      • 주시안을 선택 수술한 외사시의 수술 성적

        본인 안과에서 단안에 수술받은 60예의 기본형 와사시 중 의도적으로 주시안을 선택 수술한 33예와 편위안에 수술한 27예에서 수술 후 10△ 내외의 외관상 만족할만한 성적을 얻은 것은 주시만을 수술한 경우는 87.9%, 편위안을 수술한 경우는 70.4%였으며, 특히 60△ 이상의 큰 사시각을 가진 15예 중 주시안에 수술한 경우 13예 중 12예 (92.3%)에서 단안 수술로도 외관상 만족할만한 성적을 얻었다. The authors had operated cases of exotropia patients on the monocular eye, 33cases on dominant eye and 27 cases on non-dominant eye. Results were obtained as follow: 1.Cases of post-operative final deviation is less than 10 prism diopter In dominant eye; 29 cases among 33 cases (87.9%) In non-dominant eye; 19 cases among 27 cases (70.4%) 2.Among 13 cases of large angle exotropia over 60 prism diopter which are surgical treatment was performed on the dominant eyes, 12 cases (92.3%) is less than 10 prism diorter in postoperative final deviating prism diopter. 3.Cases of the amount of postoperative corrected angle are more than 80% of preoperative deviating angel. In dominant eyes; 27 cases among 33 cases(84.8%) In non-dominant eyes; 15 cases among 26 cases(57.7%) 4.When same amount of recession and resection was done on dominant or non-dominant eye, the amount of corrected deviation angle in dominant eye surgery was much than in non-dominant eye surgery.

      • 고혈압성 안저변화와 생리적 제인자간의 상관관계에 관한 연구

        The author investigated the correlation among physiological factors that could affect the hyperten-sive retinopathy as a line in the chain of basic investigation to prevent and manage the hypertensive disorders. The study group composed of 130 people with arteriosclerotic or hypertensive retinopathy and 260 healthy people. The results obtained were as follows: 1. Grade I was the most common in hypertensive retinopathy as 65.4%, and the distribution by age and sex showed that grade I in 50-59 year old group was the most common in male and female as 20.0% and 15.4%, respectively. 2. Grade I was the most common in arteriosclerotic retinopathy as 66.1%, and the distribution by age and sex showed that grade I in 50-59 year old group was the most common in male and female as 35.0% and 40.0%, respectively. 3. In the realtionship between hypertensive retinopathy and blood pressure, Grade I was the most common n the grade of 150-159㎜Hg, systolic blood pressure and 100-109㎜Hg, diastolic blood pressure as 22.3% and 33.8%, respectively. 4. In the case group, the level of serum cholesterol(211.26±47.53㎜g/㎗), serum triglyceride (184.05±93.3㎜Hg/㎗), low density lipoprotein(125.22±48.98㎜Hg/㎗), atherogenic index 1(3.67±1.85), and atherogenic index 2(2.86±1.64), were higher than those in the control group, however, only the level of high density lipoprotein(51.88±11.95㎜Hg/㎗) was lower. 5. The significant correlation to the grade of hypertensive and arteriosclerotic retinopathy were found in systolic and diastolic blood pressure. 6. The odds ratio between the case group and control group in serum lipid showed the significant difference in more than 200㎎/㎗ of total serum cholestrol and more than 190㎎/㎗ of serum triglyceride as 3.59 and 2.48, respectively. 7. The odds ratio between the case group and control group in atherogenic and obesity index showed the significant difference in more than 3.0 of atherogenic index 1 and more than 30% of obesity index as 2.40 and 3.67, respectively.

      • 흰쥐 각막 상피세포의 재생 과정에 대한 c-fos와 PCNA를 이용한 면역조기 화학적 연구

        각막 상피층 결손 부위의 치유는 각막 윤부 간세포의 성숙, 분화에 의해서 이루어진다. 각막 창상 치유 과정을 이해하기 위해서는 각막 창상 후 각막 중심부와 각막 윤부 세포들의 시간 경과에 따른 형태학적 변화를 면역조직 화학 염색법으로 조사하는 것이 효과적인 방법이다. 따라서 저자는 흰쥐 각막 상피에 창상을 가한 후 세포분열 양상의 지표가 되는 PCNA(proliferating cell nuclear antigen)와 c-fos에 대한 면역조직 화학 염색법을 이용하여 창상 치유 과정을 형태학적으로 규명하고자 하였다. 결과로서 PCNA와 c-fos의 양성 반응 세포는 상피기저세포의 핵에 선택적으로 염색되며 창상 초기에는 각막 중심부에, 24시간 경과 후에는 각막윤부에 다량 관찰되어 일차적인 결손 부위를 메우기 위한 작업은 중심부 각막 상피세포가 담당하고 그 이후의 치유과정은 각막 윤부 상피세포가 담당함을 알 수 있었다. Corneal epithelial defect is healed by maturation and differentiation of limbal stem cells. To understand the healing process of the corneal epithelial defect, it is effective to evaluate the progressive morphologic change of the central corneal and the limbal basal cell using the immunohistochemical staing. The author attempted to prove the regeneration process of the corneal epithelial cell morphologically using immunohistochemical staining for PCNA and c-fos in wounded rat corneal epithelium. As a result, immunopositive cells were located evenly in corneal epithelial basal cell layer selectively in PCNA and c-fos. Immunopositive cells were stained in the central cornea in the early stage of wounding, and were stained in the limbal basal cells in 24hours after wounding. The author could identify that central corneal epithelium undertake healing of epithelial defect primarily, limbal epithelial cells undertake healing process of defect after that time.

        백내장 환자의 연령에 따른 수정체낭 Type IV 아교섬유의 분포와 수정체낭 상피세포 자연사 및 전자현미경적 변화에 대한 연구

        백내장 환자의 수정체낭에서 연령차이에 따른 아교질과 type Ⅳ 아교섬유의 면역화학반응 양상, apoptosis현상 및 수정체 상피세포의 전자현미경적인 변화를 관찰하여 다음과 같은 결과를 얻었다. Van Gieson염색소견은 수정체낭의 앞층과 앞아래층에서 짙은 염색반응이 있었으나 나이가 많을수록 반응이 감소되었다. Type Ⅳ 아교섬유 면역화학반응은 수정체낭의 앞층과 상피세포 주위에서 현저하게 일어났으며 나이가 많을수록 반응이 감소되었다. Periodic acid shiff-alcian blue반응의 변화는 수정체낭의 앞층에서 나이가 많을수록 현저하게 감소되었다. 세포자연사 현상은 수정체낭 상피세포의 핵내에서 일어났으며 나이가 많을수록 반응이 일어나는 세포의 수가 감소되는 경향을 보였다. 수정체 상피세포의 전자현미경적인 변화는 40대까지는 외측 세포막의 folding (이하 주름)과 세포질내 공포가 증가되고 골지복합체가 발달되었으나 60대에서는 부분적으로 돌기는 내지만 골지복합체는 관찰되지 않았다. 수정체 상피세포의 바닥막부분에서는 나이가 들수록 거친 섬유들이 수정체 상피세포와 수정체낭을 격리시키고 있었으며, 수정체낭의 중간층은 40대군은 특징적으로 과립상의 구조물들이 집단적으로 축적된 부분들이 관찰되었고 60대군에서는 선상의 섬유성 구조들이 나타났으며 짙은 전자밀도를 가진 과립의 수가 감소하였다. Age-related changes in the lens capsule and epithelium of cataractous patients, ranging from 20 to 70 years old, were studied by means of LM, immunohistochemistry, and TEM. The lens capsule was divided into four zones; the anterior, subanterior, middle, and basal zone. The van Gieson staining reaction for collagen was prominent at the anterior and subanterior parts of the lens capsule. The reaction was more decreased in the elder group than the younger group. The collagen type Ⅳ reaction was prominent at the anterior zone of the lens capsule and around the cell. The reaction was more decreased in the elder group than the younger group. 3. The Periodic Acid Shiff-Alcian Blue reaction for mucopolysaccharide was prominent at the anterior zone of the lens capsule. The reaction was more decreased in the elder group than the younger group. The Apoptotic reaction was prominent at the nucleus of the lens epithelial cell. In the elder the cataractous group, the number of the apoptotic cells was more decreased. The electron microscopic change of lens epithelial cells were characterized by the increase of lateral fold and the cytoplasm with various vacuoles and Golgi complex. In the basal part, lens epithelial cell protruded toward the lens capsule in the 20-year-old group. The basal part of the 40-year-old group was flattened and covered with the cytoplasmic processes of adjacent cells. In the 60-year-old group, the mass of rough filaments separated lens capsule and the basal part of the lens epithelial cell. The electron microscopic change of the middle part of lens capsule was characterized by the aggregation of electron dense materials in the 40-year-old group, and the appearance of filamentous materials and the decrease of electron dense granules in the 60-year-old group.
