밀교에서 지옥은 일반적인 대승경전에 설해 있는 대로 唯心의 지옥을 설하는 것이나 지옥에 가게 될 행위가 교훈적으로 금지되어 설해지고 있는 것은 차별이 없으나 지옥고의 세세한 부분에 대해서는 별다른 언급이 없다. 그러면서 墮地獄의 원인으로 일반 소ㆍ대승경전 에서 설하는 五逆罪나 인륜을 해치는 행위와는 달리 밀교에서는 아사리의 가르침을 따르지 않고, 비밀법을 엄수하지 않으며, 삼매에서 물러나는 것 등 성불을 장애하는 것을 금지하고자 여기에 중점적으로 墮地獄을 설하고 있다. 더욱이 종자와 진언, 인계,因果의 성품이 공함을 관찰함에 의해 모든 죄장을 제거하는 방향으로 역점을 두는 破地獄의 묘사가 두드러지게 설해지고 있다. 이것은 석존의 대기설 법이 그러하듯이 미혹한 중생들을 교화하기 위한 방편적 차원에서 이루어진 것이라고 할 것이다. 즉 밀교의 지옥관에 있어서 중요한 것 은보리심이 최대의 목적이 되며 올바른수행의 길로 이끄는 指導의 성격을 지닌다는 것이다. 밀교에서 설하는 지옥은 방편이기에 불보살의 깨달음을 표현한 만다라의 세계에 포함될 수 있다. 비로자나불을 중핵으로 한 만다라에 불보살의 불국토를 위시하여 지옥중생을 위한 外曼茶羅의 공간까지 있는 이유는 大悲로써 중생들을 성숙시키는 차제의 의미가 있음을 말한다. 부처의 청정한 만다라세계는 중생들의 생사세계와 아무런 접점이 없는 것이 아니라, 중생들의 미혹세계에 구제작용을 펼치는 세계이어야 하기 때문이다. Hell in esoteric Buddhism has a characteristic that it is developed thoroughly as the mind itself (yusim) by the principle of conditioned arising. As described in the general Mahayana Sutras, Hell that appears in one’s mind is described. However, there is no difference between esoteric Buddhism and the Mahayana Sutras in the description that the acts that condemn one to Hell are described instructively. In addition, concerning the detailed parts of the pains in Hell, esoteric Buddhism does not mention in particular. There is a big difference in the reason for going to Hell. Unlike in the Mahayana Sutras, doing bad things or behavior harming the human race makes one go to Hell, in esoteric Buddhism, it is explained that one goes to Hell if one does an act that is an obstacle against enlightenment, e.g. If one does not follow the teacher's teaching, does not keep the secret Dharma, or recedes from Samadhi (concentration). In other words, what is important in view of Hell in esoteric Buddhism is that asceticism is the greatest purpose, which has a character of a guide that leads to the right asceticism. This is part of efforts to teach foolish people like Buddha’s method of teaching in conformity with the mental capacity of the thought. Hell described in esoteric Buddhism is an expedient means, which can be included in the world of Mandala that expresses enlightenment. Buddha’s pure world of Mandala can be seen as a world that is engaged in helping the foolish world of beings, instead of not having any point of contact with the world of their life and death.

In human body, lower extremity has some differences from other parts of body such as less blood supply and relatively thin subcutaneous tissue especially in pretibial area, and also soft tissue necrosis and fracture often happened in industrial and traffic accidents. The choice of reconstruction method of injured leg depends on the size and site of defect, and condition of wound as well as contour and function after reconstruction. Donor site morbidity should be considered also. In general, small defect can be covered with local flap or skin graft, but if defect is large in size or combined with bone defect or associated with serious infection like osteomyelitis, free flap is inevitable in most cases and muscle flap which has good provision of blood circulation is well known to be effective in control of infection. The rectus abdominis muscles are paired anterior paramedian flexors of vertebral column, and they serve to tense the anterior abdominal wall. Rectus abdominis muscle flap or myocutaneous flap can be used as a island or free flap and it is now one of the most frequently used flap for reconstruction of leg with many advantages. From May,1990, eleven patients underwent lower extremity reconstruction with rectus abdominis muscle flaps or musculocutaneous flaps for soft tissue defect with serious infection, osteomyelitis, bone defect. The result is good in contour and function of leg as well as control of infection, so we report this paper with review of literatures.
