growth and S-phase of cell cycle) was increased in lymphoblastoid cell line as well as in 7-day post-Epstein–Barr virus infection B cells, compared to resting B cells. <I>Conclusion</I>: Thus, this study suggests that Epstein–Barr virus infection induces <I>STMN1</I> expression, which play a role in cell cycle progression and proliferation in the human B lymphocyte.</P>

        Two cases of Meckel's diverticulurn with complication were experienced at the surgical department of Korea university medical college. One of which is the case of panperitonitis resulting from perforation of Meckel's diverticulum with ectopic gastric tissue in the diverticulum and the other is intestinal obstruction due to adhesion and of the inflamed diverticulum. Meckel's diverticulum is a developmental anomaly which is originated from remnant of the embryonic vitelline or omphalomesenteric duct. In most instances, Meckel's diverticulum will be taken into account when symptoms of lower small bowel obstruction or perforation present.

        Strip-shaped diamond field emitter arrays were fabricated by using a transfer mold technique. V-grooves were formed on a single crystalline (100) silicon substrate by an orientation dependent etching(ODE) method. Diamond film was grown on V-grooved silicon substrate by chemical vapor deposition(CVD). Diamond field emitter arrays were formed by etching silicon substrates. I-V characteristics of the fabricated diamond field emitter arrays under 2 x10^(-6) Torr showed very sharp turn-on characteristics. Empirical result which was obtained in this work is feasibility of transfer mold technique for fabrication of diamond field emitter arrays. It may be possible to use the strip-shaped diamond field emitter arrays for the field emission display (FED) applications.

      • Shewanella marina sp. nov., isolated from seawater

        <P>A motile, rod-shaped, pale-brown-pigmented bacterium, designated strain C4T, was isolated from seawater collected from the South Sea (Republic of Korea). Cells were Gram-negative, facultatively anaerobic, and catalase- and oxidase-positive. The major fatty acids were summed feature 3 (C16:1omega7c and/or iso-C15:0 2-OH; 19.4%), C16:0 (16.3%), C17:1omega8c (9.5%) and iso-C15:0 (7.7%). The DNA G+C content was 40.8 mol%. A phylogenetic tree based on 16S rRNA gene sequences showed that strain C4T formed a lineage within the genus Shewanella (92.7-96.1% sequence similarity to representative strains of the genus Shewanella) and was part of a distinct branch with the clade comprising Shewanella haliotis DW01T and Shewanella algae ATCC 51192T. Phenotypic characteristics enabled strain C4T to be distinguished from S. haliotis and S. algae. On the basis of the data presented in this study, strain C4T represents a novel species, for which the name Shewanella marina sp. nov. is proposed. The type strain is C4T (=KCTC 22185T=JCM 15074T).</P>

        원발성 Aldosteronism의 1例 보고 : a case report

        A 45 years old female patient was admitted to Seoul Red Cross Hospital through the emergency room with chief compliants of severe headache, dizziness, and muscle weakness of upper and lower extremities, Her past history was revealed hypertension for 8 years but has not been treated. Physical examination showed that she had hypertension, positive Trousseau's sign, and diminished ' deep tendon reflex. Urine analysis revealed protein (±) and urine pH 8.0. Serum electrolyte showed K 1.8 mEq/L and Na 114 mEq/L. Blood pH was 7.41. Plasma renin activity was 140ng/%. Retroperitoneal pneumogram showed enlarged left adrenal gland. And so under the diagnosis of primary aldosteronism, she was explored and found to have an adenoma in the left adrenal gland. After removal of the left adrenal gland, symptoms and signs were markedly improved.

      • Sorption of aqueous Pb<sup>2+</sup> ion on synthetic manganese oxides-intercalated with exchangeable cations

        This paper describes the preparation, characterization, and application of three different synthetic manganese oxides (K-MO, H-MO, and Mg-MO). K-MO was synthesized by the reduction of potassium permanganate in an aqueous acidic medium. H-MO was prepared by an ion exchange reaction of K-MO with H<SUP>+</SUP>, while Mg-MO was prepared by reaction of H-MO and an aqueous Mg<SUP>2+</SUP> salt solution under reflux. The subsequent solid products were characterized by a chemical composition analysis, XRD, XPS, FT-IR, SEM, and BET measurements. XPS spectra revealed that only tetravalent manganese ions coordinated octahedrally with oxygens. XRD patterns showed that K-MO turned into a layer-structured material while Mg<SUP>2+</SUP> ions were incorporated into the gallery space of the tunnel-structured Mg-MO. Each type of manganese oxide was used in a sorption study of aqueous Pb<SUP>2+</SUP> at 25<SUP>o</SUP>C. The sorption of Pb<SUP>2+</SUP> ions by manganese oxide resulted in increases of the concentrations of pre-intercalated ions (potassium ions, protons, or magnesium ions) and Mn<SUP>2+</SUP> ions. In spite of the smaller surface area and pore volume, K-MO showed greater sorption capacity for Pb<SUP>2+</SUP> ions than that of Mg-MO under the present experimental conditions, thus suggesting that ion exchange is the main mechanism for the sorption of Pb<SUP>2+</SUP> ions on manganese oxides. The results are anticipated to be applicable to the removal of heavy metal ions from wastewater and the prevention of migration of ions in landfill leachates.
