        Comparison Between Sodium Acetate and Sodium Chloride in Parenteral Nutrition for Very Preterm Infants on the Acid-Base Status and Neonatal Outcomes

        Purpose: To compare between sodium acetate (SA) and sodium chloride (SC) in parenteral nutrition (PN) with associated metabolic acidosis and neonatal morbidities in preterm infants. Methods: Preterm infants below 33 weeks gestational age, and with a birth weight under 1,301 g were enrolled and further stratified into two groups: i) <1,000 g, or ii) ≥1,000 g in birth weight. The subjects were randomized to receive PN containing SA or SC within the first day of life. The results of routine blood investigations for the first 6 days of PN were collated, and the neonatal outcomes were recorded upon discharge or demise. Results: Fifty-two infants entered the study, with 26 in each group: 29 infants had extremely low birth weight (ELBW). There were no significant differences in birth weight, gestation, sex, exposure to chorioamnionitis and antenatal steroids, surfactant doses and duration of mechanical ventilation between groups. The SA group had significantly higher mean pH and base excess (BE) from days 4 to 6 than the SC (mean pH, 7.36 vs. 7.34; mean BE −1.6 vs. −3.5 [p<0.01]), with a two-fold increase in the mean BE among ELBW infants. Significantly fewer on SA required additional bicarbonate (n=4 vs. 13, p=0.01). The rate of bronchopulmonary dysplasia (BPD) was approximately four-fold lower in SA than SC (n=3 vs. 11, p<0.01). No significant differences were observed in necrotizing enterocolitis, patent ductus arteriosus, retinopathy of prematurity, cholestatic jaundice, and mortality between groups. Conclusion: The use of SA in PN was associated with reduced metabolic acidosis and fewer BPD.

        Securing Mobile Ad Hoc Networks Using Enhanced Identity-Based Cryptography

        Recent developments in identity-based cryptography (IBC) have provided new solutions to problems related to the security of mobile ad hoc networks (MANETs). Although many proposals to solve problems related to the security of MANETs are suggested by the research community, there is no one solution that fits all. The interdependency cycle between secure routing and security services makes the use of IBC in MANETs very challenging. In this paper, two novel methods are proposed to eliminate the need for this cycle. One of these methods utilizes a key pool to secure routes for the distribution of cryptographic materials, while the other adopts a pairing-based key agreement method. Furthermore, our proposed methods utilize threshold cryptography for shared secret and private key generation to eliminate the “single point of failure” and distribute cryptographic services among network nodes. These characteristics guarantee high levels of availability and scalability for the proposed methods. To illustrate the effectiveness and capabilities of the proposed methods, they are simulated and compared against the performance of existing methods.

      • Identification of Genetic and Non-genetic Risk Factors for Nasopharyngeal Carcinoma in a Southeast Asian Population

        Background: Nasopharyngeal carcinoma (NPC) is endemic in Southern Chinese and Southeast Asian populations. Geographical and ethnic clustering of the cancer is due to genetic, environmental, and lifestyle risk factors. This case-control study aimed to identify or confirm both genetic and non-genetic risk factors for NPC in one of the endemic countries, Malaysia. Materials and Methods: A panel of 768 single-nucleotide polymorphisms (SNPs) previously associated with various cancers and known non-genetic risk factors for NPC were selected and analyzed for their associations with NPC in a case-control study. Results: Statistical analysis identified 40 SNPs associated with NPC risk in our population, including 5 documented previously by genome-wide association studies (GWAS) and other case-control studies; the associations of the remaining 35 SNPs with NPC were novel. In addition, consistent with previous studies, exposure to occupational hazards, overconsumption of salt-cured foods, red meat, as well as low intake of fruits and vegetables were also associated with NPC risk. Conclusions: In short, this study confirmed and/or identified genetic, environmental and dietary risk factors associated with NPC susceptibility in a Southeast Asian population.

        An Ensemble Classification of Mental Health in Malaysia related to the Covid-19 Pandemic using Social Media Sentiment Analysis

        COVID-19 was declared a pandemic by theWorld Health Organization (WHO) on 30 January 2020. The lifestyle of people all over the world has changed since. In most cases, the pandemic has appeared to create severe mental disorders, anxieties, and depression among people. Mostly, the researchers have been conducting surveys to identify the impacts of the pandemic on the mental health of people. Despite the better quality, tailored, and more specific data that can be generated by surveys, social media offers great insights into revealing the impact of the pandemic on mental health. Since people feel connected on social media, thus, this study aims to get the people’s sentiments about the pandemic related to mental issues. Word Cloud was used to visualize and identify the most frequent keywords related to COVID-19 and mental health disorders. This study employs Majority Voting Ensemble (MVE) classification and individual classifiers such as Naïve Bayes (NB), Support Vector Machine (SVM), and Logistic Regression (LR) to classify the sentiment through tweets. The tweets were classified into either positive, neutral, or negative using the Valence Aware Dictionary or sEntiment Reasoner (VADER). Confusion matrix and classification reports bestow the precision, recall, and F1-score in identifying the best algorithm for classifying the sentiments.

      • Influence of Ethnicity on Survival of Breast Cancer Patients in Turkey

        Background: Kurdish women with breast cancer have more unfavorable prognostic factors than their Turkish and Arab counterparts. However, the effects of these factors on breast cancer survival among these ethnic groups remain unclear. We therefore investigated the impact of ethnicity on survival in breast cancer patients in Turkey. Materials and Methods: Ethnicity, age, stage at diagnosis, tumor characteristics, treatments given (surgery, chemotherapy, radiotherapy and hormone therapy), and survival times were recorded. Kaplan-Meier analysis was used to estimate the overall survival times and survival plots. Log-rank test was used to compare the survival curves.Results: Of the 723 breast cancer patients included in the study, 496 (68.7%) were Turkish, 189 (26.2%) were Kurdish, 37 (5.1%) were Arabic and 1 was Armenian. Kurdish women with breast cancer had larger tumor sizes and higher rates of hormone receptor negative tumors than Turkish and Arab patients. Mean follow-up time was 118.4 [95% Confidence Interval (CI): 95.4-141.3] months, and it was 129.9 (95% CI: 93.7-166.2), 124.2 (95% CI: 108.4-140.1) and 103.1 (95% CI: 85.9-120.4) months for Turkish, Arabic and Kurdish patients, respectively. Conclusions: Kurdish ethnicity is associated with higher rates of hormone receptor negative and triple-negative tumors and with worse survival. Clinical and epidemiological research is warranted to elucidate reasons underlying overall survival, variations in tumor biology, differences in treatment responsiveness, and effects of social factors among ethnic groups in Turkey.

      • Benefits and Harms of Phytoestrogen Consumption in Breast Cancer Survivors

        Worldwide, breast cancer is the most common malignant neoplasm and the second most common cause of cancer death in women. This malignancy is recognized to be estrogen-dependent and due to this feature, hormone replacement therapy is regarded as potentially dangerous in breast cancer survivors who seek relief of their menopausal symptoms. Whereas hot flashes are detected in nearly half of postmenopausal women with a relatively high frequency and severity, botanic sources of estrogens have been proposed as an alternative treatment. Nevertheless, estrogenic properties of these compounds suggest possibility of stimulating cancer recurrence or worsening prognosis in survivors. As well, effects in improving vasomotor climacteric changes is controversial. Many studies have considered the subject, some focusing on efficacy of phytoestrogens for control of menopausal symptoms, and others discussing effects of these compounds on breast cancer outcome in terms of survival or recurrence. The present article is a concise review of the effects of consumption of phytoestrogens on menopausal symptoms, namely hot flashes, and breast cancer recurrence and mortality in survivors of the disease. Overall, the major part of the current existing literature is in favor of positive effects of phytoestrogens on breast cancer prognosis, but the efficacy on menopausal symptoms is probably minimal at the best.

      • A Change Impact Analysis Tool for Software Development Phase

        Accepting too many software change requests could contribute to expense and delay in project delivery. On the other hand rejecting the changes may increase customer dissatisfaction. Software project management might use a reliable estimation on potential impacted artifacts to decide whether to accept or reject the changes. In software development phase, an assumption that all classes in the class artifact are completely developed is impractical compared to software maintenance phase. This is due to some classes in the class artifact are still under development or partially developed. This paper extends our previous works on developing an impact analysis approach for the software development phase in automating the approach which we call it CIAT (Change Impact Analysis Tool). The significant achievements of the tool are demonstrated through an extensive experimental validation using several case studies. The experimental analysis shows improvement in the accuracy over current impact analysis results.
