이 연구의 목적은 한국인 젊은 성인의 얼굴뼈 CT를 3차원재구성하여 평균 턱뼈바닥각을 계측하여 알아보고 남녀 차이를 분석하고자 하였다. V-line이라 일컫는 정면에서 바라본 턱뼈바닥각을 기준으로 아래턱의 형태 분류를 하고 계측항목 간의 상관관계를 분석하였다. 연구재료로는 건국대학교병원 성형외과에서 제공한 195명의 남녀 성인 얼굴 CT (남자 97명, 여자 98명, 평균나이 25.9세, 범위 18~38세) 를 사용하였고 얼굴 CT 이미지는 소프트웨어를 이용하여 3차원재구성하였다. 3차원재구성한 얼굴뼈는 정면에서 본 턱끝에서 아래턱의 양쪽 몸통이 만나는 각도 (턱뼈바닥각)와 양쪽 턱뼈각에서 아래턱선으로 이어지는 각도 (턱모서리각)를 계측하고 턱모서리 전체길이와 턱뼈결합 높이를 계측하였다. 총 195명의 3차원이미지를 계측한 결과 두 가지 정면에서 본 아래턱각도와 턱모서리 전체길이, 턱뼈결합 높이는 남자가 더 크고 길었다(P<0.05). 아래턱각도에 따라 세그룹으로 유형을 분류할 수 있었는데 여자는 그룹 간의 대상자 분포가 비교적 골고루 분산된 반면 남자는 아래턱각도가 가장 큰 I그룹의 분포가 가장 많았다. 남녀 모두 각 그룹별로 턱모서리 전체길이와 턱뼈결합 높이는 통계적 차이가 없었지만 아래턱각도가 클수록 턱뼈결합 높이는 작아지는 상관관계가 있었다(P<0.05). 이 결과를 바탕으로 아래턱축소술 또는 양악수술 과정에서 임상의들은 좀 더 객관적이고 안정적인 수술계획을 잡을 수 있고 환자의 욕구에 부응하는 자료를 제공할 수 있을 것이다. The aim of this study was to analyze the difference of the measurements of mandible between young male and female using three dimensional reconstruction images derived from the facial bone CT images. Type classification of mandible was performed based on the angle of base of mandible and the angle of mandibular margin observed from the anterior view and the correlation among the measurement items was analyzed. One hundred-ninety five young adults facial CT were collected from the Konkuk University medical center (98 females, 97 males; mean age 25.9 years, range 18~38 years). Three dimensional images of the face were reconstructed using software from CT images of whole specimens. The angle of the base of the mandible, the angle of the mandibular margin, total length of mandibualr margin, and the height of mandibular symphysis were measured. All measurement items were larger and longer in male than female (P<0.05). Three types were classified according to the mandibular angle. In female, the subjects were evenly distributed among groups, but group I which had large mandibular angle was most common in male. The smaller the height of mandibular symphysis, the larger the angle of base of mandible or the angle of mandibular margin (P<0.05).
