        Carrageenin에 의하여 유발된 백서 슬관절염의 활액막세포에 관한 형태학적 검색

        To investigate the morphology of the synovial lining cells, synovitis was induced by carrageenin injection into the rat knee joint cavities. Synovial membranes were excised at 1, 3, 5, 7 and 14 days, and histologic, electron microscopic, histochemical (periodic acid Schiff: PAS, toluidine blue), and enzyme histochemical (acid phosphatase: ACP, nonspecific esterase: NSE and endogenous peroxidase) studies were performed. The results are as follows: Carrageenin induced synovial membrane hypertrophy with synovial cell proliferation and granuloma formation. The proliferated synovial lining cells and macrophages in the granulomatous lesion had round to oval nuclei and large, plump cytoplasm with many phagocytotic materials and vacuoles. Electron microscopically, these cells had small number of granular endoplasmic reticulum and many lysosomes, phagosomes and vaculoes. Mitotic figures were observed at early stage of experiment. PAS and toluidine blue stains showed strongly positive reaction in the cytoplasm of the proliferated lining cells and macrophages in granulomatous lesion. ACP and NSE activities were strong positive in the cytoplasm of the proliferated synovial lining cells and macrophages in the granulomatous lesion. But endogenous peroxidase stains were negative in all prolifeative lining cells and macrophages in granulomatous lesion. Conclusively, carrageenin-induced synovitis showed proliferation of synovial lining cells and granuloma formation in deep layer. The macrophages, which consisted of the lesions and have active phagocytic function, were speculated to proliferate by mitosis of superficial synovial A cells and histiocytes in the deep layer of the synovial membrane.

        와상 환자에 대한 역학적 관찰

        Abrupt increase of the height transport and public traffics due to development of the domestic industry and the automation of the farming tools due to argicultural mechanization increased the traumatic patients annually. And, the loss of the life, working capacity, means and following direct and indirect enormous medical fee payment became a significant social problems. We have made a retrospective study of 726 cases of the traumatic patients who were visited to our department of Emergency Room of Chonbuk National University Hospital from Sep. 1987 to Aug. 1988 after traumatic accidents. It was considered necessary to find out some preventive measures whIch could obtained from the epidemiologic facts. In this study, we have find out such facts; 1. Second decade of age group showed the highest numbers of accident victims, as much as 22.2 % and children group showed 17.5 %. 2. Skull of the body showed the highest frequency of trauma involved region. as much as 36.8 % according to the regIonal distribution in the body and lower extremities showed 24.6 %. 3. On day, highest occurence was during evening (18 Hr-21 Hr) (21.3 %), and afternoon (I5 Hr-18 Hr) (17.8%) coinciding with the time for home bound traffic after work. 4. Casualities showed maximum occurence during the Autumn (34.7 %) and Spring (27 %) coinciding with picnic season. 5. In the kinds of vehicles, the private taxi showed the increased accident rate, as much as 22.3 % and farm machine group showed the 4.6 %.

      • CDM-2.0을 이용한 서울시 북부지역의 대기질 예측

        This study involves air quality dispersion predictions and comparison of the predicted values with actually measured data in terms of TSP, SO₂,and NO₂concentration in north Seoul area. The prediction was made by utilizing CDM-2.0 air quality simulation computer model. It predicts long term air pollutant concentration in urban area. The TSP concentrations observed ware high at the MI-A dong, SSANG-MUN ㅜ dong and NO-WON station. The SO₂was high at BANG-HAK dong and NO-WON station, NO₂was high at DO-BONG station, although the lower concentration were observed at the WOO-IE dong, BUN dong and SUYURI for itms. The correlation coefficient of CDM-2.0 were TSP 0.4665, SO₂0.6202 and NO₂0.6421. In conclusion CDM-2.0 demonstrates more suitable to SO₂, NO₂than prediction TSP in subject area.

        그라프 연성 고정환자에서 유연성(Flexibility)과 그라프 불안정 정도(Graf`s Instability Degrees, GID)

        Study Design : To assess the changes of the flexibilities and Graf's instability degrees(GID) after Graf soft stabilization on the instability of the low back disorders, revies the dynamic films before and after low back operation. And compare flexibilities and GID with clinical results(JOA Scores, etc). Object : To determine whether GID has a clinical significance or not. These results should explain what kinds of patterns of segmental motions are developed in the course of the new adaptation, new dynamic stabilization, after the operation. Methods : In the patients who treated their back problems with Graf system as a stabilization procedure, 15 patients(instabiliy 7, spondylolysis 4, spondylolisthesis 4) were selected. they have more biomechanical problems than neurological problems, and passed over I years after the operation. Check the flexibilities & GID with computer assisting system and make a comparative studies with JOA scores & patient's self evaluation scores before and after the operation. Results : In the flexibility degrees, instability, spondylolysis and spondylolisthesis show decreased flexibility, but increased JOA scores. In the GID, instability and spondylolysis show increased GID after the operation. These findings suggest the segmental displacementIangulation and translation) was developed in the course of the new adaptation. But spondylolisthesis show decreased GID in the course of the dynamic stabilization. Conclusion : As a indicator of Graf system application and criteria of the prognosis after the operation. GID has not a clinical significance, but has a geometric advantages. Decreased flexibility is the one of the possible factors which improve the clinical symptoms, but it need multi-center clinical and biomechanical studies.

      • 요천각의 통계적 고찰 : 외래환자 중심으로 On the Basis of Out-Patients

        Low back pain is a common disease in the industrial countries, and 60-80% of population had experienced low back pain, generally. We made a statistical analysis of the factors involved in stability of lumbosacral joint in 960 patients with low back pain who had been visited the department of Orthopendic Surgery of Chonbuk National University Hospital between March 1985 to September 1988 in comparison to angles of normal group. In this study, we made the following conclusions. 1. The results of various Lumbosacral angles are followings. Ferguson Angle 31.7′ Mithchell Angle 14.3′ Promontorium Angle 135.7′ L5 Horizontal Angle 15.3′ 2. Low back pain was common before the age of3(47.6%), especially most common in third decade(30.3%). 3. Ferguson Angle, Mitchell Angle and Promontorium Angle showed progressive instability with increased age but L5 Horizontal Angle was no specific correlation with increased age. 4. Comparison to Normal group of our country, low back pain patients showed mild increase of lumbar lordosis in L5 Horizontal Angle and Promontorium angle.       • 슬관절관절경을 이용한 반월상연골 절제술에 대한 임상적 연구

        Arthroscopy of the knee joint is an essential diagnostic and treatment procedures of the knee joint lesions because of its high diagnostic accuracy, high safety and simple procedure. We analysed 58 cases of knee joint arthroscopy which were performed at Chonbuk National University Hospital from October 1988 to December 1989. The results were as follows : 1. The knee joint lesions were most common in third decade(52.3%), and in second decade(21.2%). 2. The knee joint lesions were more common in male(77.6%) than in female(22.4%). 3. The diagnostic accuracy of clinical diagnosis of knee joint lesions based on arthroscopic finding was 51.7%. 4. In the clinical diagnosis, mensical tear was 85.5%, but arthroscopically confirmed meniscal tear was 34.5%. 5. In meniscal lesions, lateral meniscus tear was more common(63.2%) than medial meinscus tear, and discoid meinscus was more common in lateral menisus. 6. Arthroscopy is very useful and essential not only to confirm the diagnosis but also to treat the knee joint lesions.
