        의료서비스 산업화와 병원 경영

        The healthcare industry is considered as a potential industry which can lead a nation to achieve global competitiveness with value-added. In this reason Korean government selected a healthcare business as one of major driving industries for next generation, and has been supporting the industry. As of 2012, national healthcare expenditures account for 7.6% of GDP, and of those expenditures, health service business accounts for more than half of total national healthcare expenditures(65% of NHE). Industrialization of health services has been attempted from mid-2000s. Due to unique characteristics of health services, however, this drive has been facing never-ending opposition and not yet completed. This paper traces background of health services industrialization. Beginning with definition of industrialization, necessity of industrialization is described. In addition, influences of industrialization into hospital management and political implication are presented.

        병원전문화 전략유형의 적합성과 조직성과 분석

        This research classified hospital specialization strategy types through cluster analysis, analyzed fitness of hospital specialization strategy types for external environment or organizational structure, and examined relation between hospital specialization strategy types and organizational performance. This research surveyed 1,437 hospitals which have more than 30 patient`s bed and practice national health service in Korea. Specifically, this research divided into two part : external fit - analysis of relation between external environment and specialization strategy, internal fit-analysis of relation between organizational structure factors and specialization strategy. also, as the organizational performance for achieving specialization strategic purpose, not only the productivity, efficiency, profit but also the medical quality was considered. In case of external fit, many hospitals chose integration type if there are a lot of competitive hospitals and regional population. Particularly, if there are many competitive hospitals, concentration type is chosen. In contrast, if there are many doctors in the region, differentiation type is chosen. In case of internal fit, according to organization type and patient`s bed number, hospitals chose different types. If it is a general hospital and has a few bed number, generalization or concentration type is chosen. Tertiary hospital or the hospital with many patient`s bed chose differentiation type. According to the number of specialists, if there are a few specialists, generalization or concentration type is chosen. If there are many specialists, differentiation type has high fitness for the hospital. In relation to strategy types and organizational performance, differentiation type has best result. Differentiation type has a good result in 7 items out of 11.

        가치사슬 활동이 종합병원 경영성과에 미치는 영향 분석

        This study, targeting Korean tertiary hospitals and general hospitals, aims to analyze how value chain model inhealth and medical institution suggested by Duncan and else influences on hospital management. A survey wasconducted to verify the actual proof analysis of this study model. 880 questionnaires were distributed to entire 88hospitals and 739 copies were returned from 76 hospitals. This study mainly consists of three steps to analyze theeffect value chain activity has on management performance of general hospitals. For the first step, we analyzedthe effects service delivery activity has on management performance. For the second step, we analyzed theeffects service support activity has on management performance and for the third, we analyzed the effectsinteraction between service delivery activity and service support activity has on management performance. Themain results of this study are as follows. First, in terms of the management performance of scale, the factors whichinfluenced on daily charge of outpatient were service activity before treatment, at the moment of treatment andvalue chain activity, while more important factors in daily charge of inpatient were organizational culture,organizational structure and value chain activity. In terms of management performance of quality, the factors whichinfluenced on the first medical examination rate of outpatient were service activity before, at the moment of andafter treatment, while activity at the moment of treatment, organizational structure, and value chain activity which isinteraction were more important factors in average length of stay. In terms of non-financial performance, themanagement performance factors which influenced on job satisfaction were service activity at the moment of, afterthe treatment and value chain activity, while organizational culture, strategy resources and value chain activitywhich is interaction were more important factors in job commitment. Secondly, all the service support activity,service delivery activity and value chain activity had statistically significant effect on management performance.Among the three factors, service support activity had relatively high effect than others.

        병원인증제도가 리더십, 조직문화, 병원경영 활동 및 성과에 미친 영향

        The purpose of this study is to analyze the change of hospitals that patients safety and quality improvement by accreditation process and to examine the impact or interrelation of leadership, organizational culture, hospital management activities and recognition of hospital management performances. The data were collected through a review of the literature, and selfadministered survey with a structured questionnaires to 714 subjects from several medical staff members, administration staff members, nursing staff members, medical technicians and other staff members working in 23 accredited hospitals in Korea. In this analysis hierarchical multiple regression and structural equation model were used. The conclusion of this study provides a theoretical model for understanding organizational changes brought about by accreditation system. Factor on improvement of efficiency and raise the morale, rather than increase of medical income and reduce of the cost factors, had a stronger influence on the accreditation process. In the future, the hospital`s participation to induce the accreditation program voluntarily will come up with an alternative policy concern about financial perspective. Also, the hospitals which preparing accreditation program to achieve the goal efficiently, will make use of transformational leadership through enhancing individual consideration and intellectual development to leading members participation. Additionally, non-accredited hospitals should aim at professional culture by innovative and creative approaches, and inviting members to learning and growth in the organization.
