9개월 전에 우측 제1 족지를 다친 이후에 발생한 통증을 주소로 내원하였다. 방사선 소견상 특별한 진단없이 타 병원에서 보존적 치료를 하였으나 증상이 호전되지 않고 제1 족지의 지절을 족저굴곡 하거나 족배굴곡 할 때 관절내 통증이 지속되어 전원 되었다. 단순 방사선 소견상 제 1 족지 지절 부위에 미세한 골편이 관찰 되어 초음파 검사를 시행하였다. 초음파 검사상 제1 족지 지절 관절내에 2개의 유리체가 관찰되었다. 수술적 제거술을 시행하여 증상이 치유 되었기에 이에 저자들은 무지 지절 유리체의 초음파 소견을 보고 하고자 한다. A 20-year-old patient who got injured on his right big toe and complained pain visited to our hospital 9 months ago. Before the patient transferred to our hospital, plain radiographs were obtained at a private hospital and the patient underwent conservative treatment with uncertain diagnosis. However, the symptom was not improved, and he continued to complain intra-articular pain on his interphalangeal joint of big toe during plantarflexion or dorsiflexion. On plain radiographs of our hospital, we observed small bone fragment in his interphalangeal joint of the big toe. On the ultrasonographic images, two loose bodies in the interphalangeal joint were found. Then, we removed the loose bodies as surgical treatment, and the symptom was subsided completely. The purpose of this study is to report the ultrasonographic finding of the loose bodies of interphalangeal joint of the big toe.

큰 대퇴 골두를 이용한 금속 대 금속 관절면 인공고관절 전치환술의 임상결과 단기 추시

인공 고관절 전치환술은 여러 가지 고관절 질환 에 있어 효과적인 치료 방법 중 하나로 사 용되고 있다. 그러나 수술 후 시간이 지남에 따라 관절면의 마모 및 골 용해, 수술 후 탈구 및 운동 범위의 제한, 감염 등 의 여러 문제점 과 합병증으로 인해 인공관절의 실패가 발생하고 있다. 이러한 관절 면 의 마모 및 여러 합병증의 발생으로 인하여 최근 세라믹 대 세라믹, 금속 대 금속 관절 면이 각광을 받고 있다. 그러나 세라믹을 이용한 관절변의 경우 세라믹 부품의 파손 과 골용해,해리 및 폴리에틸렌의 분리로 인한 조기 실패 등이 문제가 되고 있다. 금속-금속 관절면은 이러한 단점 들이 없으며 세라믹 관절면 다음으로 낮은 마모율을 보여 최근 많이 사용되고 있다. 또한 술 후 가장 흔히 발생 할 수 있는 합병증 중 의 하나인 탈구의 경우 술 후 첫 3 개월 간 에 가장 많은 빈도를 나타낸다 고 알려 져 있다. 탈구의 원인은 여러 요인 이 있으나 이 중 삽입물 관련 요인 에서 큰 대퇴 골두를 이용한 치환술의 경우 술 후 고관절의 운동범위 를 증가시켜 작은 대퇴 골두 에 비해 탈구 에 안정적이다. 이에 저자 들은 일반적으로 쓰 이 는 큰 대퇴 골두 (36mm ) 보다 더 큰 대퇴 골두를 이 용한 금속 대 금속 관절면 고관절 전치환술의 단기 추시 결과 및 합병증을 분석 하여 문헌 고찰과 함께 보고하고자 한다. Purpose: To study the early follow-up results of the metal-on-metal articulation total hip arthroplasty with use of large femoral head. Materials and Methods: 29 patients (31 hips) were follow up for more than 14 months after primary total hip arthroplasty using a metal-on-metal articulation with large femoral head. Average was 41 years old (range, 29~63). We checked the functional status and radiologic finding. Results: The average harris hip score improved from 54.4 points preoperatively to 93.5 points at the final follow up (P<0.05). All cases have s stable fixation and no evidence of osteolysis, acetabular cup loosening in radiologic finding. There was no evidence of post operative deep infection and dislocation, but one hip underwent intra-operative periprostetic fracture in around femoral stem. Conclusions: Short term follow up of metal-on-metal articulation total hip arthroplasty with use of large femoral head show a satisfactory result, but it needs a analysis for long term follow up results.

역사적 기억은 어떻게 전유되었는가? — 1930년대 봉천 조선인 사회와 삼학사 —

This paper examines how historical memories are appropriated, through a story from the 1630s. When Qing China invaded Joseon, hardliners in the Joseon government argued against signing a ceasefire agreement. Three literati were the most prominent figures of this faction, and when the war ended, with the surrender of the Joseon king to the Qing emperor, these three were immediately sent to Shenyang, then the Qing capital, where they were executed. Song Siyeol, the most respected Confucian scholar of the time, wrote a biography of these martyrs, paying tribute to the sacrifice they made for their country, and also praising them as staunch adherents of Confucian fidelity and loyalty. From his perspective, these are universal values which transcend national and ethnic differences. Some three hundred years later, the memory of the three martyrs sudden resurfaced, when Korean residents of Manchuria claimed to have found the remains of a memorial monument erected by the Qing emperor. They organized a committee, raised funds, and erected a new monument. They also published a new biography of the three martyrs. There was, however, no clear evidence for their claim. It seems that they arbitrarily appropriated the concepts of Confucian fidelity and loyalty, which were no longer valid to them, distorting and exaggerating their narrative to romanticize the actions and ambitions of Imperial Japan. 이 글은 삼학사의 의(義)와 충(忠)에 관한 이야기가 1930년대 봉천 조선인 사회에서어떻게 전유되었는지를 탐색한 것이다. 기초적인 사실관계를 논증하고, 거기에서 한걸음 더 나아가 ‘어떤 사회적ㆍ문화적 요구와 맥락이 그런 현상을 빚어냈는가’에 대해 질문하고 답을 찾았다. 오두환이 오달제의 순절처를 조사해 보려 했던 것이 가문의식의 발로였다면, 김구경이 처음 그 부탁을 받아들인 것은 학문적 호기심 때문이었다. 그러나 1933년 황윤덕이 ‘삼한산두(三韓山斗)라는 제액(題額)을 발견’했다고 주장하면서 사태는 새로운 국면을 맞았다. 보존회가 결성되어 삼학사비 재건사업이 추진되었으며, 『삼학사전』도중간되었다. 보존회에는 오두환ㆍ김구경ㆍ황윤덕 외에도 봉천 조선인 사회에서 ‘유지’ 로 대우받던 인물들, 그리고 삼학사의 후예로서 관동군에 소속된 조선인 장교들이이름을 올렸다. 김구경은 『중간 삼학사전』을 간행하면서 송시열이 구사한 ‘호(胡)’나 ‘노(虜)’라는 표현 대부분을 ‘적(敵)’으로 고치는 일종의 전략적 고쳐쓰기를 시도했다. 독자들에게 중국이 ‘적’임을 연상시키기 위해, 혹은 재만조선인들이 ‘충’으로 무장된 제국의 신민으로서 그런 ‘적’과 맞서고 있다는 점을 식민지 조선의 조선인 및 일본인 유력자들에게드러내기 위해서였다. 인륜적 질서로서의 ‘의’는 이런 방식으로 제국을 향한 ‘의’로전유되었다. 그런 ‘의’는 ‘제국에 대한 충’과 아무 차이가 없다. 조선이 주장한 ‘의사(義士)’로서의 삼학사는 바야흐로 ‘충사(忠士)’로서의 삼학사가 되어 갔다. 비석이 조선인 밀집지가 아니라 만철부속지의 일본인 집거지에 세워진 것도 같은 이유 때문이었다.
