임상 ; 산전 황산마그네슘(MgSO4) 노출에 따른 초극소 저체중 출생아의 예후

Objective: To determine whether magnesium sulfate exposure is associated with a reduced risk of perinatal complications in extremely low birth weight infants (ELBWIs). Methods: Total 351 inborn extremely low birth weight infants admitted between January 2000 and December 2008 to Samsung Medical Center were divided into the prenatal MgSO4-exposed group (n=76), and control group (n=275), and were retrospectively analyzed. Multiple demographic, clinical characteristics and neonatal complications were compared. Results: Prenatally MgSO4-exposed ELBWIs had significantly higher gestational age, birth weight, and incidence of small for gestational age. In neonatal complications, retinopathy of premature (ROP) treated with laser was less common in MgSO4-exposed group. There were no differences between two groups in the incidence of respiratory distress syndrome (RDS), bronchopulmonary dysplasia (BPD), patent ductus arteriosus (PDA), PDA ligation, intraventricular hemorrhage (IVH), periventricular leukomalacia (PVL), necrotizing enterocolitis (NEC), and mortality. Conclusion: Perinatal exposure to magnesium sulfate was associated with decreased incidence of ROP treated with laser.

자원봉사센터 재난자원봉사 담당자의 직무요구-자원(JD-R)이 직무 열의에 미치는 영향연구

본 연구는 전국의 자원봉사센터에 근무하는 재난자원봉사 업무 담당자들을 대상으로 직무요구와 자원(JD-R) 수준을 파악하고 직무 열의에 미치는 영향을 실증적으로 분석한 것이다. 구조화된 설문지를 활용하여 온라인 조사를 통해 수집된 165명의 응답 결과를 분석한 결과는 다음과 같다. 첫째, 재난자원봉사 업무 담당자들이 인식하는 직무요구 수준은 평균 3.01로 보통 수준이나 역할모호성 요인(3.13)이 비교적 높았으며, 이들을 지원하는 직무자원은 3.15점으로 평균 이상이었지만 자기개발(2.81) 요인은 낮았다. 둘째, 직무 열의는 평균 2.98점으로 평균보다 낮은 수준이었으며 특히 재난자원봉사 업무에 대한 몰두(2.78)가 낮게 나타났다. 셋째, 재난자원봉사 업무 담당자들의 직무열의에 영향을 미치는 요인으로 직무요구에서는 ‘희생’요인이 부정적인 영향(β=-.235)을 미치고 있으며, 직무자원에서는 도구적 지원((β=.193)과 자기개발(=.358)이 긍정적인 영향을 미쳤다. 이러한 분석결과를 바탕으로 재난자원봉사 업무에 대한 직무분석을 통해 역할을 명확히 하고 직무에 필요한 도구적 지원뿐 아니라 담당자의 전문성 향상을 위한 교육 등 자기개발을 위한 지원 확대를 제언하였다. The purpose of this study is to identify job demands and resource levels (JD-R) for disaster volunteers working at volunteer centers across the country, and to empirically analyze their effects on job enthusiasm. The results of analyzing the responses of 165 people collected through an online survey using a structured questionnaire are as follows. First, the job demand level recognized by disaster volunteer work managers was average of 3.01, but the role ambiguity factor (3.13) was relatively high. The job resources to support them were 3.15 points, above the average, but the factor for self-development (2.81) was low. Second, the level of enthusiasm for the job(2.98) was lower than the average and in particular, the level of preoccupation with disaster volunteer work(2.78) was low. Third, as a factor influencing the job enthusiasm of those in charge of disaster volunteer work, the sacrificial factor had a negative effect(β=-.235) in job demands, and instrumental support(β=.193) in job resources. and self-development(β=.358) had a positive effect. Based on these results, the role of disaster volunteer work was clarified through job analysis, and support for self-development such as training to improve the professionalism of the person in charge as well as the instrumental support required for the job was suggested.
