        It has been generally agreed that most humeral shaft fractures can be treated nonoperatively with anticipated good results. Recently improved results have been reported after internal fixation of the humeral shaft fracture and followed by early motion of elbow and shoulder. Internal fixation of fractures of the distal portion of the humeral shaft is problematic. Authors carried out internal fixation with plate and screws in 32 patients between January 1993 and December 1997. We analyzed the operative results of the internal fixation of fractures of the distal humeral shaft which was follow-up from 10 months to 16 months. The results were as follows : 1. Of the 32 cases, 21 cases(65.6%) were male and 11 cases(34.4%) were female. 2. Of the 32 cases, 18 cases(56.3%) were due to traffic accident, 7 cases(21.9%) fell down from a height and 5 cases(15.6%) slipped down. 3. According to AO/ASIF classification, type A(simple) were 13 cases(40.6%), type B(butterfly fragment) were 10 cases(31.3%) and type C(comminuted) were 9 cases(28.1%). 4. There were 9 cases(28.1%) of associated injuries; 2 cases were the fractures of the clavicle in ipsilateral, 2 cases were the fractures of the tibiafibula in ipsilateral and other 5 cases. 5. The complications such as limitation of elbow motion(4 cases), soft tissue infection(3 cases), delayed union(1 case), angulation(1 case) and iatrogenic radial nerve palsy(1 case) were developed. A functional range of motion in the shoulder were regained in all cases. On the basis of these results, displaced fractures of the distal portion of the humeral shaft can be managed successfully via a internal fixation with plate and screws to the distal humerus.

        Condylar fractures of the femur are uncommon, usually caused by direct violence to the region of the knee. One or both condyles of the femur may be fractured, with the fracture line extending into the knee joint causing hemarthrosis. We are reporting two cases of the femoral cleavage intercondylar fractures in which a vertical fracture lines are present without separation of the condylar one fragments with a review of the literatures and the etiology, treatment, and prognosis of this injury are discussed.

        증상있는 외측 원판형 연골의 관절경적 치료

        Purpose : Retrospective study was performed about the incidence, type and pattern of tears, and clinical results after arthroscopic management for the symptomatic discoid lateral meniscus. Materials and Methods : 54 cases in 49 patients who were managed arthroscopically and followed up for minimum one year for the symptomatic discoid lateral meniscus from Sept. 1993 to Aug. 1998 were included. Mean age at the time of surgery was 30.9 years(ranged, 12-63), most commonly in the second decade(42.5%) and mean duration of symptoms was 22.2 months. Females were more commonly affected than males(25 males and 29 females), and right knees were involved in 25 cases, left knees in 19 cases and bilateral in 5 cases. Most common symptoms was pain in 83.3% and snapping sound in 43.3%. The cause of symptoms was mostly related to previous trauma or sports injury in 66.7%, but none in 18 cases(33.3%). Discoid meniscus was classified according to the Watanabe’s classification system and tears were classified as a transvere, longitudinal, horizontal with cystic degeneration, peripheral, wear in the inferior surface and complex. All the partial meniscectomies were done by a piecemeal fashion and the menisci were sutured in a vertical and horizontal way by inside-out or outside-in technique on the basis of day surgery. Postoperative regimens of the immediate passive and active ROM and quadriceps setting exercises were prescribed, and mean follow-up was 31.7 months(ranged, 12-62). Results : According to the Ikeuchi’s scale(1969), overall 83.4% of good or excellent clinical results were obtained and poor in 2 cases. Among 5 cases of retear, 3 cases showed excellent clinical results after the second arthroscopic treatment, and 1 case was fair, but 1 case progressed to arthrofibrosis which was rated poor result, and in another one case of poor result without retear, persistent pain was complained due to preexisting osteoarthrosis. Conclusion : Arthroscopic partial meniscectomy or reshaping is recommmended for the symptomatic discoid lateral meniscus tears in children and adults, but more long-term follow-up and its clinical results about the treatment method in children and adults will be needed.
