        Due to heterogeneity in Data and Processes, healthcare systems are facing the challenge of interoperability. This heterogeneity results in different healthcare workflows of each individual organization. The compatibility of these heterogeneous workflows is possible when standards are followed. HL7 is one of the standards that is used for communicating medical data between healthcare systems. Its newer version V3 is providing semantic interoperability which is lacking in V2. The interoperability in HL7 V3 is only limited to data level and process level interoperability needs to be catered. The process level interoperability is achieved only when heterogeneous workflows are aligned. These workflows are very complex in nature due to continuous change in medical data resulting in problems related to maintenance and degree of automation. Semantic technologies plays important role in resolving the above mentioned problems. This research work is based on the integration of semantic technology in HL7 V3 standard to achieve semantic process interoperability. Web Service Modeling Framework (WSMF) is used for incorporating semantics in HL7 V3 processes and achieves seamless communication. Interaction Ontology represents the process artifacts of HL7 V3 and helps in achieving automation.

        Accuracy of mappings is the key for achieving true interoperability among different healthcare systems. The initial step towards interoperable healthcare systems is compliancy with healthcare standards (HL7, openEHR, CEN 13606). Ontologies for these standards are developed that require ontology matching to generate generalized ontology mappings. Organizations conform to specific concepts of different standards based on their requirements. This step is called as conformance claims and is based on Personalized-Detailed Clinical Model. It invalidates some of the generalized mappings because of non-conformed concepts and leads to the necessity of the proposed technique of customized ontology mappings. These customized ontology mappings compliment the generalized ontology mapping to increase the level of accuracy of mappings and thus achieving data interoperability. The proposed system ensures quality of care to patients by timely delivery of healthcare information.

      • 만성 질병환자를 위한 CDSS 를 적용한 PHR 시스템

        With the advance of Information Technology (IT) and dynamic requirements, diverse application services have been provided for end users. With huge volume of these services and information, users are required to acquire customized services that provide personalized information and decision at particular extent of time. The case is more appealing in healthcare, where patients wish to have access to their medical record where they have control and provided with recommendation on the medical information. PHR (Personal Health Record) is most prevailing initiative that gives secure access on patient record at anytime and anywhere. PHR should also incorporate decision support to help patients in self-management of their diseases. Available PHR system incorporates basic recommendations based on patient routine data. We have proposed decision support service called “Smart CDSS” that provides recommendations on PHR data for diabetic patients. Smart CDSS follows HL7 vMR (Virtual Medical Record) to help in integration with diverse application including PHR. PHR shares patient data with Smart CDSS through standard interfaces that pass through Adaptability Engine (AE). AE transforms the PHR CCR/CCD (Continuity of Care Record/Document) into standard HL7 vMR format. Smart CDSS produces recommendation on PHR datasets based on diabetic knowledge base represented in shareable HL7 Arden Syntax format. The Smart CDSS service is deployed on public cloud over MS Azure environment and PHR is maintaining on private cloud. The system has been evaluated for recommendation for 100 diabetic patients from Saint’s Mary Hospital. The recommendations were compared with physicians’ guidelines which complement the self-management of the patient.

      • KNOWLEDGEBUTTONS IN HEALTH SYSTEMS

        Infobutton is an important concept from long time in use and much has been done with respect to its standardization and context supplementation. The concept is to create contextual links to information resources from within the information systems usually health information systems. The need which has been realized by the authors of this paper is the augmentation of Infobuttons from the level of only information links to the level of knowledge links. The authors proposed the concept of knowledge links named as “Knowledgebuttons” which complements the concept Infobuttons. It adds further capabilities of getting knowledge to the users instead of just connectivity to information resources. The better representation of the information retrieved with Infobuttons is the first and foundation step to achieve the goal of getting knowledge. This paper discusses about the concept and applicability of Knowledgebuttons in health information systems. It is envisioned that this concept will add to the overall quality of patient care. Both physicians and patients can benefit from this technique as per their needs. Physicians can help in patient diagnosis and treatment critical decisions while patients can educate them to know more about their health conditions by studying the right knowledge at right time. Knowledgebuttons are able to create a true learning environment for the users while using health information systems.

        The need of Clinical Decision Support System (CDSS) in healthcare setup is increasing day by day. EHR Meaningful Use advocates CDSS as an important component of EHR/EMR systems. CDSS can be ranged from a simple to a very sophisticated system. The more complex CDSS systems need more attention to develop because of many reasons including its Knowledge Base (KB) structure/maintenance/evolution, inference capabilities and usability. Above all the KB maintenance and evolution is very crucial and important from the perspective of useful decision capabilities. Also the richness of the KB is important to cover the decision gaps handling a particular situation in the course of patient care. It cannot be expected from the clinicians to remember everything in regard to patient diagnosis and treatment. Similarly, it is also crucial for clinicians to keep themselves updated with the new research in the area. That is the reason they frequently require accessing to the online knowledge resources. Literature proved that online knowledge resources are capable providing answers to questions that might not be answered rely only on clinician wisdom and experience. This paper provides the theme of meaningful utilization of online knowledge resources in the context of diagnosis and treatment process for cancer patients more specifically Head and Neck cancer.
