Klebsiella pneumoniae가 간농양에서 차지하는 임상적 의의

Background/Aims: Klebsiella pneumoniae (K. pneumoniae) has been emerging as the leading cause of liver abscess although the most common pathogen was Escherichia coli in the past. Our study was to clarify the significance of K. pneumoniae as a pathogen of pyogenic liver abscess. Methods: We reviewed 157 cases of pyogenic liver abscess treated at Yeungnam University Hospital from 1996 to 2001. They were classified into two groups: K. pneumoniae group and non-K. pneumoniae group. The clinical presentations, characteristics of liver abscess, laboratory findings and the results of bacteriological studies were compared. Results: The K. pneumoniae group included 60 (60.6%) cases among 99 cases with positive culture. We found higher incidence of alcoholics (45.0%) or diabetes millitus (35.0%) in K. pnemoniae group. Cryptogenic cause (61.7%) was the most frequent portal entry in K. pneumoniae liver abscess. On the other hand, in non-K. pneumoniae group, the cause of portal entry was usually the secondary (23.1%) following biliary disease (61.5%). Statistically, there was no significant difference in age, sex, symptom, characteristics of abscess, laboratory findings except total bilirubin level between the two groups. Conclusions: Liver abscess caused by K. pneumoniae has emerged as an important infectious disease with new clinical significance. When clinicians see pyogenic liver abscess in patients with alcoholics or diabetes millitus, K. pneumoniae should be considered first as a cause of liver abscess. 4'-ethoxybenzylidiene-4-n-butylaniline (EBBA)과 4-(n-butyl)-tracts-cyclohexane carboxylic acid cyanophenol ester의 이 성분 혼합액정의 상거동을 결정-액정상, 액정상-등방성 용액 상전이 온도를 측정하여 상태도를 구하고 액정상의 구조적 특성을 관찰하였다. 순수성분은 네마틱 영역을 갖고 있으며 편광현미경하에서 Schlieren형의 특징을 보여 주었으며 혼합물은 공융점 (eutectic point)을 보여주었다. Azoxy형 PAA와 Schiff-base형 EBBA는 단순 공융점을 갖고 있으나 EBBA와 ester계는 공융점 근처, 25-43mole%에서 네마틱을 갖고 있으며 저온도에서 스멕틱 (Smectic)상의 부채살 모양의 특징을 나타내고 있다. The phase behavior of binary mixtures of 4'-ethoxybenzylidene-4-n-butylaniline(EBBA) and 4-(nbutyl)-trans-cyclohexane carboxylic acid cyanophenol ester was investigated by determining the transition temperatures of crystal to mesophase and mesophase to isotropic phase and by observing the optical textures of liquid crystalline phases. In the textures of pure compounds, all exhibited well-developed nematic phases having Schlieren patterns and the eutactic behavior of transition from crystal to nematic phase. The mixtures of azoxy group (p-azoxyanisole) and Schiff-base material(EBBA) have a simple eutectic behavior, while the mixtures of EBBA and ester showed mixed polymorphism near the eutectic compositions, including smectic, a fan-shaped texture, and nematic at 25-43 mole%.
