Vol.31 No.3

A 38-year-old Korean man without any other history was diagnosed with myelopathy due to a spinal dural arteriovenous fistula. Following embolization and high-dose steroid therapy, the patient was treated with complex Korean medical therapies including acupuncture, pharmacopuncture, electro-acupuncture, Chuna manual therapy, herbal medicine, and cupping therapy in addition to conventional treatment. To assess the patient’s improvement, international standard for neurological classification of spinal cord injury, Korean version of modified Barthel index, functional independence measure, spinal cord independence measure III, walking index for spinal cord injury II, modified Ashworth scale were used. After treatment, the muscle strength and sensory function of the lower extremities were improved, and the spasticity was reduced, resulting in a rapid improvement in performance of daily activities. These results suggest that complex Korean medical therapies may be effective for myelopathy, and further clinical studies are needed to clarify their effects.

Objectives This study aimed to evaluate the effectiveness of postoperative Korean rehabilitation treatment for patients with musculoskeletal disorders. Methods The medical records of patients undergoing Korean medical rehabilitation after a musculoskeletal surgery, from January 1, 2016, to December 31, 2019, were analyzed retrospectively. The effect of treatment was evaluated by using a numeric rating scale (NRS) and questionnaire for each surgical site. When the same patient was re-admitted after discharge, it was treated as the same case if it was within 7 days from the discharge date; otherwise, it was treated separately. If the period of hospitalization was within 7 days or the chief complaint was not related to musculoskeletal surgeries, the case was excluded. Results In total, 480 cases were included in this study. Acupuncture (99.8%), moxibustion (95.6%), cupping (97.5%), Chuna manual therapy (81.5%), Korean medical physiotherapy (61.0%), Daoyin exercise therapy (26.3%), and herbal medicine (87.7%) were used as the Korean medical rehabilitation treatments. A significant improvement was noted when comparing the NRS and questionnaire results for each surgical site at the time of admission and discharge. Conclusions Korean medical rehabilitation can be effectively used for patients who have undergone musculoskeletal surgeries, to relieve pain and enable returning to daily activities. However, further research with a high level of evidence is necessary to support this finding. (J Korean Med Rehabil 2020;30(3):151-162)

The aim of this study was to propose our rehabilitation protocol involving Korean medicine for patients with patellar fracture and report its effectiveness. Further, for the same patellar fracture patients, we created different protocols for those who underwent surgical treatment and those who required conservative treatment. Among patients who received inpatient treatment at our hospital for three years from August 2017 to August 2020, we included three patients who underwent surgical treatment for patellar fracture and one patient who required conservative treatment for patellar fracture. The degree of improvement of the patients was evaluated using physical examination findings of the knee joint, numeric rating scale, pain disability index, Lysholm knee score, and walking state. After each treatments, we observed that the range of motion of the knee joint, numeric rating scale, pain disability index, Lysholm knee score, and walking state improved in the patient groups. Even with the same patellar fracture, we observed that different rehabilitation treatments involving Korean medicine should be applied to patients who underwent surgical treatment and those who required conservative treatment. However, further studies are needed to design a rehabilitation protocol involving Korean medicine. (J Korean Med Rehabil 2020;30(4):203-217)

Objectives To investigate the trends in clinical study on rheumatoid arthritis in all academic fields in Korea. Methods We searched seven Korean web databases for clinical studies published from 2008 to 2021. All identified studies were reviewed and classified by the year of publication, academic field, and study design. Results A total of 347 clinical studies were selected. An average of 26.7 studies were published each year. Regarding the distribution of studies by academic fields, 309 studies were published in the medical field and 17 studies were published during the same period in Korean medicine, and 21 studies were published in other fields. The distribution of studies by study design was as follows: 8 studies, meta-analyses; 5 studies, systematic reviews; 16 studies, experimental studies; and 318 studies, observational studies. Conclusions Multidisciplinary access and management are required for the early diagnosis of rheumatoid arthritis, effective management of disease progression, and prevention of rheumatoid joint deformities. Additionally, study design with high-level of evidence on the intervention of rheumatoid arthritis are warranted. (J Korean Med Rehabil 2021;31(3):19-29)

Objectives The purpose of this review was to analyze the trend of clinical research related to fibromyalgia and to suggest the direction of intervention Methods We reviewed the clinical research by searching seven Korean web databases and classified the studies by the year of publication, academic field, and study design. Results A total of 75 clinical studies were selected. Except for 2002, more than one study was annually published; however, the number was below 10. In the academic field, there were 34 in the medical field, 21 in the Korean medicine field, and 15 in the nursing science field. By study design, there were 62 observational studies, mostly case reports and case-control studies. There were 13 experimental studies, including non-random control studies and one group pre and post-test design studies. Conclusions Fibromyalgia-related clinical research requires a greater number of studies, and a study design with a high level of evidence should be conducted along with various observational and intervention studies. Additionally, multidisciplinary and integrated intervention plans and research are needed.

Objectives This study aimed to analyze the research trends in clinical studies on the traditional Korean medicine used for cancer treatment between 2011 and 2020. Methods We searched 7 Korean web databases (Research Information Sharing Service [RISS], Oriental Medicine Advanced Searching Integrated System [OASIS], Koreanstudies Information Service System [KISS], Korean Medical database [KMBASE], DBpia, Korea Institute of Science and Technology Information [KISTI], and KoreaMed) and classified collected papers dated between 2011 and 2020 by the year of publication, publication journal, study design, type of cancer affecting the patients, interventions used, evaluation system used, and combination with conventional medicine. Results A total of 167 papers were collected. More than 10 papers were published annually. The papers were mainly published in the Journal of Korean Traditional Oncology (28.7%). The predominant type of the study was case study (67.7%). The main type of cancer was breast cancer, affecting 262 patients. Most of the studies reported using multiple traditional Korean medicine treatments (55.1%). Eighty-nine studies reported using clinical laboratory tests for evaluation. Most of the patients took conventional medicine simultaneously (608 patients). Conclusions Research related to Korean medicine for cancer patients is at a rudimentary level. Therefore, for further clarity, high evidence-based researches, including randomized controlled trials and cohort studies, should be conducted. (J Korean Med Rehabil 2021;31(4):75-86)
