        부주의한 뚜렛장애 환자에게 뉴로피드백을 병행한 치험 1례

        We have cared for a 13years old boy, who has Tourette Syndrome and symptoms of vocal tic and motor tic for 5years, since 9months ago. We have treated him with korean herb medicine , 8-constitution acupuncture, Chimsband, Neurofeedback, EFT therapy. After taking treatment, his YGTSS score was down to 38 from 119, and his tic symtoms altered for the better. We think korean herb medicine had a powerful effect and Neurofeedback was effective. Especially, SMR Beta Training of all programs of Neurofeedback was effective and well-directed. The EFT program helped him improved also, but it need much of time to apply. When we evaluated the condition of patient who was under the Cans 3000, we could not find any relationship between tic symtoms and Cans 3000. When we treat Tourette Syndrome, having the patience is important at a distance of time. Because Tourette Syndrome is a kind of unstable symptoms, so we would not care to predict what the result will be in a brief space of time.

        한의원에 내원한 틱장애 환자 292례 증례분석

        Objectives: In this study, patients with tic disorders who visited an Oriental medicine clinic were examined for their demographic characteristics, characteristics of symptoms, relation to Attention-deficit Hyperactivity Disorder(ADHD) and peculiarity according to various variables such as motor and vocal tics. Methods: After surveying 292 patients who visited an Oriental medicine clinic with tic symptoms as main complaints for 17 months, SAS 9.1, a statistical program was used for statistical analysis. Results: 1. The BMI of male tic patients was significantly higher than female ones and it was similar to or higher than the normal group. 2. Patients who are eldest children were 1.7 times higher than those who are not eldest ones. 3. The most usual case of motor tics was the eye blink and the most one of vocal tics was a dry cough. 4. There was no significant difference between male and female patients for all symptoms of motor and vocal tics, but male patients had significantly more obsessions related to tics than female ones. 5. There was no significant difference in the age of initial occurrence of Transient tic disorder(TTD), Chronic tic disoder(CTD) and Tourette`s disorder(TD). 6. For the general disorder of a tic and Conners` ADHD rating scale, there was no significance in TTD, CTD and TD. 7. 66% out of the total subjects of 197 cases were found to score more than 65 points in more than 1 items among 8 items such as the time, hearing, wrong alarm, mean response time and standard deviation in the response time, etc. of the ADHD diagnosis system(ADS). 8. The eye blink among motor tics was shown mainly by patients under 10 years old and the frown, movement of the head, shrug and movement of the arms were shown mainly by 11-19 years old patients. Conclusions: For the number, frequency, seriousness and inconvenience in life of tics, TD showed a significantly higher result than TTD and CTD.

        소풍순기원(疏風順氣元)이 고지방식이 비만 대사증후군 병태 흰쥐에 미치는 효과

        Objectives: We investigated the effects of Sopungsungi-won(Shufengshunqiyuan) (SSEx1, SSEx2) on the metabolic syndrome in high-fat diet induced obese mice. Methods: 8 weeks old, high fat diet induced obese male mice were divided into 4 groups: C57BL/6 lean control, obese vehicle control, SSEx1, SSEx2. After mice were treated with SSEx1, SSEx2 for 12 weeks, we measured body weight gain, food intake, feeding efficiency ratio, fat weight, plasma leptin, insulin, glucose and lipid levels. We also observe the morphology and count for the numbers of Adipocyte and evaluate the weight of organs and it`s function. Results: 1. Compared to Obese Control Group, SSEx1 gained significantly lower body weight and showed lower Feeding Efficiency Ratio. 2. Compared to Obese Control Group, SSEx1 showed lower weights of epididymal adipose tissue, troperitoneal adipose tissue, inguinal adipose tissue, brown adipose tissue. SSEx2 showed higher weights of epididymal adipose tissue, troperitoneal adipose tissue, inguinal adipose tissue, brown adipose tissue. 3. Compared to Obese Control Group, the size of adipocytes was significantly decreased by SSEx1, whereas the number of adipocites per unit was significantly increased. Hepatic lipid accumulation was decreased significantly by SSEx1. 4. Concerning the weights of Liver, Heart, Spleen, Kidney and Pancreas, SSEx1, SSEx2 showed little differences with those of Lean Control, Obese Control. 5. Compared to Obese Control Group, SSEX1, SSEx2 showed lower level of plasma triglyceride, but SSEx1 had significance only. SSEx1, SSEx2 showed little lower level of plasma HDL-cholesterol, LDL-cholesterol, total cholesterol, but had no significances. 6. Concerning the levels of plasma glucose, insulin and leptin, SSEx1 and SSEx2 showed littele changes with those of Lean Control, Obese Control. 7. The leves of Plasma AST, AST, ALT, free fatty acid, BUN, creatinine were in the physiological range at 4 groups all;Lean Control, Obese Control, SSEx1, SSEx2. Conclusions: These results showed SSEx1 can be used as therapeutic agent for Obesity and metabolic syndrome caused by long-period high fat diet.

      • 소생 후 혼수환자의 예후평가 지표로서의 자기공명 영상소견

        Purpose: The prognosis of comatose survivors after cardiac arrest is predicted by various methods such as neuralagic examination, magnetic resonance spectroscopy (MRS), biochemical markers, and somatosensory-evoked potentials (SSEP). The objective of this study was to investigate differences in magnetic resonance imaging (MRI) findings between good(CPC 1-2) and poor (CPC 3-5) outcome for comatose survivors after cardiac arrest. Methods: We retrospectively studied the cases of 19 comatose survivors after cardiac arrest who underwent MRI between March 1, 1997 and August 31, 1999. All MRI readings were performed by a neuro radiologist blindly and best CPC during 6 months after ROSC was used as final outcome. Results: Sixteen of the 19 patients had abnomal findings on the T2 weighted images: hippocampal infarct in 13 (68%), basal ganglia/thalamic infarct in 9 (47%), cortical infarct in 7 (37%), cortical watershed infarct in 6 (32%), and cerebellar infarct in 5 (26%). Among 6 patients with good outcome (CPC 1-2), 4 patients (67%) had hippocampal infarct, 1 (17%) basal ganglia/thalamic infarct, 1 (17%) white matter infarct, and 1 (17%) cerebellar infarct. Among the 13 patients with poor outcomes (CPC 3-5), 9 patients (69%) had hippocampal infarct, 8 (62%) basal ganglia/thalamic infarct, 7 (54%) cortical infarct, 6 (46%) cortical watershed infarct, and 4 (31%) cerebellar infarct. Cortical and cortical watershed infarcts were only detected in patients with poor outcomes (p<0.05). Conclusion: On the MRI findings, comatose survivors after cardiac arrest were susceptible to hippocampus, basal ganglia/thalamus, cortex, cortical watershed, and cerebellar infarcts. Cortical and cortical watershed infarcts may be useful as a prognostic tool for comatose survivor after cardiac arrest.

        Gerbil의 전뇌허혈 모델에서 허혈증 경도 저체온에 의한 뇌의 Apoptosis 억제 효과

        Background: A brief episode of global forebrain ischemia produces selective and often extensive neuronal loss in several vulnerable brain structures. This cell death does not occur immediately, but is delayed for hours to days. This process is termed delayed neuronal death (DND). Recently, several reports have suggested that an apoptotic process may be involved in DND. The most effective treatment at present is intraischemic hypothermia. Thus, we designed this study to investigate whether intraischemic mild hypothermia could inhibit apoptosis following transient forebrain ischemia in gerbils. Methods: Twenty-four gerbils were divided into two groups based on intraischemic rectal temperature: 34℃(n=12) and 37℃(n=12). Two additional gerbils underwent a sham operation. Cerebral ischemia was produced by occluding both carotid arteries for 10 minutes. The DNA fragmentation in the gerbil hippocapal CA1 area was determined by using the TUNEL method, and the results for the normothermic and the hypothermic groups were compared at 1, 3, and 7 days following 10-min transient ischemia (n=4 for each time point, respectively). Results: 1. Percent dead hippocampal neurons were significantly decreased in the hypothermic group compared with the normothermic group at 1,3, and 7 days following transient ischemia (p$lt;0.05). 2. In the intraischemic normothermic group, TUNEL positive cells were first detected in the hippocampal CA1 at 3 days (1.9±0.6 cells/section), and the number was larger at 7 days following transient ischemia (127.8 ± 16.3 cells/section). 3. In the intraischemic hypothermic group, no TUNEL positive cells were detected in the hippocampal CA1 at 1,3, and 7 days following transient ischemia. Conclusion: The data suggest that delayed neuronal death following transient ischemia is, in part, apoptotic and that intraischemic mild hypothermia affords significant neuronal protection and prevents DNA fragmentation.
