        Gryllus bimaculatus extract protects against palmitate‑induced β‑cell death by inhibiting ceramide synthesis

        Type I diabetes mellitus is an autoimmune disease characterized by the destruction of β-cells, leading to severe insulin deficiency. Environmental factors and genetic predisposition are implicated in β-cell destruction, which is the final step in a cascade of complex events. Possible triggers of β-cell destruction are activation of Fas, activation of perforin, increased generation of reactive oxygen species, increased production of inflammatory cytokines, and endoplasmic reticulum (ER) stress. In this study, we examined whether Gryllus bimaculatus (GB) extract could prevent palmitate-induced β-cell apoptosis. Exposure to GB extract prevented palmitate-induced death of MIN6 cells, a mouse pancreatic β-cell line. Palmitate increased total ceramide levels with the elevation of ceramide synthase (CerS)1, CerS4, and CerS6 expressions. Treatment with GB extract decreased the levels and expressions of ceramides related to insulin resistance. CerS4 and CerS6 overexpression, but not CerS1 overexpression, increased palmitate-induced MIN6 cell death by increasing ceramide synthesis. Oppositely, inhibition of ceramide synthesis by fumonisin B1 treatment partially recovered palmitate-induced MIN6 cell death. Furthermore, GB extract reduced ER stress (phosphorylation of PERK and eIF2α), NF-κB–iNOS signaling, and the phosphorylation of MAP kinase (JNK, p38). GB extract reduced pro-apoptotic Bax protein expression but increased anti-apoptotic Bcl2 expression. In addition, CerS4 and CerS6 overexpression aggravated impairment of insulin secretion by palmitate, but GB extract recovered it. In conclusion, GB could be a functional food that improves palmitate-induced β-cell death and insulin secretion.

        Recent advances in understanding insulin secretion, action and signaling have led to the development of new pharmacological agents. Several new emerging drugs and drug classes for the management of diabetes are under development, including the incretin mimetic agents (exenatide, dipeptidyl peptidase 4 inhibitors, and glucagon-like peptide 1 analogues), the amylin analogue pramlintide, the cannabinoid-1 receptor antagonist rimonabant, the mixed peroxisome proliferator-activated receptor agonists muraglitazar and the inhaled insulin preparation Exubera. New drugs and technologic advances being made available will help achieve the goals of treating patients with diabetes to all the appropriate metabolic targets. Longer term studies will help providers weigh the benefits, adverse effects, cost, and unknown long-term risks of these medications. (Korean J Med 72:446-450, 2007)

        의예과 학생을 대상으로 한 팀워크 향상 프로그램의 효과

        Background: The purpose of this study is to conduct a teamwork program for pre-medical students using a table-top simulation game called Friday Night at the ER®, and to investigate the effect. Methods: This is a one group pretest-posttest design study 34 students. The tools were understanding of interpersonal relationships (11 questions), problem-solving initiative (8 questions), and system thinking (20 questions) were used. Satisfaction with the program was investigated, and a self-reflection essay was written. For the analysis, paired t-test, frequency analysis, and content analysis were used. Results: After participating in the program, interpersonal understanding, proactivity in problem-solving, and systems thinking all significantly improved (p<.001), and the effect size of the program was also above medium (Cohen’s d= 0.580∼1.019). As a result of analyzing the contents of the self-reflection journal, it was classified into 7 themes and 3 categories; Teamwork competency, systems thinking skills, interpersonal skills. Students were generally satisfied with the program. Conclusion: The simulation game-based teamwork program was effective in improving students’ teamwork. These results can be used as basic data for medical schools designing such programs.

        Urinary and Blood MicroRNA-126 and -770 are Potential Noninvasive Biomarker Candidates for Diabetic Nephropathy: a Meta-Analysis

        <P><B><I>Background/Aims:</I></B> Diabetic nephropathy (DN), a major diabetic microvascular complication, has a long and growing list of biomarkers, including microRNA biomarkers, which have not been consistent across preclinical and clinical studies. This meta-analysis aims to identify significant blood- and urine-incident microRNAs as diagnostic/prognostic biomarker candidates for DN. <B><I>Methods:</I></B> PubMed, Web of Science, and Cochrane Library were searched from their earliest records through 12th Dec 2016. Relevant publications for the meta-analysis included (1) human participants; (2) microRNAs in blood and urine; (3) DN studies; and (4) English language. Four reviewers, including two physicians, independently and blindly extracted published data regarding microRNA profiles in blood and/or urine from subjects with diabetic nephropathy. A random-effect model was used to pool the data. Statistical associations between diabetic nephropathy and urinary or blood microRNA expression levels were assessed. <B><I>Results:</I></B> Fourteen out of 327 studies (n=2,747 patients) were selected. Blood or urinary microRNA expression data of diabetic nephropathy were pooled for this analysis. The hsa-miR-126 family was significantly (OR: 0.57; 95% CI: 0.44-0.74; p-value < 0.0001) downregulated in blood from patients with diabetic kidney disease, while its urinary level was upregulated (OR: 2931.12; 95% CI: 9.96-862623.21; p-value = 0.0059). The hsa-miR-770 family microRNA were significantly (OR: 10.24; 95% CI: 2.37-44.25; p-value = 0.0018) upregulated in both blood and urine from patients with diabetic nephropathy. <B><I>Conclusions:</I></B> Our meta-analysis suggests that hsa-miR-126 and hsa-miR-770 family microRNA may have important diagnostic and pathogenetic implications for DN, which warrants further systematic clinical studies.</P>

        A qualitative study on physicians perceptions of specialty characteristics

        Purpose: There has been limited research on physicians’ perceptions of the specialty characteristics that are needed to sustain a successful career in medical specialties in Korea. Medical Specialty Preference Inventory in the United States or SCI59 (specialty choice inventory) in the United Kingdom are implemented to help medical students plan their careers. The purpose of this study was to explore the characteristics of the major specialties in Korea. Methods: Twelve physicians from different specialties participated in an exploratory study consisting of qualitative interviews about the personal ability and emotional characteristics and job attributes of each specialty. The collected data were analysed with content analysis methods. Results: Twelve codes were extracted for ability & skill attributes, 23 codes for emotion & attitude attributes, and 12 codes for job attributes. Each specialty shows a different profile in terms of its characteristic attributes. Conclusion: The findings have implications for the design of career planning programs for medical students.

        Diabetic myonecrosis is a rare complication associated with poorly controlled diabetes. Although diabetes may be a common disease, diabeteic myonecrosis is a rare complication. The usual presentation is sudden onset of pain at the involved muscles associated with swelling and tenderness without previous trauma or fever. Diagnosis is clinical but MRI is the best investigation for diagnosis. Muscle biopsy is necessary in cases of doubt or to confi rm the imaging diagnosis. Management consists of conservative measures including analgesia and rest. We report one case of diabetic myonecrosis admitted in a tertiary care hospital. We present the case of a 37-year-old male, with 10 years history of well controlled diabetes mellitus type 2, on insulin therapy. Diabetic neuropathy, nephropathy requiring regular hemodialysis, retinopathy, hypertension and dyslipidemia were clinically evident. There was no history of injury of the right thigh. Right thigh was hot, swollen, tender and fi rm with non-pitting edema. MRI revealed T2 high signal intensity and swelling of the vastus lateralis, distal vastus intermedius and medialis, rectus femoris, and short head of biceps femoris muscles. We performed ultrasound-guided muscle biopsy. It showed that multifocal necrosis and fi brosis with atrophy of muscle fi ber and some lymphocytic infi ltration. He was treated conservatively with analgesics and antibiotics. The pain improved in a period of 3-4 weeks. But he complained the pain of the left thigh 4 month later. Early differential diagnosis of diabetic myonecrosis is important. The diagnosis of diabetic myonecrosis is a clinical one that can be supported by MRI. Muscle biopsy is important, to exclude other causes such as pyomyositis, necrotising fasciitis, and neoplasm. Conservatively management including tight glycemic control should be achieved. Nonsurgical therapy appears to provide the most favorable outcome.

        저용량의 corticosteroid 복용이 B형 간염 바이러스 재활성화에 미치는 영향

        목적: 만성 B형 간염 바이러스 감염 환자에서 세포 독성 혹은 면역억제 치료를 받는 경우 상당수에서 B형 간염 바이러스의 재활성화가 일어나는 것으로 보고되고 있다. 이에 본 연구자들은 저용량의 corticosteroid 복용이 B형 간염 바이러스의 재활성화에 미치는 영향을 알아보고자 하였다. 방법: 1998년 8월부터 2007년 3월까지 삼성서울병원에서 악성 종양 혹은 장기 이식을 제외한 염증성 질환으로 1주 이상 경구 혹은 주사용 corticosteroid 제제를 투여 받은 병력이 있는 만성 B형 간염 바이러스 감염 환자들을 대상으로 후향적 의무기록 분석을 시행하였다. Corticosteroid 제제 투여 전후로 HBV DNA 역가가 1 log10 이상 증가한 경우 B형 간염 바이러스의 재활성화로 간주하였다. 결과: 대상 환자들은 16명이었으며, 평균 연령은 45.4±16.7세였고, 남녀비는 14:2였다. 동반 질환은 기관지 천식, 알레르기성 두드러기, 알레르기성 비염 등이었으며, 사용된 corticosteroid의 종류는 및 prednisolone, dexamethasone methyl-prednisolone 등이었다. Corticosteroid 사용량은 prednisolone equivalent dose로 환산하였을 때 하루 2.5~50 mg이었으며, 하루 20 mg 이하 사용군이 11명이었다. 평균 corticosteroid 투여 기간은 60.1일(범위: 7-364일)이었다. 대상 환자들 가운데 corticosteroid의 투약 전후로 B형 간염 바이러스의 재활성화를 보인 경우는 1예였다. 이 환자는 prednisolone (5 mg/day)을 364일간 복용한 강직성 척수염 환자였고, cortico-steroid 투약 전 HBeAg 양성이었으며, HBV DNA는 26.8 pg/mL에서 2183.2 pg/mL으로 증가하였으나 투약 전후 혈청 ALT를 포함한 albumin, bilirubin 및 PT의 악화 소견은 없었다. 결론: 만성 B형 간염 바이러스 감염 환자에서 단기간 저용량의 corticosteroid 복용은 B형 간염 바이러스 재활성화에 그다지 영향을 미치지 않을 것으로 여겨지나 장기간 복용시 재활성화를 일으킬 가능성이 있어 주의를 요한다. Background/Aims: We investigated the effect of low dose corticosteroid therapy on HBV reactivation in patients with chronic HBV infection. Methods: From August 1998 to March 2007, the HBsAg-positive patients who received oral or intravenous corticosteroid therapy for more than 1 week at Samsung Medical Center were included in this retrospective study. We included those patients who received anticancer chemotherapy or organ transplantation, or concurrent antiviral therapy or other immunosuppressive agents. HBV reactivation was defined as a 10-fold increase in the HBV DNA levels compared with baseline. Results: A total of 16 patients were included. They were 45.4±16.7 years of age, and the male:female ratio was 14:2. Their combined diseases included bronchial asthma, allergic urticaria, allergic rhinitis, etc. The corticosteroid doses were converted to prednisolone equivalent doses and these varied from 2.5 mg to 50 mg per day. Eleven patients used less than 20 mg of prednisolone per day. The mean medication duration was 60.1 days (range: 7-364 days). Among the patients, only one patient showed HBV reactivation. This ankylosing spondylitis patient was a 31-year old man who took prednisolone 5 mg/day for 364 days. He displayed HBeAg-positivity before corticosteroid treatment. There was no aggravation of the levels of ALT, albumin, bilirubin, and PT between the pre-and post-medication in this patient. Conclusions: The short term use of low dose corticosteroid is not likely to be related with HBV reactivation in those patients with chronic HBV infection, yet long term use may lead to viral reactivation. Further large scaled, prospective studies on this subject are needed.(Korean J Med 74:619-623, 2008)
