긴 골수강내 금속정을 이용한 불안정성 고관절 전자부 골절의 치료

목적: 골절선이 하부로 연장되었거나 골수강이 넓은 불안정성 고관절 전자부 골절을 긴 골수강내 금속정으로 치료한 후 방사선적, 임상적 결과를 분석하였다. 대상 및 방법: 2005년 1월부터 2010년 1월까지 불안정성 고관절 전자부 골절(AO/OTA 31 A2.2 이상) 환자들 중 긴 골수강내 금속정으로 치료하고 추시 기간이 12개월 이상인 20예를 대상으로 방사선적 평가는 TAD, Cleveland index, 대퇴 경간각 변화, 지연 나사의 활강 정도, 골유합 시기를 측정하였으며 임상적 평가는 수술 시간, 수혈량, Parker와 Palmer의 mobility score를 분석하였다. 결과: 골유합의 시기는 평균 16.4주(10-21주), 대퇴 경부 지연 나사의 활강 거리는 평균 4.3 mm(1-10 mm), 술 후 대퇴 경간각의 변화는 평균 5.1°(3-8°)였다. 수술 시간은 평균 118분(60-140분), 수술 중 평균 수혈량은 2.0 pint(0-4 pint), Parker와 Palmer의 가동성 점수는 술 전 평균 7.5점, 술 후 평균 6.9점이었다. 결론: 불안정성 대퇴골 전자부 골절의 치료 시에 골절선이 하부로 연장되거나 골수강이 넓을 경우 긴골수강내 금속정의 사용이 유용할 것으로 사료된다. Purpose: The purpose of this study is to analyze the clinical and radiographic results of treatment of unstable pertrochanteric hip fractures in which the fracture line is extended distally or the width of the canal is wide using a long intramedullary nail. Materials and Methods: From January 2005 to January 2010, we conducted an analysis of 20 cases of unstable pertrochanteric hip fractures (over AO/OTA 31 A2.2) treated using a long intramedullary nailing and followed up for more than 12 months. Using postoperative and last follow-up radiographs, we measured TAD, Cleveland index, neck-shaft angle, sliding length of the lag screw, and union time. Clinically, we measured the operation time, the amount of transfusion, and Parker and Palmer`s mobility score. Results: The mean bony union time was 16.4 weeks(10-21 weeks). The mean sliding length of the lag screw was 4.3 mm(1-10 mm) and the mean varus angulation after surgery was 5.1°(3-8°). The mean operative time was 118 min(60-140 min), and the mean transfusion amount was 2.0 pint(0-4 pint). According to Parker and Palmer`s mobility score, mean preoperative score was 7.5 points and mean postoperative score at last follow-up was 6.9 points. Conclusion: In treatment of unstable pertrochanteric hip fractures, when the fracture line was extended to below the lesser trochanteric level or the medullary canal width was wide, use of a long intramedullary nail showed excellent clinical and radiologic results and can be regarded as a good treatment option.
