망막색소변성환자에서 외상 후 발생한 장액망막박리를 동반하는 지연 발현 푸르처망막병증 1예

적: 외상 이후 발생한 시력저하로 의뢰된 망막색소변성환자에게서 장액망막박리를 동반하는 푸르처망막병증 1예를 경험하였기에 이를 보고하고자 한다 증례요약: 36세 남자 환자가 교통사고로 흉부에 외상을 입은 이후 발생한 양안 시력저하를 주소로 진료 의뢰되었다. 과거력상 망막색 소병증으로 개인 안과에서 경과관찰 중이었다. 교정시력은 우안 0.3, 좌안 0.6이었다. 전안부 검사상 특이소견은 없었다. 안저검사에서 양안에서 특징적인 망막색소변성 소견과 함께 우안 황반부에 경계가 비교적 분명하고 투명한 장액성 융기로 의심되는 병변이 있었고 시신경 주변으로 삼출물로 의심되는 소견이 관찰되었다. 빛간섭단층촬영에서 우안 장액 망막박리소견이 있었고, 형광안저혈관조영 에서 망막색소변성의 특징적인 소견을 볼 수 있었다. 외상 7일째 내원 시 우안 안저검사상 면화반 소견은 더 증가하고 현저해졌으며 망막출혈소견도 관찰되었다. 환자의 병력과 특징적인 안저소견을 바탕으로 푸르처망막병증으로 진단하였다. 환자는 1개월 뒤 우안 장액 망막박리소견은 호전되었지만 우안 시력저하와 우안 전시야맹 소견이 남았다. 결론: 망막색소변성증 환자에게 푸르처망막병증이 동반된 경우는 매우 드문 경우이며, 망막하액을 동반하고 전형적인 임상소견이 지연 발현된 점은 일반적인 푸르처망막병증의 임상양상과는 다르기에 본 증례를 보고하고자 한다. Purpose: To report a case of Purtscher’s retinopathy accompanied by serous retinal detachment in a patient with retinitis pigmentosa (RP) who was referred to us for treatment of post-traumatic visual discomfort. Case summary: A 36-year-old man with history of RP was referred to us with the chief complaint of bilateral visual discomfort after chest injury from a traffic accident. His corrected visual acuity was 0.3 and 0.6 in the right and left eyes, respectively. Fundus examination revealed findings characteristic of RP in both eyes, along with a lesion in the right eye, which was suspected to be a serous elevation of the macula, as well as suspected exudates near the optic nerves. Optical coherence tomography indicated serous retinal detachment in the right eye, and fluorescein angiography findings were characteristic of RP. Seven days later, the amount of cotton-wool exudate in the right eye had increased and was more distinct than at the initial examination, and retinal hemorrhage was observed. Based on the medical history and specific fundus findings, the patient was diagnosed with Purtscher’s retinopathy. One month later, the serous retinal detachment in the right eye had improved, but the vision loss and total anopsia in the right eye persisted. Conclusions: The concomitant occurrence of RP and Purtscher’s retinopathy is very rare. Moreover, the presence of accompanying serous retinal detachment and delayed onset of typical clinical symptoms are not present in typical Purtscher’s retinopathy. 목적: 안내염 환자에게서 유리체절제술 이후에 전방염증이 호전과 악화를 수차례 반복하는 드문 경험을 하였기에 이를 보고하고자 한다. 증례요약: 간농양으로 본원 소화기내과에 입원 중인 54세 남자 환자가 2일 전 발생하여 1일 전 심해진 좌안 시력저하를 주소로 진료 의뢰되었다. 전안부 및 초음파 검사를 통한 안저검사에서 좌안 내인성 안내염이 의심되어 초진 당일 유리체절제술 시행 후 topical vancomycin (2.5%)과 ceftazidime (5%)을 점안하며 경과 관찰하였다. 소화기내과에서 혈액배양검사 및 간농양 배양검사 직후 바로 전신적 항생제를 사용하고 있던 환자로 유리체절제술 시에 채취한 전방수 및 유리체 배양검사에는 폐렴막대균이 동정되지 않았지만 혈액배양검사와 간농양 배양검사 모두 폐렴막대균이 동정되어 폐렴막대균에 의한 내인성 안내염으로 확진할 수 있었다. 수술 후 환자는 호전양상을 보이다 수술 후 20일째, 40일째 각각 뿌옇게 보이는 증상과 함께 전방 염증 및 축농이 다시 관찰되어 전방내 항생제 주사치료 및 전방세척술을 시행하였으며 이후 현재까지 추가적인 염증소견 없이 경과관찰 중이다. 결론: 안내염 환자에게서 유리체절제술 이후에 3차례의 전방축농 및 전방염증이 악화되는 소견이 있었으나 전방내 항생제 주사치료및 전방세척술을 통하여 효과적으로 염증조절을 할 수 있었다. Purpose: To report an unusual case of endogenous endophthalmitis in a patient with Klebsiella pneumoniae primary liver abscess. Case summary: A-54-year-old man with diabetes mellitus and liver abscess was referred to us for consultation of visual loss in his left eye for 2 days. On the first examination, the patient’s left visual acuity was hand motion and the left intraocular pressure was 13 mmHg. Vitreous opacity and inflammatory membrane were detected with increased echogenicity using ultrasonography. Vitectomy and intravitreous antibiotic injection were performed under the impression of endogenous endophthalmitis caused by liver abscess on the day of the first visit. Culture revealed Klebsiella pneumoniae from blood and liver abscess. After vitrectomy, the patient showed improvement. However, on the 20th and 40th postoperative days, the patient complained of blurred vision, and inflammation and hypopyon were observed in the anterior chamber. An intracameral antibiotic injection and anterior chamber washing were performed. The patient has not complained of any other symptoms to date. Conclusions: In this patient with endophthalmitis, inflammation and hypopyon in the anterior chamber were evident three times after vitrectomy. Ultimately, the inflammation was effectively controlled by intracameral antibiotic injection and anterior chamber washes. J Korean Ophthalmol Soc 2017;58(11):1301-1306
