사려 깊은 의학의 이데올로기 : 일상생활의 의료화라는 의학적 표상체계의 문제

우리의 몸은 자연의 질서에 속한 자명한 실체로서의 몸이라기보다는 혈압수치, 콜레스트롤 수치, 심박수, 간수치, 비만도, 혈당치 등의 온갖 지표들을 통해 표상되는 의학적 현실로서의 몸이다. 이런 지표들은 물질적이며 직접적인 몸과는 다른, 일련의 추상에 불과하다. 그러나 우리가 우리의 몸을 인식하고 사유하고, 나아가 개입하고 변화시킬 수 있는 현실의 몸으로 직조하는 유일한 방법은 바로 그 추상의 과정밖에 없다는 점에서 그것은 현실적 추상이다. 우리는 우리의 몸을 지시하는 수많은 건강 지표들과 나란히 출산율, 사망률, 질병률과 같은 통계들과 조우한다. 그런 표상의 질서 속에서 의학적 현실은 정치적, 경제적 현실로 분절되고 확장된다. 일상생활의 의료화란 이데올로기에 대한 과학의 승리가 선언된 시대에 이러한 표상의 질서가 형성되는 방식을 가리키는 용어다. 이 글은 인간의 몸과 생명이 어떤 종류의 지식을 통해 표상되는지, 그리고 그러한 새로운 표상이 왜 필요하고, 어떤 개입의 장을 개시하는지를 질문하면서 그런 표상의 질서의 계보를 그린다. 그를 위해 이 글에서는 제약산업의 성장, 유전학의 발달, 그리고 보험산업이 제공하는 리스크 관리 전략 등을 통해 만들어진 인간의 몸과 생명을 표상하는 새로운 테크놀로지들이 탐색될 것이다. 그 후 그러한 표상의 질서가 내재한 문제들을 드러내고, 그 문제를 타개하기 위한 돌파구로서 등장한 생명윤리학이라는 지배적 담론에 대해 살펴볼 것이다. 마지막으로 건강이나 질병 혹은 생명의 문제와 관련해 생명윤리학이 지닌 한계와 무능을 살펴봄으로써, 탈이데올로기 시대라고 선언된 이 시대에 필요한 것은 이데올로기 비판임을 주장할 것이다. Our body is not a body as self-evident substance that belongs to natural order, but one as medical reality represented by the same indexes blood pressure, cholesterol, heart rate, degree of obesity, blood-sugar level and so forth. Unlike material body, such indexes are nothing but abstraction. The abstraction, however, is a ‘real abstraction’ in that the only way that we can perceive and be aware of our body, and further make it a real body, the body that can be intervened and changed, is the very process of the abstraction. Along with the health indexes referreing to our body, we encounter statistical data inclusive of birth rate, mortality rate, morbidity rate and so on. Within the order of representation, medical reality is articulated and extended to politico-economic reality. Medicalization of everyday life is the term referring to the way in which the order of representation is shaped in an era in which modern science declares its victory over ideology. Thus, this essay envisions the genealogy of order of representation by examining what kind of knowledge represents our body and life, why we require the order of representation, and what field of intervention can be opened. To do so, this essay explores new techonologies to construct our body and life, which are formed by growth of pharmaceuticals, development of genetics, and risk management stratagy of insurance industry. Next, the essay foregrounds problems inherent in the order of representation and examines dominant discourse of bioethics as a wayout to solve such problems. Finally, the essay argues that ideological critique is required in an era of post-ideology by explaining limits and incapability in bioethics.
