Although human insulin is widely used, insulin allergy is still an important issue. Desensitization is one of the strategies to overcome insulin allergy, but there are few report concerning the mechanism of immune tolerance to insulin achieved by desensitization. Based on the previous studies regarding immunotherapy, it can be understood that changes in allergen specific antibody may be a probable mechanism. We report time-serial changes in insulin-specific antibodies after successful desensitization of type I hypersensitivity to insulin. A 68-year-old man visited our clinic due to dyspnea, loss of consciousness and whole body rash with erythema immediately after the injection of human insulin. The patients showed positive skin reactions to all available human insulins. Enzyme linked immunosorbent assay (ELISA) demonstrated that human insulin specific IgE level increased. We injected human insulin (aspart) to patient subcutaneously with gradual increase manners and finally achieved successful insulin desensitization without serious systemic reactions. Human insulin-specific antibody was measured 4, 8 and 12 weeks after desensitization, which showed interesting time-serial changes; specific IgE gradually decreased, while IgG1, IgG2 and IgG4 gradually increased. Doxorubicin, a chemotherapeutic agent of the anthracycline family, has commonly been used to treat a wide range of cancers including leukemia, Hodgkin`s lymphoma, and cancers of the breast, uterus, ovary and bladder. Hypersensitivity is a rare adverse reaction to anthracyclines including daunorubicin, idarubicin and epirubicin. Although cross-reactivity among chemotherapeutic agents of the anthracycline family is not completely established, these agents are not clinically recommended if a patient has show hypersensitivity reaction to any anthracyclines. Desensitization is an alternative treatment option to overcome hypersensitivity reaction to anthracyclines, but cases of anthracycline desensitization have rarely been reported till now. We report successful epirubucin desensitization in a breast cancer patient with doxorubicin hypersensitivity. (Korean J Asthma Allergy Clin Immunol 2009;29:127-131)
두부 CT 검사 시 테이블 높이에 따른 선량과 화질에 관한 연구

The height of the table should be considered important during computed tomography (CT) examination, but according to previous studies, not all radiology technologists set the table at the patient's center at the examination, which affects the exposure dose and image quality received by the patient. Therefore, this study intends to study the image quality exposure dose according to the height of the table to realize the optimal image quality and dose during the brain CT scan. The head phantom images were acquired using Philips Brilliance iCT 256. When the image was acquired, the table height was adjusted to 815, 865, 915, 965, 1015, and 1030 mm, respectively, and each scan was performed 3 times for each height. For the exposure dose measurement, optically stimulated luminescence dosimeter (OSLD) was attached to the front, side, eye, and thyroid gland of the head phantom. In the signal to noise ratio (SNR) measurement result, The SNR values for each table height were all lower than 915 mm. As a result of exposure dose, the exposure dose on each area increased as the table height decreased. The height of the table has a close relationship with the patient's radiation exposure dose in the CT scan.
