Purpose: Recently, moderate to high-dose of statin were recommended for diabetic patients. However, it is not clear whether it is appropriate to all patients. The aim of this study is to evaluate the efficacy of low-dose statins for Korean diabetic patients. Methods: We analyzed a retrospective data in single diabetic center that consisted of Korean patients with diabetes who had been prescribed any of the low-dose statins (atorvastatin 10 mg, pitavastatin 2 mg, pravastatin 10 mg, or simvastatin 10 mg). We analyzed their lipid and safety profiles before and 3 months after each statin therapy. Results: After 3 months statin therapy, mean low density lipoprotein (LDL)-cholesterol reduction was -38.6% (interquartile range, -49.3% to -34.3%). Atorvastatin 10 mg (n=61), pitavastatin 2 mg (n=60), pravastatin 20 mg (n=63), and simvastatin 10 mg (n=64) reduced LDL-cholesterol level by 38.1%, 39.2%, 41.6%, and 38.4% respectively. Among the total subjects, 70.2% of the patients achieved a target LDL-cholesterol level <100 mg/dL, 68.8%, 72.4%, 70.6%, and 67.8% of the patients achieved a target in atorvastatin 10 mg, pitavastatin 2 mg, pravastatin 10 mg, and simvastatin 10 mg respectively. Conclusion: A low-dose statin was enough to manage LDL-cholesterol level in most Korean diabetic patients. The efficacies of those statins were higher than expected in Korean compared to Caucasian.

Hypoglycemia is common but can lead to life-threatening consequences. Accurate diagnosis is important to establish the appropriate treatment strategy. Most cases of hypoglycemia are caused by hypoglycemic agents, although it can occur in individuals without diabetes. A systemic and comprehensive diagnostic approach is required to diagnose hypoglycemia in patients without diabetes. It is important to perform appropriate blood testing during an episode of hypoglycemia. This review will focus on the definition, differential diagnosis, causes, and treatment of hypoglycemia, particularly in people without diabetes. (Korean J Med 2021;96:484-492)
