分娩中 使用한 Methoxyflurane(Penthrane)의 臨床的 考察

Most analgesics used in parturients have many disadvantages such as fetal and maternal cardiorespiratory depression and poor uterine contractability etc. An inhalating anesthetic, methoxyflurane(Penthrane), and ethyl-methyl ether was clinically evaluated on 37 nulliparous and 28 multiparous women in the second stage of labor. The gas was inhalated through analgizer which was designed to produce conscious analgesia on patient demand. The induction time was about 5.5 minutes and the recovery time was about 4. 0 minutes. The analgesic effect was. satisfactory in almost all cases during-labor and repairing episiotomy wound without prolongation of delivery course. During and after the use of this agent, patient cooperation’s were good and there were no significant untoward reactions as well as no remarkable alterations in maternal blood pressure, pulse rate, respiratory rate and, fetal heart tone. More comfortable and painless delivery was experienced in about 90% of multiparous women compare to previous delivery, and there was not a case who didn't want the next use.

Chymotrypsin is an endopeptidase, fractionated from bovine pancreas, by modification of the Kunitz and Northrop method. Innerfield observed that intravenously administered trypsin induced lytic effect on artificially formed intravascular thrombi in animal experiments. More recent data suggest that chymotrypsin(parenteral or buccal) may be successfully used to prevent tie pain, edema and ecchymosis of episiotomy wounds. Preliminary studies of several authors indicate that chymoral has clinical effectiveness exactly parallel of that obtainable with the parenteral and buccal forms of chymotrypsin. This proteolytic enzymes are increasing interest and have been widely employed clinically as anti-inflammatory and antiedemic remedies. The double-blind study was carried out in 100 obstetric patients to determine clinical effectiveness of chymoral to aid in relieving edema, pain and ecchymosis following perineal laceration or episiotomy. The result of the study are as follows. 1. Chymoral proved to be an effective drug in reducing edema during the 3 postpartum days when it is most marked. 2. Pain. and ecchymosis were also considerably reduced in treated group. 3. There was no complete absence of side effects which could be ascribed to the drug.
