일상생활 동작 중심 거울치료를 적용한 뇌졸중 환자의 상지 기능 회복 -증례보고-

Background and Purpose The aim of this study was to determine the effect of Mirror therapy focused on activities of daily living on recovery of motor function of upper limb and on the changes of corticospinal excitability in acute hemiplegia after stoke. Subjects and Methods The 2 subjects(2 males) were patients with stroke. Subjects( onset <6 months, K-MMSE >24) received mirror therapy focused on activities of daily living for 30 minutes after each treatment, additionally with the traditional rehabilitation programs, 5 days per week for 4 weeks. Some activities of mirror therapy were selected with consideration for interests and experiences of the subjects. To measure the improvement of motor function of upper limb, we performed Fugl-Meyer assessment(FMA), manual function test(MFT), Jamar strength test and pinch force test for each sessions. And before and after the 4 weeks intervention, Motor Evoked Potential(MEP)s of the Abductor Digit Minimi(ADM) of affected upper limb were obtained by the repetitive Transcranial Magnetic Stimulation (rTMS) to investigate the changes of corticospinal excitability. Results The 2 subjects showed significant improvements in FMA, MFT, Jamar strength test and Pinch force test. One subject showed the response at the reassessment, compared to ``no response`` at the initial assessment, although that is ``prolonged`` latency and ``low`` amplitude. The other showed increased MEP amplitude compared to initial assessment. Conclusions Mirror therapy focused on activities of daily living can be used as an adjuvant therapeutic technique by improving motor function of upper limb and facilitating of corticospinal excitability for acute hemiplegia. Further individual studies of mirror therapy focused on activities of daily living should be conducted in order to generalize the effects.

Purpose The aim of this study was to determine the effect of somatosensory training focused on upper limb with trunk muscle stability of upper limb fuction performance in chronic hemiplegia after stroke. Methods Two subjects(2 males) were patients with stroke. Subjects(onset >6 months, K_MMSE >24) received somatosensory training focused on upper limb with trunk muscle stability for 50 minutes after each treatment, 5 days per week for 4 weeks. Activities of Somatosensory training focused on upper limb were scapular orientation, scapular setting, hand extrinsic and intrinsic muscle stimulation. Also the activity included induced friction with both hands on the ground for mid_line formation. Finally, trunk muscle exercises conducted for trunk stabiltiy. To measure the outcome, Manual Function Test, Box and Block test, Jebson_Taylor hand function test, Jamar strength test and pinch force test. Manual Muscle test, Light_Touch test were conducted. Results The 2 subjects showed significant improvements in manual function test(MFT), Box and Block test, Jebson_Taylor hand function test, Jamar strength test and pinch force test. Manual Muscle test, Light_Touch test. Conclusion Somatosensory training focused on upper limb with trunk muscle stability can be used as a therapeutic technique in order to improve upper limb function and strength for chronic hemiplegia. In addition, further investigation should examine individual studies of somatosensory training focused on upper limb with trunk muscle stability should be conducted in order to generalize the effects.
고유수용성 감각 운동 프로그램이 편마비 환자의 운동기능 및 일상생활동작에 미치는 영향

Purpose The purpose of this study was to determine effects of proprioceptive exercise for proprioceptive sense and motor function, ADL in hemiplegia. Methods Six person with hemiplegia was participated this study, they carried out proprioceptive exercise for 30 minites of one day three times per one weeks for six weeks. In order to compare between pre and post of proprioceptive exercise, we evaluated PAR (Passive Angle Represent) for sensitization of proprioception, FIM(Functional Independence Measure) for ADL, SIAS(Stroke Impairment Assessment Scale) for motor function. Data was analyzed using by paired t-test between pre and post of proprioceptive exercise and ANOVA among value of pre, after 3 weeks, 6weeks. Results The average value of PAR and FIM was increased, but there was not significantly difference. The motor function and total score of SIAS show significantly difference between pre and post of proprioceptive exercise. There was no significantly value of ANOV A among pre and after 3, 6 weeks. Conclusion Proprioceptive exercise can enhance the motor function and effect on PAR, FIM in hemiplegia.
