        지역사회 거주 노인의 근감소증 유병률, 인지기능과의 관계 및 중재연구 동향:

        본 연구는 국내 외 지역사회에 거주하는 노인의 근감소증과 인지기능에 관한 연구 동향을 파악하기 위해 수행된 체계적 문헌 고찰이다. 본 연구의 선정 논문은 국내 외 문헌 중 지역사회에 거주하는 노인을 대상으로 근감소증의 유병률 또는 근감소증과 인지기능의 상관관계를 조사하였거나, 근감소증 완화 목적으로 시행된 중재 연구로, 총 43편의 논문이 포함되었다. 국내 외 근감소증의 유병률은 총 25편의 논문에서 2.5-42.4%의 다양한 범위로 보고되었으며, 인지기능과의 관계를 분석한 6편의 논문 중 4편에서 근감소증과 인지기능 저하가 유의한 양의 상관관계로 보고되었다. 근감소증 개선을 위한 중재 연구 12편에서 근육량, 근력 또는 신체 기능 향상에 대한 중재 효과를 확인하였으나, 근감소증 결과지표로 인지기능 평가를 시행한 논문은 없었다. 지역사회거주 노인을 대상으로 근감소증을 예방 및 개선하고 효과를 평가하기 위해서는 표준화된 진단을 적용한 지역 사회 내 근감소증 감시체계와 신체기능 뿐만 아니라 인지기능 평가를 포함하는 포괄적이고 지속적인 근감소증 관리체계가 필요하다. 또한 지역사회 기반 근감소증 예방을 위한 근거를 생산하기 위해 지속적인 중재연구의 개발 및 적용이 요구된다. This systematic review was performed to investigate relationship between sarcopenia and cognitive function among community-dwelling older adults. Studies which reported prevalence rates of sarcopenia in community-dwelling older adults, relationships between sarcopenia and cognitive dysfunction, or effective interventions in alleviated symptoms of sarcopenia were selected. Of 43 studies reviewed, prevalence rate of sarcopenia ranged from 2.5-42.4% in 25 studies, and 4 studies revealed a significantly positive correlation between sarcopenia and cognitive dysfunction. Among 12 randomized-control trials (RCTs), exercise intervention was used in 2 studies; nutrition intervention in 6; and 4 studies included both the exercise and nutrition interventions. All interventional studies reported positive effects on muscle mass, muscle strength, or physical function. However, only 6 studies met the high quality criteria of Risk of Bias, and there was no study which evaluated cognitive function as an outcome measure. Therefore, this study draws attention to the lack of high-quality RCTs and exclusion of cognitive function as an outcome measure in the research of sarcopenia. A comprehensive and sustained system including standardized sarcopenia screening, assessments of cognitive functioning, and evidence-based intervention programs is needed in order to prevent and manage sarcopenia amongst older adults in the community

      • TPP 농업부문 협상결과와 시사점

        The 12-country’s Trans-Pacific Partnership (TPP) negotiations were finally concluded on 5 October 2015 through 19 times of official meetings as well as several ministerial meetings and informal meetings. The text of TPP agreement consists of 30 chapters including elimination of tariff and non-tariff barriers. In addition, it includes provisions on Rules of Origin, Trade Remedies, Sanitary and Phytosanitary Measures (SPS), Technical Barriers to Trade (TBT), Investment, Cross Border Trade in Services, Financial Services, Telecommunications, Electronic Commerce, Government Procurement, Competition Policy, State-Owned Enterprises, Intellectual Property, Labour, Environment, Cooperating and Capacity Building, Competition and Business Facilitation, Development, Small and Medium-sized Businesses, Regulatory Coherence, Transparency and Anti-Corruption, Administrative and Institutional Provisions, Dispute Settlement, Exceptions etc. Since TPP negotiations were focused on opening markets of all goods, TPP was a pressing issue for Korea to which the agricultural industry responds very sensitively. Most of the TPP participants including the US have put efforts to consider internal sensitiveness using various ways. Thus, when we enter TPP, it is expected that Korea will secure a certain level of sensitiveness. On the other hand, it would be not easy to present its bargaining power if Korea participates alone. Moreover, Korea has already concluded Free Trade Agreements (FTA) with most of the TPP participants and the level of tariffs varies depending on the FTAs. This may cause higher level of tariff reduction. Therefore, it is needed to establish a strategy which can secure Korea’s sensitiveness enough by referring to negotiation results among the TPP members. Given that there are countries which have intention to participate in TPP negotiations such as Taiwan, the Philippines, Colombia, Thailand etc., the second round would be better for Korea to negotiate with new members. Besides, since TPP has set stronger and stricter standards on Rules than existing WTO level, it is expected for Korea to change its overall system. Thus, Korea needs to analyse impact of TPP and prepare more throughly.

      • Post-FTA 농업통상 현안 대응 방안

        Background of Research The world economy had been pursuing multilateral systems under the WTO. However, regionalism has intensified with rapid expansion of free trade agreements since 1990. On the other hand, the international trade order is recently changing from bilateral FTAs to Mega FTAs. TPP, which was concluded at the end of 2015, is an agreement, intended for economic integration of 12 countries, and by TPP, negotiation of RCEP is also accelerating. Korea has concluded 15 FTAs with 52 countries as of July 2016 and bilateral FTAs with major countries are about to be completed. Therefore, it is time to review negotiation strategies on TPP and RCEP as well as trade policy of Mega FTA. This study reviews issues to consider when negotiating TPP and RCEP, drawing negotiation strategy. Also, it aims to suggest trade strategy which can minimize impact on the agricultural sector and utilize Mega FTAs. Method of Research Literatures regarding trends of international trade order and discussion on Mega FTAs were reviewed to study background of new international trade order. Also, current trade issues which Korea is facing were analyzed through the results of TPP negotiation, discussion and issues of RCEP, previous studies and statistical data. Regular forums and seminars were held to discuss current agricultural trade issues and the implications drawn from the forums were reflected in this study. The agricultural trade forum was formed of experts from agricultural and non-agricultural sectors and it was used as a door to collect extensive opinions. Research Results and Implications It is expected that the international trade negotiation will be developed, mainly by Mega-FTA and certain agendas in future. In particular, TPP established regulations on new trade issues over free trade of goods, including regulations which have not been treated in the WTO. Korea is required to have attitude of careful access since it has to adopt the existing agreement as a late comer. Most of all, conflicts between agricultural and non-agricultural sectors concerning market opening are likely to occur, so responses need to be prepared to minimize social conflicts and costs considering domestic and overseas requirements. In particular, we need to prepare for additional market opening of sensitive products such as rice and measures on reinforcement of SPS regulations. Since Korea opened ASEAN and Chinese markets at a low level through FTAs, RCEP is likely to have bigger effect than that of TPP. Particularly, because there are a lot of agricultural exporters in RCEP, we need to prepare for responses to market access and regulations. Through Mega FTA, Korea will be able to create added value in terms of backward participation which exports manufactured products using the materials from overseas. Not only expanding exports of processed food with customs benefit of materials imported through Mega FTAs, but we also need to make it as an opportunity to export agricultural materials to the member countries.
