        부중대뇌동맥 기시부의 동맥류 - 증례보고 -

        A case of unruptured cerebral aneurysm at the junction of accessory middle cerebral artery and the distal portion of the $A_1$ segment of the anterior cerebral artery is reported. To the authors' knowledge, this is the first reported case of cerebral aneurysm developed at the junction of accessory middle cerebral artery, demonstrated on magnetic resonance angiography(MRA). The accessory middle cerebral artery is a rare vascular variant of middle cerebral artery. Furthermore, it is extremely rare for an aneurysm to be developed at the origin of the accessory middle cerebral artery. The development of the accessory middle cerebral artery is very important in surgery of cerebral aneurysm and collateral circulation of cerebral infarction. Review of the literature regarding the genesis and anatomical variation of the accessory middle cerebral artery is also presented.

        Nontraumatic, incracranial giant aneurysm has rarely been reported as the cause of the spontaneous subarachnoid hemorrhage in childhood. Multiple, dissecting giant aneurysms on the left middle cerebral artery with sudden onset of headache in a 14-year-old girl were successfully clipped and followed by complete relief of symptoms. The rarity and characteristics of such lesion in childhood and its successful surgical treatment are discussed briefly.

        전교통 동맥에서 발생한 독립적인 2개의 동맥류

        More than one cerebral aneurysm on the same artery is uncommon. Furthermore multiple aneurysms on the anterior communicating artery(ACoA) is very rare. We surgically experienced one case of two separate aneurysms on the ACoA ; one developed at the junction of the ACoA with the right second branch of anterior cerebral artery, and the other on inferior wall of the ACoA itself. The former, confirmed as ruptured on operation, projected superiorly and the latter directed inferiorly toward the prechiasmatic cistern. Radiographic and operative findings are presented with review of literature.

        외상 후 후각이상에 대한 방사선학적 진단

        Objective : To evaluate objectively the sites of injury in patients with posttraumatic olfactory deficits and to suggest the diagnostic procedure for evaluation of posttraumatic anosmia. Methods : Ten patients with posttraumatic olfactory dysfunction were examined by means of olfactory testing, sinoscopy, contrast filled paranasal sinus computed tomography(contrast filled PNS CT) and magnetic resonance imaging(MRI). Five normal persons without olfactory dysfunction were also evauluated. The aerodynamic patency of olfactory cleft was examined by contrast filled PNS CT. The olfactory system(oflactory bulbs, olfactory tracts, inferior frontal region, hippocampi, or temporal lobes) was investigated in detail with MRI. The difference in the size of the olfactory bulb between normal volunteers and anosmic patients was evaluated by Student's t test. Results : Contrast filled dynamic CT scan was useful method for the evaluation of dynamic patency of the olfactory cleft. Paranasal CT scan of the all anosmic patients showed dynamic reflux of contrast media in olfactory cleft on valsalva maneuver. For the largest cross-sectional area and great height, the difference in olfactory bulb size between normal volunteers and patients was statistically significant(p<0.001) in MRI study. Conclusion : Posttraumatic anosmia was completely evaluated by olfactory testing, sinoscopy, and contrast filled CT scan for differentiation between conductive type and neurogenic type. Neurogenic anosmia was confirmed by perfect localization with MRI study.

        제4뇌실에서 발생한 비정형성 수막종

        The authors report a case of atypical meningioma in the fourth ventricle. Intraventricular meningioma is not uncommon intracranial neoplasm, especially that in the trigone of lateral ventricle, however, those in the third and fourth ventricle are very rare. There are only two cases of atypical meningiomas in the fourth ventricle among 29 cases of fourth ventricular meningiomas reported 17 literature. The clinical features, findings of magnetic resonance image and operation of our case, and review of literature are presented.

        Gliosarcoma is a rare malignant tumor of the central nervous system consist of gliomatous and sarcomatous elements. The authors report a case of gliosarcoma in the right frontal lobe. Sharp demarcation of the tumor from surrounding tissue and its superficial location may lead to complete removal and prolonged survival despite high malignancy.

        Diaphragma sellae meningioma is a rare entity and its clinical and radiologic findings of are similar to those of pituitary macroadenomas. It is important to differentiate the rare diaphragma sellae meningioma from the common non-functioning pituitary macroadenoma, especially with regards to the surgical approach. We report a rare case of diaphragma sellae meningioma mimicking non-functioning pituitary macroadenoma in a 65-year-old male patient who presented with bitemporal hemianopsia and panhypopituitarism. The patient underwent the tumor removal via the transsphnoidal and transcranial approaches in two times. Careful MRI evaluation is exceedingly valuable for differentiation between two disease entities. The main findings considered are contrast enhancement, visibility and displaced direction of the normal pituitary gland, the center of the lesion and sellar enlargement.

        통합 과학교육을 실천하고 있는 두 중등학교의 사례 : 무엇이 통합 과학교육을 가능하게 하는가?

        본 연구는 통합을 실천하고 있는 두 학교의 사례를 통해 학교 차원의 통합 과학교육의 가능성을 알아보고자 하였다. 이를 위하여 통합을 실천하고 있는 학교에서 일어나는 통합은 어떤 형태인지 그리고 일반 학교와 구별되는 그 학교의 상황 요소는 무엇인지를 수업 참관, 교사 면담, 학교교육계획서 수집, 수업 동영상 분석, 학생 결과물 수집과 같은 다중적인 방법을 사용하여 살펴보았다. 본 연구의 결과는 다음과 같다. 첫째, 두 학교는 통합의 유형, 수준, 교사의 역할, 학생 활동에서 차이를 드러냈으며 이러한 차이가 다른 모습의 통합 과학교육 사례를 보여주었다. 둘째, 두 학교가 통합을 실천할 수 있게 한 학교 상황 요소가 있었다. 통합교육을 실천한 학교를 대상으로 한 선행연구 결과와 비교했을 때 ``리더십``, ``교사 팀의 협력 활동``, ``교육계획을 위한 시간확보``, ``유동적인 시간 운영``, ``지역사회와 연계``와 같은 유사한 학교 상황요소가 발견되었다. 그러나 두 학교의 사례에서는 통합을 기존의 안정적 성격의 교육적 시도가 아닌 보수적인 교사 문화를 극복하여 학교 교육의 혁신을 추구하는 것으로 보았다. 따라서 이 연구에서는 ``보수적 문화극복/혁신 추구``를 새로운 학교 상황 요소로 제안하였다. 또한 두 학교는 안정적인 교사 관계를 형성하였으나 선행 연구에 비해 단기간에 대규모로 실시되었기 때문에 선행 연구에서 제안한 ``소규모의 안정적인 학습 환경`` 대신 ``협력적 친밀한 관계``로 요소를 수정하였다. This is a case study on two schools practising integrated science education (hereafter ISE). The purposes of this study are to investigate the types and features of ISE in the schools actively practising ISE, to identify the contextual factors of the schools, and to give implications for implementing ISE in schools. This study investigated the contextual factors in practicing ISE with a focus on the two schools, a middle school in Gyeonggi-do and a high school in Busan. They were breaking down the boundaries among teaching subjects and providing student-oriented instruction with problems in the real world. The data were collected by observing classes, by interviewing teachers, and by reviewing school documents and students` reports. The research findings are as follows: first, the two schools took part in ISE actively. They teach science to students providing integrated experiences mainly by using interdisciplinary knowledge and/or by solving the problems pertaining to the real world. While the former integrated subjects centering on topics, the latter focused on a project-based learning driven by students. They have differences in regard to the role of teachers and students, the level of integration and the type of integration. Second, the contextual factors that enabled ISE to be implemented there were found. The previous studies revealed six contextual factors in practising ISE: small and stable learning environment, leadership, team activities, in-school planning time, flexible timetable and community links. This study also found similar factors. However, the cases of this study provided 1SF on a large scale and in a short period of time, instead of a small and stable learning environment. Teachers viewed the process of ISE not only as a tool to overcome the conservative culture of teachers but also as a pursuit of innovation.

      • 기초 : Subtractive Hybridization법에 의한 교모세포종에서 특이적으로 발현하는 신규 유전자의 기능 연구

        A variety of genetic alterations in human glioblastomas comprises signal transduction and cell cycle arrest control of cellular processes. In an attempt to identify novel genes implicated in glioblastoma development, we performed experiments using suppression subtraction hybridization(SSH) with human glioblastoma tissue, as well as with cloned genes, that are expressed more than in normal brain tissues. After the identification of a novel gene, RT-PCR was performed to determine its mRNA expression in human glioblastoma tissues. In order to learn more about the expression profile of this gene, PCR was performed using various commercially available normal or carcinoma cell lines. The novel gene was found to be strongly expressed in glioblastoma tissues and other carcinoma cell lines. To determine whether this novel gene was associated with cell cycle regulation, a serum stimulation study with normal WI-38 fibroblast cells treated medium containing 0.1% FBS for 48hr was used for its examination. The novel clone was identified as being expressed in several glioblastoma tissues and carcinoma cell lines. The time-course expression of this novel gene in the WI-38(8PDL) normal lung cell line indicated a significant increase for G1-phase arrest. We have used suppression subtractive hybridization(SSH) to identify cDNA of a gene “Clone 15” overexpressed in human glioblastoma. When the expression of “Clone 15” was screened in numerous cell types, we could confirm that this novel gene was overexpressed in tumor compared to matched normal tissue.

      • The Combined Transmastoid Transjugular Transtubercular High Cervical Approach for Resection of Jugular Foramen Tumors

        Objective : Deep location, hypervascularization, involvement of cranial nerves and vessels, and large extension within the posterior fossa are the main difficulties for surgical resection of the jugular foramen tumors. We describe a combined transmastoid transjugular transtubercular high cervical approach for radical resection of these tumors. Methods : Six patients with jugular foramen tumors were surgically treated using combined transmastoid transjugular transtubercular high cervical approach between January 2000 and June 2008. The complex approach for total jugular foramen exposure can be simplified in a stepwise fashion: 1) postauricular infratemporal incision; 2) retrolabyrinthine mastoidectomy; 3) high cervical exposure; 4) Lateral suboccipital craniotomy and transtubercular exposure; 5) removal of the internal jugular vein (IJV), jugular bulb, and sigmoid sinus; and 6) intradural exposure. Results : Gross total resection was achieved in 5 patients and subtotal resection in one patient. The histologic examination of the tumors revealed as follows: schwannoma (3 cases), meningioma (1 case), paraganglioma (1 case), and chondrosarcoma (1 case). The most frequent complication was a new deficit of lower cranial nerves. There were no facial nerve injury or cerebrospinal fluid leakage. Conclusions : The combined transmastoid transjugular transtubercular high cervical approach described above allows for single-staged radical resection of large complex jugular foramen tumors. This approach has the advantage of providing total exposure of the jugular foramen with multidirectional angles of attack without facial nerve transposition.
