        북한 보건의료전략의 분석과 치과 분야에서의 적용 현황 고찰

        In preparing for the era of Korean reunification, it is essential to consider the integration of medical systems and human resources. While Korean dental practitioners are expanding their activities in various fields both domestically and internationally, there are many restrictions on the activities for improving the health of North Korean people due to political and historical reasons even nowadays. In addition, there is little is known about the current state of dental health in North Korea. We analyzed the reports published by the World Health Organization (WHO) and the Ministry of Public Health of North Korea prepared individually or in cooperation, and investigated the current status of the health care strategy applied to the dental field by conducting a full investigation of the 2018 『Rodong』newspaper. Based on the above, we tried to grasp the major health care strategies in North Korea and their application. Understanding the direction and status of North Korea"s health care system would be an important cornerstone for international cooperation and practical activities to improve oral health care of North Koreans in the future. And there is a need that studies should be steadily conducted in various methods to overcome the heterogeneity of the two Koreas in the long-term perspective.

        노년기 건강의 적, 노쇠(frailty)와 구강노쇠(oral frailty)

        With a dramatic increase in the older population, there is a growing interest in strategies for managing oral disorders in the elderly. This study aims to introduce oral frailty and propose dental and oral health services for older people. This study reviewed the literature on oral frailty and frailty in older people by searching academic databases, government documents, and related websites. There is a growing awareness that the recovery and maintenance of oral function help delay the onset of frailty and conditions requiring nursing care, thereby extending a healthy life expectancy. The Japanese Society of Gerodontology (JSG) proposed the concept of oral hypofunction. JSG defined oral hypofunction as a presentation of 7 signs or symptoms: oral uncleanness, oral dryness, a decline in occlusal force, decline in the motor function of the tongue and lips, decrease in tongue pressure, decline in chewing function, and reduction in the swallowing function. JSG suggested diagnosing oral hypofunction if the criteria for three or more signs or symptoms are met. More evidence should be gathered from clinical studies and trials in the Korean community care setting to develop better diagnostic criteria and management strategies for the Korean elderly. Better health in older people can be achieved by strengthening the dental and oral health provider’s accountability of healthcare systems. The evolving concept of oral frailty paves the way for effective dental care for the elderly in Korea.

        우리나라 노인의 구강건강 주요지표

        Korean society is experiencing a very rapid change in population aging. Oral diseases, including dental caries and periodontal disease, are usually cumulative and make oral health worse with age. Preventing tooth loss through the prevention and treatment of caries and periodontal diseases are essential for retaining oral function later in life. This study aimed to identify the trend in oral health status among elderlies over 65 years old, using major oral health indicators of Health Plan 2020. The fifth, sixth, and seventh Korea National Health and Nutrition Examination Survey (KNHANES V, VI, VII) were used in the present study to estimate the number of remaining natural teeth, the rate of 20 or more natural teeth, the rate of chewing difficulty, and the rate of oral health check-up of elderly. The numㄴber of remaining natural teeth increased from 15.6 in 2010 to 17.1 in 2015, and the rate of 20 or more natural teeth also increased from 45.8% in 2010 to 53.7% in 2015. The rate of chewing difficulty was similar but declined a little from 44.3% in 2010 to 42.9% in 2017. Finally, the rate of oral health check-ups of the elderly increased significantly from 12.2% in 2010 to 21.1% in 2017.

        함 요동에 따른 ISAR 영상품질 분석 및 산란점 위치 식별 연구

        표적의 요동이 발생하는 상황에서 2차원 inverse synthetic aperture radar(ISAR) 영상 형성시 관측시간동안 표적평면과 영상투영면의 부정합이 지속적으로 발생하게 된다. 두 평면사이의 부정합은 cross-range의 비선형성뿐만 아니라, downrange의 왜곡을 야기한다. 본 논문에서는 2차원 ISAR 영상 엔트로피를 이용하여 ISAR 영상품질을 분석하고, 표적요동정보를 이용하여 표적 입사방위각과 고각을 계산하여 영상투영면의 시간적 변화특성을 분석하였다. 또한, 표적의 radar cross section(RCS) 저감방안을 마련하기 위해 입사방위각과 고각을 이용하여 표적의 RCS 특성을 분석하고, RCS 분석결과에 따라 표적내 산란점 위치를 식별하였다. 그리고, time windowing 최적화 기법을 이용하여 관측시간동안 표적의 영상투영면이 최소한으로 변화하는 구간을 찾아 ISAR 영상분석을 수행하였다. Misalignments between the target plane and image projection plane occur continuously during the observation period when a two-dimensional (2D) inverse synthetic aperture radar (ISAR) image is formed while the target fluctuates. The mismatch between the two planes causes not only cross-range nonlinearity but also down-range distortion in the ISAR image. This study analyzed the quality of ISAR images via 2D entropy, and the target aspect and elevation angles were calculated using target motion information. To reduce the radar cross section (RCS) of the target, the RCS characteristics of the target were analyzed based on the aspect and elevation angle, and the scattering point position in the target was identified according to the RCS analysis results. In addition, upon application of the time-windowing optimization technique, ISAR image analysis was performed to determine the section in which misalignment between the target plane and image projection plane is minimum during the observation period.

        In situ 연구를 통한 치약의 연마도 평가

        Objectives. The aim of this study was to evaluate the effect of two different abrasive toothpastes on the dentine surface using an in situ method. Methods. The dentifrices used in this study had relative dentine abrasion (RDA) values of 30 and 100 when measured by a radioactive dentine abrasion method. They were analyzed using a double blind, randomized, split mouth, in situ study. Dental hygienists trained the subjects in correct tooth brushing techniques with intra-oral appliance using a powered toothbrush prior to the experimental demonstration. The subjects wore bi-lateral, lower appliances from 9 a.m. to 3 p.m. (excluding one hour of lunchtime) for 14 days. Brushing was performed 3 times a day (9 a.m., 12 p.m. and 3 p.m.) for 14 days. The human dentine specimens installed on the appliance were brushed with different tooth brush heads depending on the type of toothpaste, and 1 g of the toothpaste was applied to each specimen for 60 seconds. After 14 days, specimens were removed from the appliances and average dentine loss was measured by a non-contact profilometer. The average dentine loss of each dentifrice was compared by a paired t-test using SPSS 17.0 software. Results. Sixteen subjects were included in the main analysis. The average age of subjects was 23.7 (range; 21∼29). The dentine loss depth of the high-abrasive toothpaste (6.60 μm) was statistically higher than that of the low-abrasive toothpaste (4.23 μm) (p<0.05). Conclusions. The low-abrasive toothpaste group (RDA 30) showed a significantly lower average dentine loss depth when compared to the high-abrasive toothpaste (RDA 100), and this difference observed in vitro also appeared to exhibit valid results when applied to the in situ study.

        네트워크 치과의 사회문화적 역할에 관한 연구

        The purpose of this study was to explore the socio-cultural function of network dental system and suggest the improvement of limitations of the current network dental system format. The Paker E-system, which appeared in the early 20th century in the United States, was examined to identify the early stage of the network dental system. Next, large network systems in other areas were compared as examples to consider the current operational form of network hospitals and clinics. The network dental system shows its strength in securing profits for healthcare systems, but it has ethical issues in terms of publicness of medical establishments. When comparing network dental systems to medical, Korean medical and pharmacy networks, the dental network was identified as being in the middle of networks of other domains in terms of expertise, universality, and vertical relationships. Brand value is lower than other domains. This study suggests that the network dental system needs to specialize in the role of the Management Service Organization to strengthen their values. Institutional supports should be provided to perform public functions in the community.

        한국 공공구강보건의료의 정의 및 역할에 대한 구강보건의료분야 전문가 인식도

        Objectives: The importance of public healthcare has been further emphasized by the arrival of the era of super-aged societies. This study investigates the landscape among oral health professionals, focusing on the concept development of the public oral health care (POHC) and essential oral health care (EOHC). Methods: A questionnaire survey of oral health professionals was conducted at six associations or societies who have an interest in POHC from December 21 to December 29, 2022. Chi-squared and logistic regression analyses were adopted to identify significant differences between the responses according to general characteristics. Significant differences were considered at a P -value of 0.05. Results: A total of 100 oral health professionals (48 dentists and 52 dental hygienists) participated in this study. The results revealed that there is a need for improvement of the POHC and the establishment of concepts related to the POHC and EOHC. The agreement rate was 90%, 85%, and 86% for the responsible organization, the target object and field, and the concept in the definition of the POHC, respectively. In the case of the construction of the EOHC, the agreement rate was 91% for “Quality of life,” and 85% for “Life and safety.” Among the community oral health programs as the POHC programs, “Oral health education program” showed the highest agreement rate. In healthcare institutions that are capable of providing the POHC services, “Oral health center in the public health center” had the highest agreement rate. Conclusions: The POHC would be reasonable to define to ensure universal access to oral healthcare services for all citizens. In the case of EOHC, further research is needed to establish terminology and specific concepts in the future. This study could contribute valuable insights to the field of the POHC in an era of super-aged societies. As further research, more oral health professionals need to participate in the POHC-related policy and health care system.

        Consolidated Standards of Reporting Trials의 소개: 무작위 대조군 연구의 보고 지침

        Recently, there are lots methodologies to improve the quality of research in medical and public health fields. One of them is to checking a process of research with a reporting guideline before reporting paper. Especially, reporting guideline for randomized controlled trials (RCTs) is the consolidated standards of reporting trials (CONSORT). Therefore, this review describes the latest version of the CONSORT statement, CONSORT2010, including intent, background, and contents of this. The CONSORT statement comprises a checklist of essential items that should be included in reports of RCTs and a diagram for documenting the flow of participants through a trial. This report guideline aims at providing guidance to authors about how to improve the reporting of their trials. As consulting the CONSORT, authors could be strictly reported about research methods and results. From this review, we expect to elevate the quality of such research about dental hygiene and to establish the foundation for evidence-based dental hygiene.

        한국 서해 천수만 선현세 간월도 퇴적층의 퇴적환경

        한국 서해 천수만 조간대 퇴적체는 약 20m 두께에 달하며, 지난 빙하기동안 노출되어 형성된 부정합면을 경계로 상위의 현세 조간대 퇴적층(Unit M1)과 하위의 후기 플라이스토세 간월도층(Unit M2)으로 구성된다. 퇴적단위 M1은 간월도층을 하부층으로하는 부정합면 위로 니질의 상부 조간대층과 사니질 또는 니사질의 혼합 조간대층이 순차적으로 발달하는 상향조립화의 해침층서를 갖는다. 퇴적단위 M2 퇴적층은 그 두께가 약 14 m에 이르며, 퇴적후 노출에 의한 풍화의 정도에 따라 상부 산화대층과 하부의 비산화대층으로 구분된다. 전반적으로 퇴적물은 니질 또는 사니질 입자로 구성되며, 조수 리듬 퇴적구조, 플라저층리, 엽층리, 게 구멍 화석, 천해성 와편모조류 등과 같은 조수퇴적 기원의 증거들을 함유하고 있다. 이러한 결과들은 퇴적단위 M2의 간월도 퇴적층이 상대적으로 해수면이 높았던 선현세 간빙기 동안 퇴적된 조간대 퇴적층임을 지시한다. 한편, 퇴적단위 M2의 상부 3${\sim}$4 m는 퇴적 후 초기현세까지 계속되는 저해수면 동안 대기중에 노출되어 풍화 및 산화작용으로 인하여 퇴적물의 특성이 변질된 층으로 해석되며, 이는 서해 연안 퇴적층에서 현세와 선현세를 구분해주는 뚜렷한 층서적 건층으로 제시된다. The late Quaternary deposit of Cheonsu Bay, up to 20 m in thickness above the Jurassic granite basement, consists of two sedimentary units: an upper Holocene mud and sandy mud deposit (Unit M1), and a lower late Pleistocene sand and mud deposit (Unit M2; 'Kanweoldo Deposit&apos). Unit M1 is a typical Holocene tidal-flat deposit of Cheonsu Bay, showing a coarsening upward, retrogradational facies trend. This retrograding facies trend is probably due to a relative low sedimentation rate during Holocene transgression. Overlain unconformably by Unit M1, Unit M2 deposit reaches up to 14 m in thickness and is mainly composed of muddy sediment with yellow to gray color. This unit is characterized by a variety of tide-influenced signatures such as rhythmic bedding, flaser bedding, crab burrow fossil, marine dinoflagellate assemblage and authigenic glauconite mineral, indicating very similar depositional environment to those of Unit M1 deposit. It suggests that Unit M2 was probably accumulated under the tidal-flat environment during a pre-Holocene sea-level highstand. In particular, the uppermost 3-4 m of Unit M2 appears to have undergone subaerial exposure and subsequent weathering during the sea-level lowstand after deposition. Therefore, stratigraphic unconformity between Holocene and late Pleistocene sediments is highlighted by the desiccated and weathered surface of Unit M2.

        레이더 전자보호 성능시험을 위한 송.수신 간섭신호 제거 기법

        Jamming simulator has developed for the purpose of the test and evaluation on the electronic protection capabilities of the tracking radar onboard ship. This simulator has the capabilities to generate and radiate the jamming signals against the radar as well as those to receive, analyze and identify the radar signals at a real sea environment. The limited space of ship superstructure has led to the serious distortion caused by the ring around phenomenon that some sidelobes of the jamming beams were coming back to the receiving antenna. In this paper, we have proposed the methods to eliminate the ring around. First, we have inserted the groove metal screen between transmitting and receiving antennas. Second, we have used the PRI(Pulse Repetition Interval) tracking loop to control the switching timing of the input radar and the output jamming signal. Finally, we have demonstrated the performance and effectiveness of the proposed methods through the sea trial.
