        중소병원 내부마케팅이 고객지향성과 경영성과의지에 미치는 영향 및 시장지향성의 매개효과

        Objectives: The objectives of this study were to examine the effect of internal marketing on the employees` customer orientation and their business performance will, and to verify the mediating effect of market orientation in local hospital setting. Methods: A questionnaire-based survey was conducted among 300 employees working at 9 hospitals in Changwon area, and 279 responses were analyzed. The data were collected from April 4 to April 17, 2015. SPSS 18.0 program was used for descriptive statistics, t-test, ANOVA, factor analysis and reliability test, and AMOS 18.0 program was used for path analysis. Results: First, delegation of authority, education and training, and compensation, among other factors of internal marketing, showed a statistically significant effect on market orientation. Second, market orientation had a significant effect on both customer orientation and business performance will. Third, internal marketing influenced rather indirectly on the customer orientation and business performance will, mediated by market orientation. Conclusions: Leveraging market orientation could help local hospitals make the most of their internal marketing activities in order to improve customer orientation at the individual level and the business performance at the organizational level.

        광곽향 추출물의 멜라닌 생성 저해 효과

        본 연구는 천연 미백소재 개발을 위하여 광곽향 추출물과 광곽향 추출물에서 분리한 활성물질의 멜라닌 생성에 연관된 생리활성을 분석하였다. 광곽향 추출물은 100 μg/mL 이하에서 세포 독성이 없는 것으로 확인되었으며 free radical 소거능(DPPH)과 superoxide radical 소거능 결과는 각각 IC<sub>50</sub> = 24.2 ± 2.85 μg/mL, IC<sub>50</sub> = 118 ± 0.43 μg/mL을 나타내었다. B16 melanoma 세포에서의 멜라닌 생합성 저해 효과는 20 μg/mL 농도의 광곽향 추출물을 72 h 동안 처리한 세포에서 멜라닌 억제율이 23 %로 나타났으며, 50 μg/mL 농도에서 세포 내 tyrosinase의 활성을 18 % 저해하였다. 이러한 광곽향 추출물의 활성물질을 분리하여 <sup>1</sup>H-NMR, <sup>13</sup>C-NMR, Mass analysis 등의 기기분석을 실시한 결과 sesquiterpene 계열의 활성물질인 patchouli alcohol으로 동정되었고, patchouli alcohol의 free radical 소거능과 superoxide radical 소거능 결과는 각각 IC<sub>50</sub> = 3.14 ± 0.12 μg/mL, IC<sub>50</sub> = 49 ± 3.24 μg/mL을 나타내었다. 또한 멜라닌 저해효과를 확인한 결과 IC<sub>50</sub> = 3.9 μg/mL으로 나타났으며, 10 μg/mL 농도에서 세포 내 tyrosinase의 활성을 40 % 저해하였다. Western blot을 이용하여 tyrosinase와 tyrosinase related protein-2 (TRP-2) 단백질의 발현 감소를 확인하였다. 그러므로 광곽향 추출물과 patchouli alcohol은 우수한 미백 효능을 갖는 화장품 소재로서 개발 가능성이 클 것으로 기대된다. To develop a new natural whitening agent for cosmetics, we investigated the inhibitory effects of Pogostemon cablin Bentham extracts (PCE) and its active component on melanogenesis. PCE showed ROS scavenging activities in 1,1-diphenyl-2-picrylhydrazyl (DPPH) radical and xanthine/xanthine oxidase system with the IC<sub>50</sub> values of 24.2 ± 2.85 μg/mL and IC<sub>50</sub> = 118 ± 0.43 μg/mL, respectively. PCE reduced melanin contents of B16 melanoma cells in a dose-dependant manner and decreased to about 23 % at a concentration of 20 μg/mL without cell cytotoxicity (below 100 μg/mL). And the PCE reduced intracellular tyrosinase activity about 18 % at concentration of 50 μg/mL. We purified one active compound from PCE and identified its structure. It was identified as patchouli alcohol, sesquiterpene family, by <sup>1</sup>H-NMR, <sup>13</sup>C-NMR, and Mass analysis. Patchouli alcohol also inhibited ROS scavenging activities in DPPH radical and xanthine/xanthine oxidase system with the IC<sub>50</sub> values of 3.14 ± 0.12 μg/mL and 49 ± 3.24 μg/mL, respectively. Patchouli alcohol inhibited melanin synthesis in a dose dependent manner (IC<sub>50</sub> = 3.9 μg/mL). And the patchouli alcohol reduced intracellular tyrosinase activity about 40 % at concentration of 10 μg/mL. Patchouli alcohol inhibited tyrosinase and TRP-2 expression at protein level. These results suggest that PCE and patchouli alcohol reduced melanin formation by the inhibited of tyrosinase activity and expression in B16 melanoma cells. Therefore, we suggest that PCE could be used as a useful whitening agent.

        피그말리온 리더십이 병원 종사자의 조직몰입에 미치는 영향과 리더

        본 연구는 부산시내 9개 병의원에 근무하는 근로자 349명을 대상으로 구조화된 설문지를 통하여 자료를 수 집하였으며, 의료조직에서 피그말리온 리더십이 종사자의 리더-구성원 교환관계와 조직몰입에 미치는 영향을 파악함 으로써 병원의 조직성과 제고방안을 제시하고자 하였다. 수집한 설문자료는 SPSS ver.18.0과 AMOS ver.18.0 프로그 램을 이용하여 분석하였으며, 빈도분석, 상관분석, 요인분석, 신뢰도분석, 경로분석 등을 수행하였다. 주요 분석결과는 다음과 같다. 첫째, 병원조직에서도 피그말리온 리더십이 리더-구성원 교환관계의 질에 영향을 미치는 것으로 나타났 다. 둘째, 피그말리온 리더십은 병원 종사자의 조직몰입에 직·간접적으로 영향을 미치는 것으로 나타났다. 셋째, 리더- 구성원 교환관계는 조직몰입의 정서적 몰입, 지속적 몰입, 규범적 몰입에 각각 직접적인 영향을 미치며 그 중에서도 정서적 몰입에 가장 큰 영향을 미치는 것으로 나타났다. 넷째, 리더-구성원 교환관계는 병원 조직에서 피그말리온 리 더십과 조직몰입의 관계를 부분적으로 매개하는 것으로 나타났다. 본 연구는 병원 조직에서 피그말리온 리더십과 리 더-구성원 교환관계, 조직몰입 간의 관계를 최초로 연구하였다는 데 의의가 있다. This study attempted to examine the relationship between pygmalion leadership and the organizational commitment among hospital employees, and the mediating effect of the leader-member exchange (LMX) on their relationship. For this purpose, we designed structured self-response survey questionnaires, and collected data from 349 employees working at 9 hospitals and ambulatory clinics in Busan area. Frequency analysis, correlation analysis, factor analysis, and reliability analysis were performed using SPSS ver.18.0 program, and the path analysis was performed using AMOS ver.18.0 program. Major findings are as follows. First, some factors of pygmalion leadership were found to directly influence the quality of LMX, while some had both direct and indirect influences on organizational commitment. Second, the quality of LMX was found to partly mediate the relationship between pygmalion leadership and the organizational commitment in hospital organizations. This is the first in kind which was conducted in the hospital setting.

        구강보건 지식과 실천이 우식경험치아와 삶의 질에 미치는 영향-구조방정식을 이용한 경로분석을 중심으로-

        This study aims to measure oral health knowledge and practice level among dental clinic patients, and examine their relationships with decayed, missing and filled teeth (DMF) index and the oral health-related quality of life (QOL). Empirical data have been collected from 311 patients through structured questionnaires and dental examinations conducted in Busan area. According to the analysis of causal relationship, oral health knowledge turned out to have a direct effect on oral health practices, which exerted both direct and indirect effects on DMF index and oral health-related QOL. While DMF index directly affected the oral health-related QOL, oral health knowledge proved to have only an indirect effect on DMF index and oral health-related QOL, mediated by oral health practices. Considering all these findings, effective oral health education and other intervention programs should focus on motivating people to participate in the active practices of desirable oral health behaviors, which will lead to the prevention of oral diseases and the subsequent improvement in the QOL.

        신규간호사 ‘태움(Taeoom)’에 대한 개념 분석

        Purpose: This study was conducted to identify the conceptual definition and attributes of ‘Taeoom’ (or workplace bullying) inflicted to newly employed nurses in Korea. Method: Walker & Avant(2011)`s eight-stage process was used to perform a conceptual analysis of ‘Taeoom’. Literature review of 40 studies published between Jan. 2009 and March 2018 published in Korea was followed by the in-depth interview with nine newly employed hospital nurses and the ex-post review of results by nursing experts. Findings: ‘Taeoom’ was associated with five attributes: (1) bullying for no reason, (2) peer rejection for no reason, (3) decreased physical function and psychological withdrawal, and (4) verbal abuse, defamation and nagging, (5) impotent feeling due to power imbalance. Four antecedents found in this study include offensive action, distrust, power imbalance, and undue workload exceeding capacity. As consequences of Taeoom, negative physical and psychological symptoms and turnover intention were increased while nursing performance was decreased. Conclusion: This study suggests the need for more qualitative researches with more comprehensive approach on Taeoom and the development of effective program to improve the organizational culture in nursing field. This study is significant in that it provides a qualitative but comparative review on the attributes, antecedents and consequences of Taeoom through literature review accompanied by focus group interview and expert review.

        천연 약용식물 추출물의 여드름 원인균에 대한 항균 및 항염증 효과

        여드름 완화 효과를 갖는 천연 약용식물 추출물을 개발하기 위하여 여드름 원인균에 대한 항균 활성 및 항염증 효과를 다양한 추출물을 대상으로 분석하였다. 검색 결과 얻어진 후보 소재들 중, 비교적 높은 항균 활성을 갖는 보골지, 당후박과 항염증 효과를 갖는 목향, 구절초 추출물을 대상으로 선정하였고 이후의 실험을 진행하였다. 보골지(Psoralea corylifolia L., AC-1), 당후박(Magnoliae officinalis, AC-2), 목향(Inula helenium L., ACF-1), 구절초(Chrysanthemum zawadskii var. latilobum, ACF-2) 추출물 및 복합소재(AC)는 50 μg/mL 이하에서 세포 독성이 없는 것으로 확인되었다. 보골지, 당후박 추출물 및 복합소재의 경우 여드름 유발 원인균인 Propionibacterium acnes (P. acnes)에 대해 양성 대조군으로 사용한 10 % salicylic acid 대비 각각 약 2.8배, 2.5배, 3.2배의 높은 항균 효과를 보였다. 또한 Staphylococcus aureus (S. aureus)에 대해서도 당후박 추출물과 복합소재의 경우 양성 대조군으로 사용한 10 % Methylparaben 대비 각각 약 1.4배, 1.5배 높은 항균 효과를 보였다. 목향, 구절초 추출물 및 복합소재의 경우 열에 의해 사멸시킨 P. acnes에 의해 활성화된 THP-1 세포의 IL-8 발현을 50 μg/mL 농도에서 각각 27 %, 38 %, 44 % 저해하였고 TNF-α 발현을 50 μg/mL 농도에서 각각 90 %, 88 %, 90 %로 크게 저해하였다. 이러한 항균, 항염증 활성을 보이는 보골지, 당후박, 목향, 구절초 추출물의 복합 소재를 이용하여 여드름 완화 효과를 갖는 화장품으로의 개발이 가능할 것으로 보인다. To develop natural therapeutic agents for acne vulgaris, we investigated antibacterial and anti-inflammatory effects of various medicinal plant extracts. Among candidate extracts, we selected Psoralea corylifolia L. extract (AC-1) and Magnoliae officinalis extract (AC-2) which showed the relatively high antibacterial effects, and Inula helenium L. extract (ACF-1) and Chrysanthemum zawadskii var. latilobum extract (ACF-2) which showed the relatively high anti-inflammatory effects for further investigations. All of them did not show cytotoxic effects below the concentration of 50 μg/mL. The antibacterial effects of AC-1, AC-2 and extract complex (AC) against P. acnes were 2.8, 2.5 and 3.2 times higher than that of 10 % salicylic acid respectively. And the antibacterial effect of AC-2 and extract complex against S. aureus were 1.4 and 1.5 times higher than that of 10 % methylparaben respectively. Also, it was shown that ACF-1, ACF-2 and extract complex had anti-inflammatory effects. All of them exhibited inhibitory effects for the secretion of IL-8 and TNF-α from THP-1 cells activated by heat-killed P. acnes. They reduced about 27 %, 38 %, 44 % of IL-8 secretion and 90 %, 88 %, 90 % of TNF-α secretion at concentration of 50 μg/mL respectively. These results showed that the complex of medicinal plant extracts, AC-1, AC-2, ACF-1, and ACF-2, had therapeutic effects to acne vulgaris through antibacterial and anti-inflammatory effects. Therefore, we suggest that extract complex of AC-1, AC-2, ACF-1 and ACF-2 may be used as a useful agent for development of natural cosmetics which have therapeutic effects to acne vulgaris.

        복령피 추출물의 멜라닌 생성 저해 효과

        천연 미백 소재 개발을 위하여 복령피 추출물과 복령피 추출물에서 분리한 활성 물질의 멜라닌 생성에 연관된 생리 활성을 분석하였다. 복령피 추출물은 100 μg/mL 이하에서 세포 독성이 없는 것으로 확인 되었으며 free radical 소거능(DPPH)과 superoxide radicals 소거능 결과는 각각 IC<sub>50</sub> = 19.4 ± 2.21 μg/mL, IC<sub>50</sub> = 103 ± 3.33 μg/mL을 나타내었다. 50 μg/mL 농도에서 B16 melanoma 세포 내 tyrosinase의 활성을 34 % 저해 하였으며, 세포 내 멜라닌 생합성 저해 효과는 50 μg/mL 농도의 복령피 추출물을 72 h 동안 처리한 세포에서 51 % 억제율을 보이는 것으로 나타났다. 이러한 복령피 추출물의 활성 물질을 분리하여 1H-NMR, 13C-NMR, Mass analysis 등의 기기 분석을 실시한 결과 triterpene류의 3-β-hydroxylanosta-7,9(11),24-trien-4-oic acid로 동정되었고 100 μg/mL 이하에서 세포 독성이 없는 것으로 확인 되었다. 3-β-hydroxylanosta-7,9(11),24-trien-4-oic acid의 free radical 소거능과 superoxide radicals 소거능 결과는 각각 IC<sub>50</sub> = 4.3 ± 0.15 μg/mL, IC<sub>50</sub> = 54 ± 1.67 μg/mL을 나타내었다. 10 μg/mL 농도에서 세포내 tyrosinase의 활성을 43 % 저해 하였으며, 멜라닌 저해 효과를 확인한 결과 IC<sub>50</sub> = 3.6 μg/mL으로 나타났다. Western blot을 이용하여 tyrosinase, tyrosinase related protein-1 (TRP-1), TRP-2 단백질의 발현 감소를 확인하였다. 복령피 추출물과 3-β-hydroxylanosta-7,9(11),24-trien-4-oic acid는 우수한 미백 효능을 갖는 화장품 소재로의 개발 가능성이 클 것으로 기대된다. And PCBE reduced melanin contents of B16 melanoma cells about 51 % at concentration of 50 μg/mL without cell cytotoxicity (below 100 μg/mL). It was identified as 3-β-hydroxylanosta-7,9(11),24-trien-4-oic acid, triterpene family, by 1H-NMR, 13C-NMR and Mass analysis. 3-β-hydroxylanosta-7,9(11),24-trien-4-oic acid showed ROS scavenging activities in DPPH radical and xanthine/xanthine oxidase system with the IC<sub>50</sub> values of 4.3 ± 0.15 μg/mL and 54 ± 1.67 μg/mL, respectively. Also, it was shown that 3-β-hydroxylanosta-7,9(11),24-trien-4-oic acid reduced intracellular tyrosinase activity about 43 % at concentration of 10 μg/mL. And it inhibited melanin synthesis in a dose dependent manner (IC<sub>50</sub> = 3.6 μg/mL) without cell cytotoxicity (below 100 μg/mL). 3-β-hydroxylanosta- 7,9(11),24-trien-4-oic acid inhibited tyrosinase, TRP-1 and TRP-2 expression at protein level. These results suggest that PCBE and 3-β-hydroxylanosta-7,9(11),24-trien-4-oic acid reduced melanin formation by the inhibition of tyrosinase activity and expression in B16 melanoma cells. Therefore, we suggest that PCBE could be used as a useful whitening agent.
