기혼자의 자기분화와 결혼만족과의 관계에서 불안정 성인애착의 매개효과

본 연구는 사회적 재난과 같은 환경의 위협과 스트레스 상황에서 위기를 겪는 부부를 돕고자 기혼자의 자기분화, 결혼 만족, 불안정 성인애착 간의 관계를 알고, 그들이 지각하는 자기분화 수준과 결혼 만족의 관계에서 불안정 성인애착의 매개효과를 검증하는 것을 목적으로 하였다. 설문지는 서울, 경기지역에 거주하고 있는 기혼 내담자 200명을 대상으로 하여, 자기분화 수준, 결혼 만족, 불안정 성인애착을 측정하였다. 설문지에 잘못 기재하거나, 무성의하게 응답한 28부를 제외한 172부의 자료만 최종 분석에 사용하였다. 세 변인들 간의 관련성을 살펴보기 위해 상관분석을 실시하였고, 매개효과를 알아보기 위하여 SPSS MACRO PROCESS을 활용하여 회귀 기반 경로분석 (ordinary least squares regression-based path analysis)을 실시하였다. 본 연구를 통하여 밝혀진 결과는 다음과 같다. 변인 간의 상관을 살펴보면 자기분화는 불안정 성인애착과 불안정 성인애착과 결혼 만족도와 부적 상관을 보였다. 자기분화는 결혼 만족에 직접적인 영향을 끼치지는 않지만, 자기분화가 낮을수록 불안정 성인 애착이 높음으로써 결혼만족이 낮아지는 완전 매개효과가 나타남을 알 수 있다. 본 연구를 통해 결혼의 만족에 어려움이 무엇인지를 확인하는 한편, 부부 상담 접근에 적용하기 위한 유용한 자료를 제공하고자 한다. 또한, 본 연구 결과를 토대로 후속 연구에 대한 제언과 시사점을 논의하였다. The purpose of this study is to understand the relationship among differentiation of self, marital satisfaction, and insecure adult attachment in married people. This study is to help couples who suffer crises in environmental threats such as social disasters, and to verify the mediating effect of insecure adult attachment on the relationship between their perceived levels of differentiation of self and marital satisfaction. The questionnaire measured the level of differentiation of self, marital satisfaction, and insecure adult attachment to 200 married clients living in Seoul and Gyeonggi Province. Only 172 copies of data were used for the final analysis, excluding 28 copies that were incorrectly entered in the questionnaire or responded insincerely. Correlation analysis was conducted to examine the relevance between the three variables, and regression-based path analysis was performed using SPSS MACRO PROCESS to find out the mediating effect. The results revealed through this study are as follows. Looking at the correlation between variables, differentiation of self was found to be negatively correlated with insecure adult attachment and marital satisfaction. Differentiation of self does not directly affect marital satisfaction, but it can be seen that the lower the differentiation of self, the higher the insecure adult attachment, resulting in a complete mediating effect in which marital satisfaction decreases. Through this study, it is intended to provide useful data for application to marital counseling access while identifying what difficulties are in marital satisfaction. In addition, suggestions and implications for subsequent studies were discussed based on the results of this study.

신경섬유종증과 관련된 폐고혈압은 심각한 기능적, 혈역 학적 장애를 초래할 수 있다. 따라서 신경섬유종증 환자에서 호흡곤란, 기절, 피로 등 폐고혈압의 증상이나 징후가 나타 난다면 임상의들은 가능한 조기에 심초음파 등의 검사를 하여 폐고혈압을 발견하고 치료를 시작할 수 있도록 해야 한다. 또한 신경섬유종증과 관련된 폐고혈압의 병태생리적 이해를 위해 체계적이고 규모 있는 연구가 필요하며 이를 바탕으로 생존율 향상에 도움이 되는 추가적인 치료법에 대한 논의가 이루어져야 할 것이다. Neurofibromatosis type I is a genetic disease caused by mutations in the neurofibromin 1 (NF1) gene. Although it is characterized by a number of distinct clinical features, including cafe au lait macules, freckling in the axillary or inguinal regions, neurofibromas, and Lisch nodules (iris harmartomas), it can affect all physiological systems in the body [1]. Neurofibromatosis-related pulmonary hypertension has also been reported, and some patients showed a poor prognosis despite having received proper medical treatment [2-4]. We herein describe a case of pulmonary hypertension in a patient with neurofibromatosis type I who had no identified risk factors of pulmonary hypertension. 개복 담낭절제술 후 오랜 기간 후에 발생한 혈액 담즙증

A 77-year-old woman was admitted with 5 days history of melena. She had an open cholecystectomy 30 years ago. Abdominal computed tomography and duodenoscopy revealed massive hemobilia. Angiography showed right hepatic arterial fistula to common bile duct near the surgical clip. Embolization was done successfully and the patient recovered. We experienced a case of a massive hemobilia which was occurred after a long period of time since open cholecystectomy without pseudoaneurysmal change of the right hepatic artery. And we suggest the direct vessel injury and fistula between the bile duct and a blood vessel as a possible cause of hemobilia in this case. Korean J Pancreatobiliary 2014;19(1):42-46
