        Dermoscopy로 감별한 피부섬유종 양상으로 나타난 피부유육종증

        Sarcoidosis is a multisystem granulomatous disease of unknown cause. The lung is the most commonly affected organ, but the skin is frequently involved. Specific sardoidal lesions are most often found on the head and neck area. The most common specific presentation is multiple papular lesions. The present case shows solitary cutaneous sarcoidosis resembling dermatofibroma, located on the thigh. Dermoscopy can help in the differential diagnosis of cutaneous sarcoidosis. Dermoscopic findings are orange globular-like structures and linear vessels, and areas of scar- like depigmentation. Histopathologic findings show non-caseating granulomas with sparse inflammatory cells. We report a case of cutaneous sarcoidosis resembling dermatofibroma, which was diagnosed using dermoscopy.         Background: Poly-γ-Glutamic acid (γ-PGA) is a safe and biodegradable polymer that is produced by Bacillus subtilis. Recent research showed that γ-PGA can be used as an immune-stimulating and anti-tumor agent. But the mechanism of its actions is still unexplored. Objective: To investigate the further define the potential of γ-PGA as a modifier of monocyte activity. Methods: Peripheral blood mononuclear cells (PBMNC), obtained from three healthy volunteers, were incubated after treated with lower molecular γ- PGA (1000 Da) and higher molecular γ-PGA (5000 kDa). The levels of the cytokines IL-1β, IFN-γ and TNF-α were measured. We then examined the levels of these inflammatory cytokines after treated with lipopolysaccharide (LPS) in the presence or absence of γ-PGA. Results: Low or high molecular γ-PGA alone induced release of IL-1β, IFN-γ and TNF-α significantly. High molecular γ-PGA induced more cytokines response than low molecular γ-PGA. LPS induced IL-1β, IFN-γ and TNF-α synthesis was enhanced significantly. This effect was augmented significantly by costimulation with LPS/high molecular γ-PGA versus LSP alone. Conclusion: These results suggest that γ-PGA, especially high molecular γ-PGA, play a role in immune response. As a potent immune modifier, PGA contributes to stimulation of immune responses.
