연중독치료시 혈중연, 뇨중연, 뇨중 Coproporphrin, 뇨중 ${\delta}$-Aminolevulinic acid의 변화

In order to study the change of laboratory parameters of lead poisoning, 8 persona who had not been treated previously for lead poisoning (Group 1 and 6 persons who had been inadequately treated for few months for chronic lead poisoning at local clinic (Group 2) were examined. They had occupational exposure to lead for 3 to 18 years (mean, 7.6). In group 1 blood lead, urine lead, urine coproporphyrin and ${\delta}$-aminolevulinic acid levels before our treatment exceeded the critical levels of lead poisoning. In group 2 urine lead level exceeded but blood lead, urine coproporphyrin and ${\delta}$-aminolevulinic acid levels were within normal limits. All of them were treated with D-penicillamine for 4 months as inpatients at Industrial Accident Hospital. The dose of D-penicillamine was the same in all patients; 600 mg per day p.o. and the chelating agent was administer every other week. For laboratory analysis, 24 hour urine and 10 gm of whole blood were collected every 1 month on last day of non-administration period. The results were as follows: 1. It was found that urine lead level was decreased below the cirtical level of lead poisoning after 4 month's treatment with D-penicillamine and blood lead level was decreased more progressively below the critical level after 1 month treatment. 2. Urine coproporphyrin and ${\delta}$-aminolevulinic acid levels were decreased progressively to normal range after 1 month treatment. 3. Two months after treatment, blood lead, urine lead, urine coproporphyrin and ${\delta}$-aminolevulinic acid levels showed some increasing trends. 4. Urine lead level should be checked in a person who had been inadequately treated with chelating agents because blood lead, coproporphyrin and ${\delta}$-aminolevulinic acid might be in normal range.

DEA를 활용한 전통시장 지원정책의 효율성과 고객유치 활성화 방안

본 연구에서는 16개 광역자치단체의 전통시장 관련 자료를 바탕으로 전통시장지원정책의 효과성과 효율성을 실증적으로 살펴보고자 하였다. 전통시장 지원정책의 목표를 시설개선이 아닌 전통시장의 매출액 및 고객증가라고 할 때 기존의 지원방식은 이러한 지원목표와 상당한 괴리가 있음을 상관분석을 통해 확인할 수 있었다. 또 DEA모형의 효율성 분석을 통해 전통시장지원이 기존의 시설개선사업에서 탈피하여 산출변수와 실질적인 관련성이 높은 사업부문으로의 전환될 경우 어떻게 효율적인 지원이 가능 할 것인가에 대해 논의하였다. 매출액증대의 경우에는 고객안내센터, 원산지표시, 자율포장대, 방송시설과 같은 분야의 지원이 보다 효과적이며 방문고객증대의 경우에는 고객안내센터, 방송시설, 방송광고, 경품행사와 같은 분야의 지원이 요구됨을 알 수 있었다. 구체적인 지원 방안으로 인천지역의 시장 상인과 소비자 실태 조사한 자료를 바탕으로 다양한 상품 개발과 교차 판매, 고객 친화적인 고객정보센터 운영, 상품권 활성화, 시장 정보시스템 구축 등의 방안을 제시하였다. In this research, we analyze about the effect of the traditional market supporting policy about 16 large unit self-governing bodies and effeciency through DEA analysis. The plan for supports of the traditional market were shown. Now, the object of the traditional market supporting policy was any more not improvement of facilities and the thing which is the attraction of customer activity for the sales increase of the traditional market could be confirmed. For the sales increase, supporting of the field like the client information center, source indicator, autonomic packing stand, and the broadcasting facility are effective more. In addition, for the visiting customer inducement activation, we could know that supporting of the field like client information center, broadcasting facility, broadcasting advertisement, and the premium event for gift certificate were required. The method including the customer distribution service operation, which is the various product development and cross sellings and client friendly based on data which it investigates the actual conditions the market merchant on the Incheon area and consumer with the concrete plan for support gift certificate activation, the market information system construction, and etc. was shown.

Eccrine spiradenoma usually occurs as a solitary tumor with a wide distribution especially on the ventral surface of the upper body. It generally appears in young adults and charac-teristically is tender or spontaneously painful. We describe a 54 year-old male patient with an indolent eccrine spiradenoma, which had been diagnosed histopathologically.

Letterer-Siwe disease, or acute disseminated histiocytosis X usually occurs in infants. The prognosis generally is serious, particularly if the disease is extensive. The most common manifestations are fever, anemia, thrombocytopenia, enlargement of the liver and spleen, lymphadenopathy and pulmonary infiltrations. Osteolytic lesions are uncommon. In about 80% of the cases, cutaneous lesions are present. They often are the first sign of the disease and are therefore of considerable diagnostic importance. This case occured in a 14 month-old-male showing hepatosplenomegaly, dyspnea and hemorrhagic crused papular eruptions on the entire body. The diagnosis was confirmed by clinical symptoms, laboratory data, histopathologic and radiologic findings.

We present two cases of acute febrile neutrophilic dermatosis developed in 42-year old woman and 53-year old man. The lesions in the female patient was associated with upper respiratory tract infection. After 15 days of treatment with prednisolone, the skin lesions were improved. The lesions in the male patient was in acute myelogenous leukemia. After 20 days of admission, the skin lesions was improved during treatment and exacerbated during not treated periods he died of pneumonia in of anticancer therapy.

This study was carried out for the purpose of evaluating the normal urinary mercury concentration of healthy Korean during the period of 7 months from may 1st to december 31st, 1977. Samples of urine were collected from 454 subjects (male 227, female 277) who had never been exposed to mercury. The mercury level in urine was determined by selective atomic absorption determination of total and inorganic mercury in undigested biological samples. The results obtained from this study were as follows ; 1. The mean of total urinary mercury level of healthy Korean was 26.71±17.33㎍/ℓand the range 0∼93㎍/ℓ. 2. The mean of inorganic mercury level was 19.51±16.55㎍/ℓ and the range 0∼86㎍/ℓ. 3. The mean of organic mercury level was 7.2±6.0㎍/ℓ and the range 0∼33㎍/ℓ. 4. The total, inorganic, organic mercury level did not show any relationship with age and sex. 5. There was good correlation between total mercury jevel and inogranic meroury level(r=0.94, Y=3.64+1.19X). 6. The normal ranges determined by exponential distribution were 0∼80.0㎍/ℓ, for the total mercury, inorganic mercury range 0∼58.5㎍/ℓand organic mercury range 0∼21.5㎍/ℓ.

D-penicillamine 투여에 의한 무기수은중독의 치료효과

The author has experienced two cases of chronic inorganic mercury poisoning that occurred at a tungsten rod manufacturing factory. The mercury concentrations in workroom air were 0.1 and 0.45mg/㎥. The diagnosis was established by clinical and laboratory findings. After 14 weeks of treatment with oral D-penicillamine, both cases were discharged without any complication. The effect of D-penicillamine on mercury poisoning were briefly reviewed.
