        심부정맥혈전증으로 응급실에 내원한 환자에서 폐동맥색전증을 진단하기 위한검사가 필요한가?

        Purpose: Deep vein thrombosis (DVT) is a risk factor of pulmonary thromboembolism (PTE), however it is not clear who should be evaluated for a PTE and a DVT at the same time. The purpose of this study is to determine the clinical characteristics of PTE in patients with DVT who visited the emergency department (ED). Methods: This was a retrospective cohort study of ED patients who visited with DVT and were simultaneously evaluated for a PTE from January 2012 to December 2013. We compared clinical characteristics between non-PTE and PTE patients with confirmed DVT in the ED. Results: Of these 166 patients, 96 patients (57.8%) were confirmed PTE by computed tomography. In multivariate analysis, patients with PTE had more systemic neoplasm (OR 2.03, 95% CI 1.04-3.93, p=0.037) and right heart strain pattern in electrocardiography (OR 5.29, 95% CI 1.71- 16.36, p=0.004) than patients without PTE. Femoral DVT was more likely in the non-PTE group (87.1% vs. 65.6%, p=0.002) and popliteal DVT was more likely in the PTE group (62.9% vs. 80.2%, p=0.013). However the number of DVT sites including both femoral and popliteal vein was not statistically different. Conclusion: In patients with systemic neoplasm or right heart strain patterns in electrocardiography, simultaneous PTE evaluation may be required in patients with DVT.

        수치모델을 이용한 인공 연안 사주가 있는 해빈 단면 변화 연구

        자연 연안 사주와 고정된 인공 연안 사주가 있는 해안에서의 해양 물리 환경 변화에 따른 해빈 단면변화를 수치모델을 통하여 연구하였다. 수치 모델은 준3차원 파랑-흐름-퇴적물 이동모델에 인공 구조물 위에서의 퇴적물 이동에 대한 경계조건을 제안하여 적용하였다. 수치실험 결과 자연 사주의 경우 해수면이나 파고의 변화에 따라 위치를 이동하며 적을 하였으나 고정된 인공 사주의 경우 사주 후면에 새로운 자연 사주를 형성시키거나 혹은 사주 후면에서 세굴 현상이 발생하기도 하였다. 본 연구는 잠제 설치에 따른 해안 지형 변화를 연구하는데 응용될 수 있다. The changes of beach profile with a natural longshore bar and beach profile with a fixed artificial bar are studied, respectively, using a numerical model. The quasi three dimensional wave-current-sediment transport model is applied with an addition of boundary condition for sediment transport on the artificial structure under water. The study shows that the natural bar adapts itself to the change of coastal physical environment by adjusting its location but the fixed artificial bar causes the formation of a second natural bar seaward of the fixed bar and scouring at the rear of the fixed bar. This study can be applied to work on the change of beach profile with submerged breakwaters.

        우리나라 연안침식 비디오 모니터링 사업에 대한 개선 방향 제언

        This study examines the status of the video monitoring project in Korea which has been operating since 2003, and suggests plans for future improvement. The current status of video monitoring system in other countries and image analysis techniques used in video monitoring are also reviewed. Though the coastal area in Korea is small compared to other countries, the nearshore video monitoring is very intensive in terms of site and camera numbers, but the monitoring data are mainly used only to calculate the change of beach area by erosion without other applications. In order to extend the application scopes, diversification of image collection and storage method is needed for appling to newly developed coastal image analysis techniques. In addition to this, if data standardization is made by using NetCDF, the data will be more easily reached by users in other countries as well as in Korea. Especially, in order to apply the video data to coastal management and beach tourism area, promotion and training for using video data are needed. Longterm video monitoring for shoreline change is very important not only at severly eroded beach by artificial projects but also at natural beach free from human approach, to check the natural response of the beach to the sea level rise caused by global climate warming. Although several new beach monitoring techniques such as drone or satellites are developed, ground based video monitoring techniques is still competitive in terms of cost and unmaned longterm monitoring. The newly developed techniques for measuring wave heights and nearshore depth will improve the efficiency of video monitoring system on the beach. 비디오 영상을 활용한 연안 침식 모니터링은 장기무인관측 시스템으로 비용과 효율성에서 타 관측에 비하여 우수하며 여러 나라에서 활용하고 있다. 우리나라에서도 여러 해안에서 연안침식 비디오를 모니터링을 수행하고 있으며 해안에 따라서는 10년 이상의 관측 자료를 축적하고 있다. 향후 사업의 계속에 따른 효율성을 높이기 위해서는 자료 생산의 표준화를 통하여 외국 영상 처리 기술의 접목을 용이하게 하고 생산된 영상이 해안 관광 및 관리에도 활용될 수 있도록 홍보와 교육을 병행하는 것이 필요하다. 특히 인공적인 영향으로 침식이 나타나는 해안은 물론 자연적인 요인에 의한 해안 지형 변화의 장기 관측을 통하여 전 지구 기후 변화에 따른 해수면 상승에 의한 변화와 함께 인공적인 원인에 의한 해안 지형변화도 효과적으로 분석할 수 있을 것이다.

        건선의 과거력이 없던 환자에서 발생한 치료저항성 홍색피부건선 1례

        Erythroderma presents with diffuse erythema and scaling involving all or most of the skin surface area. It can be caused by inflammatory dermatosis, drug reaction, internal malignancy or idiopathic way. Erythrodermic psoriasis (EP) is the most common type of erythroderma, accounting for about 25%. A 70-year-old man presented with diffuse erythematous patch and scaling on whole body for three months. The patient denied any previous history of inflammatory dermatosis or treatment. Laboratory tests and radiologic findings showed no evidence of internal malignancy or infection. Histopathologic findings revealed parakeratosis, Munro microabscess, regular acanthosis, and clubshaped epidermal ridges. Diagnosis of erythrodermic psoriasis was made based on clinicopathologic findings. The skin lesions were recalcitrant to systemic steroid and immunosuppressants. To our knowledge, EP without a history of psoriasis is rare and tends to be recalcitrant to various treatments. Herein, we report a rare case of recalcitrant EP without a history of psoriasis.

        Comparison of Physiological and Biochemical Responses of Two Poplar Species under Drought Stress

        Drought stress is a crucial environmental factor for plant survival, and the development of drought-tolerant varieties isone of the goals of all cultivated plant researchers. In particular, the seedling stage is important for plant growth and development andis also the period most affected by drought. We investigated the initial response to drought stress in seedlings of two species of poplar(Populus alba × Populus davidiana and Populus davidiana) that phenotypically differed in drought tolerance. Comparative analysis interms of shoot height, photosynthetic pigments, soluble sugars, antioxidants, proline, soluble protein, malondialdehyde (MDA), andhydrogen peroxide (H2O2) contents was used to measure the physiological and biochemical characteristics of drought stress, anddrought-related genetic changes were also examined. Significant changes in shoot height, chlorophyll fluorescence, chlorophyllcontents, MDA and H2O2 appeared more adversely in Populus alba × Populus davidiana than in Populus davidiana, whereasreductions in soluble protein, carotenoids, superoxide dismutase (SOD) and catalase (CAT), which are indicators related to droughttolerance, appeared less in Populus davidiana. The change pattern of genes related to electron transfer and H2O2 production were almostsimilar in the two species, and among the drought response genes, lipid transfer protein 3 (LTP3) was greatly upregulated only inPopulus davidiana. In the initial response to drought stress of both poplars, Populus davidiana, which had good antioxidantmaintenance, showed better drought tolerance than Populus alba × Populus davidiana, which had a faster response to osmotic balancecontrol.

        과거 및 가상 지진해일에 의한 임원항의 침수예상도

        The scale of disaster and damage witnessed in the 2004 Indian Ocean Tsunami and the 2011 Great East Japan Tsunami has motivated researchers in developing foolproof disaster mitigation techniques for safety of coastal communities. This study focuses on developing tsunami hazard map by numerical modeling at Imwon Port to minimize losses of human beings and property damage when a real tsunami event occurs. A hazard map is developed based on inundation maps obtained by numerical modeling of 3 past and 11 virtual tsunami cases. The linear shallow-water equations with manipulation of frequency dispersion and the non-linear shallow-water equations are employed to obtain inundation maps. The inundation map gives the maximum extent of expected flooded area and corresponding inundation depths which helps in identifying vulnerable areas for unexpected tsunami attacks. The information can be used for planning and developing safety zones and evacuation structures to minimize damage in case of real tsunami events.
