        Purpose To minimize the radiation exposure during the computed tomography (CT) examination, the practical amount of radiation exposure was reviewed in terms of anthropometric characteristics of the examinees and the methods of exposure control - automatic and manual. Materials and Methods During 5 months from March to July 2006, radiation exposure data of 200 examinees who had either chest or abdominal CT examination were collected from 16 channel slice multidetector row CT (16 slice MDCT, equipment A) and 64 channel slice multidetector row CT (64 slice MDCT, equipment B) in Hwasun Chonnam national University Hospital. Data were analyzed in relation to examinee’s body mass index (BMI), age, sex and noise index. The difference of radiation exposure between automatic and manual exposure control were also analyzed respectively Results Of total 400 examinees (267 men and 133 women), the amount of radiation exposure was significantly correlated with BMI, body weight and the noise index. The noise index was positively related with body weight. In both equipments, the automatic exposure generally showed lower radiation exposure than manual exposure. The radiation exposure in abdominal examination in e quipment A, however, showed higher radiation exposure in the automatic exposure than in the manual exposure, although statistically insignificant. The exposure levels were higher in the automatic exposure in the 82 percent of abdominal examinees for the equipment A. Conclusion Generally speaking, the automatic exposure method are superior to the manual exposure method to minimize the amount of radiation exposure during the CT examination. But this is not always the case. This study suggest that all the exposure protocol for the CT examination be reviewed through practical evaluation at the actual settings.

      • 소아 Brain CT 검사에서 진정 (Sedation) 없이 Wide volume axial scan 과 보조기구를 활용한 검사방법의 효용성에 관한 연구

        목적 : 소아 Brain CT검사에서 진정을 시행하지 않고 volume axial scan과 보조기구 및 검사보조자의 사전 교육을 통한 검사 방법이 과거 시 행한 helical scan에 비하여 보다 더 효율적인 검사방법인지 알아보고자 하였다. 대상 및 방법 : 급성 외상 및 내과적 질환에 의한 의식혼미, 의식저하, 졸린 상태 등 실험기구를 적용할 수 없는 환아를 제외한 소아 남,녀 (평균연령 3세) 240명을 대상으로 volume axial scan, 보조기구, 사전교육을 적용한 실험군 (volume axial scan)과 보조기구 및 사전교육이 선행되지 않고 단순히 환자를 잡고 검사한 대조군 (helical scan)으로 나누어 두 집단을 비교하였고 검사시간, 검사 대기시간, 응급실체류시간,선량, 재촬영 건수 및 진정유무로 나누어 분석하고자 하였다. 두 집단 모두 환자를 잡고 검사하는 것을 전제로 필요시 진정 시행 유무를 달리하여 검사하였고 실험군은 실험기구 (보조기구, 사전교육)를 적용하였다. 결과 : 실험기구를 이용한 실험군이 그렇지 않은 대조군에 비해 검사시간은 95 % 단축되었고 검사 대기시간 및 응급실 체류시간은 각각 39%, 46 % 줄어들었다. 진정의 비율은 volume axial scan이 helical scan에 비해 건수로는 11건 감소하였고 비율로는 약 43 % 줄어든 수치를 보여줬다. 재촬영 건수는 volume axial 스캔이 helical 스캔에 비해 5건 감소하였고 비율로는 25 % 감소하였다. 하지만 p =0.360로 통계적 유의성은 없었다. Rando phantom으로 측정한 두개부 선량의 DLP는 axial 스캔이 helical 스캔에 비해 26 % 줄어든 선량분포를 나타냈고 집단선량은 axial 집단이 helical 집단에 비해 CTDI, DLP값이 낮게 나왔으며 두 집단간 p-value값은 0.002로 통계적인 유의한 차이가 있었다. 결론 : 소아 brain CT 검사에서 소아라는 특수성 및 제한점이 있음에도 불구하고 volume axial 스캔이 helical 스캔에 비해 검사시간을 보다 빠르게 할 수 있었으며 검사대기 시간 및 응급실 체류시간을 줄였고 낮은 선량으로 진정을 최소화하면서 검사하였다. 결국 volume axial scan과 보조기구 및 사전교육을 활용한 검사방법은 그렇지 않은 helical scan 집단에 비해 보다 더 효율적인 검사방법임을 보여 주었다. Purpose : The purpose of this study is to investigate whether the method which uses volume axial scan, assist device and pre-training of assistant is more efficient than the one using the previous helical scan. Material and method : It was evaluated by 240 pediatric patients (mean age 3 years) in this study, except for those who were not able to use the experimental equipment such as conscious congestion, decreased consciousness and sleep state which were caused by acute trauma and medical illness. The experimental group applied with pre-training, volume axial scan, and assist device was compared with the control group in which patients without pretraining and assist device were only held. The test was analyzed by testing tie, waiting time, stay time in emergency room, radiation dose, the number of retake and sedation. Assuming that both groups were held on the test, it was carried out by checking if sedation are used as we need. And the experimental group was applied with the experimental equipment (assist device, pre-training). Result : In the experimental group using the experimental equipment, the test time was reduced by 95 %, and also waiting time and stay time in emergency room were reduced by 39 % and 46 %, respectively. as compared with the control group. The rate of sedation was reduced 43 % (11cases) in axial scan compared with helical scan. In the number of retake, The rate of axial scan decreased by 25 % (5cases), compared to helical scan. P-value was 0.360, but there was no statistical significance. Radiation doses measured by rando phantom were reduced by 26 % in axial scan compared to helical scans. In collective dose, CTDI, DLP values of axial group were lower than the ones of helical scan. There was statistical significance because p-values of both groups were 0.002. Conclusion : Although there were the specificity and limitations of pediatric patients in brain CT scan, the test reduced examination time, stay time in emergency room and also minimized sedation with low radiation dose.
