Meningiomas are common, slowing growing, benign intracranial tumors arising from arachnoidal cap cells. They account for 13 to 26 percent of all intracranial neoplasms. Intracranial meningiomas extending to the middle ear are very rare, with fewer than 150 cases reported to date. Presenting symptoms of the middle ear meningioma may include hearing loss, aural fullness, tinnitus, dizziness and facial palsy. Otitis media with effusion (OME) secondary to Eustachian tube dysfuction has also been reported. In case of therapy resistant OME, temporal bone computed tomography and magnetic resonance imaging (MRI) examination should be considered. In this report, we describe a case of intracranial meningioma extending into the middle ear cavity and clinically presenting as a OME with a review of the literatures.
