        치료적 클라이밍 훈련이 뇌졸중 환자의 상지기능과 삶의 질에 미치는 영향

        본 연구는 치료적 클라이밍 훈련이 뇌졸중 환자의 상지기능과 삶의 질에 미치는 효과를 알아보고, 임상적으로 중재될 수 있는지에 대한 가능성을 제시하는데 그 목적이 있다. 본 연구는 14 명의 뇌졸중 환자를 대상으로 하였으며, 7명의 치료적 클라이밍 훈련군과 7명의 대조군으로 지정하였다. 실험군은 일반적 물리치료와 작업치료를 시행하였고, 추가적으로 포츠담모델을 이용한 치료적 클라이밍 훈련을 6주 동안 주 3회, 총 30분 씩 훈련을 진행하였다. 대조군은 일반적 물리치료와 작업치료를 적용하였다. 상지기능은 Manual Function Test로 측정하였고, 삶의 질은 Short Form 36-item으로 측정하였다. 측정 결과, 그룹 내 시기에 따라 실험군의 상지기능은 유의한 차이가 있는 것으로 나타났다 (p<.05). 삶의 질은 그룹 내 시기에 따라 실험군의 유의한 차이가 있었고, 각 그룹 간의 비교에서는 실험군이 대조군에 비하여 유의한 차이가 있었다(p<.05). 본 연구를 통해 뇌졸중 환자에게 포츠담 모델을 적용한 치료적 클라이밍 훈련이 상지기능과 SF-36점수를 증가시켜 삶의 질에 긍정적인 효과를 가져왔다. 따라서 향후 치료적 클라이밍을 이용한 훈련을 통해 뇌졸중 환자의 재활이나 임상 적용 중재에 있어서 가치가 있음을 확인할 수 있었다. This study was conducted to investigate the effects of therapeutic climbing training on upper limb function and quality of life of stroke patients and to suggest possibilities for clinical intervention. This study investigated 14 stroke patients who were assigned to either a treatment climbing training group or a control group (n=7 each). The experimental groups performed general physical therapy and occupational therapy. In addition, therapeutic climbing training using the Potsdam model was conducted for 30 minutes three times a week for 6 weeks. The control group performed general physical therapy and occupational therapy. A manual function test was used to evaluate upper limb function, and a short form 36-item questionnaire was used to evaluate quality of life. The results showed that there was a significant difference in upper limb function of the experimental group according to time (p<0.05). There was also a significant difference in the quality of life according to time in the experimental group, and a significant difference in quality of life between the experimental group and the control group (p<0.05). Therapeutic climbing training by stroke patients positively affected quality of life by increasing the SF-36 scores and upper limb function. Taken together, these findings confirmed that training using therapeutic climbing is valuable for rehabilitation and clinical intervention of stroke patients.

        고교야구선수의 배트 스윙 속도와 몸통 근육 두께 및 비대칭 간의 상관관계

        The purpose of this study is to investigate the relationship between the bat swing speed, trunk muscle thickness of dominant side and non-dominant side, and asymmetry. This study was conducted on 30 high school baseball hitters. B-mode ultrasound imaging was used to measure the thickness of the both side of rectus abdominis, external abdominal oblique, internal abdominal oblique, transverse abdominal, abdominal wall and lumbar multifidus. As a result, there was a significant correlation between internal abdominal oblique of dominant side, abdominal wall and bat swing speed. There was no statistical correlation between the bat swing speed and muscle asymmetry. In conclusion, the size of muscle is more important than the asymmetry of muscles in the dominant and non-dominant sides.

        속도가 다른 트레드밀 훈련이 다리 근두께에 미치는 영향

        본 연구의 목적은 속도가 다른 트레드밀 훈련이 다리 근두께에 어떠한 영향을 미치는지 알아보고자 하였다. 대상자는 대학생 남녀 36명을 평보군 (n=12)과 속보군 (n=12), 교차보행군 (n=12)으로 나누어 평보군은 평균 속도의 100%의 속도로 훈련을 실시하였고, 속보군은 평균 속도의 130%의 속도로 실시하였으며, 교차보행군은 130%의 속도와 100%의 속도를 각각 교차하면서 보행 훈련을 실시하였다. 훈련은 6주 동안 시행하였고, 주당 3회, 회당 60분간 시행하였다. 측정은 넙다리곧은근과 가쪽넓은근, 장딴지근, 앞정강근의 두께를 비교하기 위해 초음파영상장치를 이용하였다. 연구 결과 넙다리곧은근, 가쪽넓은근, 장딴지근에서 시간에 따라 유의한 증가가 있는 것으로 나타났고, 상호작용 또한 있는 것으로 나타났다. 그리고 앞정강근에서는 시간에 따라 유의한 증가가 있는 것으로 나타났다. 이상과 같은 결과로 보아 평속보다는 속도를 달리한 훈련을 실시하였을 때 다리 근육의 두께 변화에 영향을 미치는 것으로 생각되며, 추후 연구에서는 신경계나 근골격계 질환으로 인한 보행 장애를 가진 환자를 대상으로 재활 프로그램에 적용하는 연구가 지속적으로 이루어져야 할 것으로 생각된다. The purpose of this study was to determine the effects of speed variations in treadmill training on the thickness of lower extremity muscles. A total of 36 university students were divided into three groups: MVTG (n=12), HVTG (n=12), and ATG (n=12) . Subjects in MVTG underwent treadmill training with their own average speed; those in HVTG underwent treadmill training with 130% speed of their own average speed; and those in ATG underwent treadmill training with alteration of speed, between 100% and 130% of their own average speed. Treadmill training was performed for 60 minutes a day, three times per week, for a total of 6 weeks. Ultrasonography was used to compare the muscle thickness between rectus femoris, vastus lateralis, gastrocnemius, and tibialis anterior. The result was as follows: The rectus femoris, vastus lateralis, and gastrocnemius were significantly increased after the training period, and they also were statistically significant in interaction . Moreover, tibialis anterior was also significantly increased. Therefore, compared to the average-speed treadmill training, speed variations had a greater effect on thickening the lower extremity muscles. In the near future, we will conduct a study applying the findings from this study in a rehabilitation program for patients with gait disturbance due to nervous or musculoskeletal system disease.

        공기압 차이에 따른 불안정지지면이 다리 근활성도에 미치는 영향

        The purpose of this study was to determine the effect of the changes that occur in the leg muscle activity of unstable surface with different levels of air pressures. Three groups of college students have been placed randomly on unstable surfaces with different air pressures at group 1.0 psi (n₁=36), group 1.4 psi (n₂=40), and group 1.8 psi (n₃=40). Using surface electromyography, the recruitment of the tibialis anterior, peroneus longus, and the gastrocnemius was measured. Maximal voluntary isometric contraction was measured at the different air pressures based on the manual muscle test, then normalizing the value to %maximal voluntary isometric contraction (%MVIC). The tibialis anterior muscle activity was significant change from the unstable surface with difference levels of air pressures between group 1.0 psi and 1.8 psi and between group 1.4 psi and 1.8 psi. peroneus longus muscle activity was significant changes in muscle activity occurred between 1.0 psi and 1.4 psi group and between 1.0 psi and 1.8 psi group. Gastrocnemius muscle activity was significant change in muscle activity occurred between 1.0 psi and 1.4 psi group and between 1.0 psi and 1.8 psi group. In conclusion it identify that 1.0 psi group is most effective on muscle activity than the other groups. These suggest that the rehabilitation or strengthening of patients with ankle injuries, balance exercise with low air pressure like 1.0 psi can be more effective.

        초등학교 야구선수에 상지 근력과 배트 스윙 속도 간의 상관관계 분석

        본 연구는 초등학교 야구선수에 상지 근력과 배트 스윙 속도 간의 상관관계 분석을 알아보고자 하였다. 연구대상자 32명을 대상으로 실시하였다. 상지 근력은 양쪽 어깨 관절에 폄근·굽힘근·모음근·벌림근, 양쪽 팔꿉관절에 폄근·굽힘근의 근력과 배트 스윙 속도를 측정하였다. 상지 근력과 배트 스윙 속도의 상관관계를 알아보기 위해 pearson’s cerrelation analysis를 사용하여 분석하였다. 우세 측 상지 근력과 배트 스윙 속도 간의 상관관계에서는 어깨 관절 모음근, 어깨 관절 벌림근, 팔꿉 관절 굽힘근, 그리고 어깨 관절 굽힘근 순으로 유의한 양의 상관관계를 보였으며, 비 우세측 상지 근력과 배트 스윙 속도 간의 상관관계에서는 어깨 관절 모음근, 팔꿉 관절 굽힘근, 어깨 관절 벌림근, 어깨 관절 폄근, 팔꿉 관절 폄근 그리고 어깨 관절 굽힘근 순으로 유의한 양의 상관관계를 보였다. 이와 같은 결과로 볼 때 상지근력이 강할수록 배트 스윙 속도가 증가하는 양의 상관관계를 나타내는 것을 알 수 있었다. The purpose of this study was to determine the correlation between upper limb muscle power and bat swing speed in elementary school baseball players. This study subjects were 32 subjects. Upper extremity muscle strength was measured in the Both Shoulder joints extensor·flexor·abductor·adductor, Both elbow joints extensor·flexor and bat swing speed. The correlation between upper extremity muscle strength and bat swing speed was analyzed using pearson’s correlation analysis. There was a significant correlation between dominant upper extremity muscle strength and bat swing speed in the order of shoulder adductor, shoulder abductor, elbow flexor, and shoulder flexor and in correlation between non-dominant upper extremity muscle strength and bat swing speed in the order of shoulder adductor, elbow flexor, shoulder abductor, shoulder extensor, elbow extensor, and shoulder flexor. From these results, it can be seen that the stronger the upper extremity muscle strength, the bat swing speed is the more positive correlation.

        고교투수의 구속과 균형간의 상관분석 및 다중선형회귀분석

        The purpose of this study is to investigate the relationship between pitcher’s ball speed and balance. Thirteen high school pitchers participated in this study. The balance measurement equipment was used to measure the center of pressure movement distance when standing on one foot. The measurements were made with the eyes open and the eyes closed, respectively. Ball speed was measured using a speed gun. The analysis used correlation analysis and multiple linear regression. As a result, although ball speed and balance are correlated, the foot that is the axis during the pitching independently affects ball speed. In conclusion, balance is an important factor for pitchers, but a feet that becomes the axis during pitching affects ball speed.

        진동을 이용한 운동프로그램이 여대생의 몸통 근 두께에 미치는 영향

        The purpose of this study is to find out how vibration-based exercise programs affect the thickness of the trunk muscles of female university students, and to develop exercise programs using them and to provide basic data in clinical practice. A total of 34 subjects were randomly classified using a random number table, and then 14 vibration groups and 14 non-vibration groups were exercised 3 times a week for 8 weeks, for a total of 40 minutes and 50 minutes. The trunk muscle thickness was measured using ultrasound. As a result, in the vibration group, there was a significant difference in external abdominal oblique, internal abdominal oblique and rectus abdominis in the period of trunk muscle thickness(p<.05), and in the non-vibration group, there was a significant difference in the left external abdominal oblique, both rectus abdominis(p<.05). Through this study, it was found that the exercise program using vibration has a positive effect on improving the thickness of the trunk muscle in female university students. Therefore, clinical studies applied to various patients as well as trunk muscles will be needed in the future.

        만성 허리통증환자의 라이프 케어 증진을 위한 중추성과 말초성 자극에 따른 감각변화의 차이

        연구는 만성 허리통증환자의 라이프 케어 증진을 위해 중추성과 말초성 자극에 따른 감각변화의 차이를 알아보고자 하였다. 12명의 만성 허리통증환자는 중추성 자극군(central stimulation, CS, n=6)과 말초성자극군(peripheral stimulation, PS, n=6)으로 무작위 할당하여 정량적감각검사(QST), 압력통각역치(PPT) 그리고 요통기능장애지수(ODI)를 정량적으로 측정 및 분석하였다. 연구결과, QST, PPT 그리고 ODI는 시기별 유의한 차이를 나타냈으나(p<.01) 두 군 간의 차이는 나타나지 않았다(p>.05). 따라서, 두 자극 모두 통증으로 인해 민감해진 근육의 감각역치 증가와 기능개선에 유의한 효과를 보여 만성 허리통증환자를 위한 라이프 케어 증진에 상당한 의미가 있다고 생각된다. This study aimed to investigate the difference of sensory changes by central and peripheral stimulation for improving life care in chronic low back pain patients. Twelve chronic low back pain patients were randomly assigned to central stimulation (CS, n=6) and peripheral stimulation (PS, n=6). Quantitative sensory test (QST), pressure pain threshold (PPT) and Korean oswestry back pain disability index (KODI) were used to quantitatively measure and analyze. As a result, QST, PPT and KODI showed significant differences by period (p<.01) but did not showed any difference between the two groups (p>.05). Therefore, both stimulations had significant effects on increased sensory threshold and function improvement of the muscles that became sensitive due to pain. It is thought to be significant in improving life care for patients with chronic low back pain.

        진동을 이용한 몸통 근력 운동이 몸통근 두께와 균형에 미치는 영향

        PURPOSE: The aim of this study was to verify the effect of trunk strengthening exercise using oscillation by comparing trunk muscle thickness, as well as balance of healthy adults during exercises performed with an oscillatory device and non-oscillatory device. METHODS: Twenty-two participants were randomly assigned to one of two groups: the trunk strengthening exercise using oscillation (TSEO) group (n=11) or the trunk strengthening exercise using non-oscillation (TSEN) group (n=11). Subjects in all groups performed the exercises three days per week for 6 weeks. All subjects performed four types of exercises: pull over, seated twist, power push, and diagonal power plank. Trunk muscle thickness of the rectus abdominis (RA), internal oblique (IO), external oblique (EO), transverse abdominis (TrA), and multifidus (MT) were measured with an ultrasonography. The balance ability were evaluated using the Romberg test with eyes open, eyes closed, one-leg standing test (OLST), and limits of stability (LOS). All tests were performed before the intervention, as well as after 6 weeks and 8 weeks of exercises. RESULTS: There was a significant difference of RA, IO, TrA, and MT according to the main effect of the time (p<.05). There was a significant difference of IO and LOS according to interaction effect between the time and group (p<.05). CONCLUSION: As intended, the cyclic forces induced by the oscillating device did increase trunk muscle thickness. However, the effect was limited and significant only for the IO muscle. Combining trunk strengthening exercise with oscillation appears to be more effective in improving dynamic balance.

        케틀벨을 이용한 힙드라이브 스윙운동이 다리근력에 미치는 영향

        본 연구의 목적은 케틀벨을 이용한 힙드라이브 스윙운동이 다리근력에 어떠한 영향을 미치는지 알아보고 추후 임상에서의 다리근육의 근력 강화에 활용될 수 있는지의 가능성을 제시하고자 한다. 연구를 위하여 20대 성인 30명을 실험군 15명, 대조군 15명으로 무작위 배치하였으며, 8주 동안 주 3회 운동을 실시하였다. 각 집단은 운동 전과 운동 8주 후에 휴대용 근력 측정기를 사용하여 엉덩 폄, 무릎 폄, 무릎 굽힘, 발바닥 굽힘의 근력을 측정하였다. 자료 분석을 위하여 독립표본 t-검정과 대응표본 t-검정을 사용하였다. 그 결과 그룹 간에는 유의한 차이가 없었고, 시기에서 실험군은 엉덩 폄, 무릎 폄, 무릎 굽힘, 발바닥 굽힘의 근력에서 시기에 따라 유의한 차이가 있었고(p <.05), 대조군은 무릎 폄, 발바닥 굽힘에서 유의한 차이가 있었다(p <.05). 따라서 향후 다리근력에 대한 본 연구의 케틀벨을 이용한 힙드라이브 스윙운동이 임상에서 다리근력 강화 중재 방법의 자료로 제공될 수 있을 것으로 사료 되며, 또한 다양한 환자들을 대상으로 연구가 이루어져야 할 것으로 생각된다. The purpose of this study was to investigate the effect of hip drive swing exercise using a kettlebell on lower extremity muscle strength and to examine its possible use for strengthening lower extremity muscles in clinical practice. Thirty adults were randomly and equally assigned to an experimental group or a control group, all thirty were exercised 3 times a week for 8 weeks. Strengths of hip extension, knee extension, knee flexion, and plantar flexion were measured using a portable muscle strength tester before and after the 8-week exercise period. The analysis was conducted using the independent t-test and the paired t-test. Hip extension, knee extension, knee flexion, and plantar flexion strengths increased significantly in the experimental group during the exercise period (p <.05), and knee extension and plantar flexion strengths increased significantly in the control group (p <.05). However, no significant difference was found between the two groups. Our findings suggest that hip drive swing exercise using kettlebells for strengthening lower extremity muscle strength could be used as an interventional method for strengthening lower extremity muscle strength in clinical practice. We recommend that the merits of this lower extremity muscle strengthening method be investigated further in a variety of patients.
