뇌수술 환자의 심부정맥혈전증 예방과 피부반응에 미치는 실험연구

본 연구는 뇌수술 후 부동 환자에게 부츠형 또는 무릎형 간헐적 공기 압박기 적용 후 심부정맥 혈전증 예방과 피부 반응에 미치는 영향을 평가하기 위한 실험연구이다. 연구대상은 2015년 5월 부터 11월 까지 뇌수술 후 외과계 중환자실에 입실한 60명을 대상으로 하였다. 심부정맥 혈전증 평가는 7일 동안 대퇴정맥 혈류속도의 변화를 매일 측정하였고 피부 반응 정도도 매일 규칙적으로 관찰하였다. 그 결과 부츠형 간헐적 공기 압박기 적용군이 무릎형 간헐적 공기압박기 적용군보다 시간이 경과함에 따라 대퇴정맥 혈류속도가 증가함을 확인하였다. 즉, 7일 동안 측정한 평균 대퇴정맥 혈류속도는 시간경과 에 따른 측정시기 오른쪽(F=64.41, p <.001), 왼쪽(F=58.21, p <.001)모두 통계적으로 유의한 차이를 나타내었다. 두 군 간의 발등 둘레는 측정시기와 집단 간의 상호작용에서 오른쪽(F=9.13, p <.001), 왼쪽(F=9.29, p<.001) 모두 통계적으로 유의한 차 이를 보였다. 또한 부츠형 간혈적 공기 압박기 적용군은 피부 합병증도 발생하지 않았다. 이러한 결과는 뇌수술 후 부동환자 에게 부츠형 간헐적 공기 압박기 적용이 심부정맥 혈전증을 예방하는데 보다 효과적임을 알 수 있다. This study examined the prevention of deep vein thrombosis and skin response after the application of boots or calf intermittent pneumatic compression (IPC) devices in immobile patients with intracranial surgery. The subjects of this study included 60 patients in a surgical intensive care unit after receiving intracranial surgery from May to November in 2015. The blood flow velocity was measured daily to assess the deep vein thrombosis for 7 days, and the skin response of the legs was observed regularly. As a result, the boots IPC group showed an increased blood flow velocity over time compared to the calf IPC group. A significant statistical difference was observed in both the boots IPC group and calf IPC group over time (Right side; F=64.41, p<.001, Left side; F=58.21, p<.001). The dorsum circumference over time and the correlations between two groups (Right side; F=9.13, p <.001, left side; F=9.29, p<.001) also showed a significant difference. In addition, the boots IPC group showed no skin complications. In conclusion, the boots type IPC is a more effective method for preventing deep vein thrombosis for immobile patients with intracranial surgery.

여순사건 진압을 위한 대항 게릴라 작전과 민간인 희생화 전략

여순사건은 14연대 군인봉기에서 시작되었지만, 여수·순천을 비롯한 전남 동부지역 좌익세력과 결합되면서 내전으로 확대되었다. 여수·순천을 비롯한 ‘반란’지역은 새로운 세상과 해방을 꿈꾸는 공화국의 근거지가 되었고, 대중들의 흥분과 열정이 모든 것을 지배하였다. 여순사건에서 많은 우익인사들이 ‘악질반동’으로 규정되어 참혹하고 잔인하게 처형되었다. 해방 후 좌우대립과 투쟁, 정부의 억압과 탄압 속에서 형성된 적대와 분노는 여순사건이라는 내전과정에서 계급의 적에 대한 극단적인 분노로 폭발하였다. 군경의 여순사건 진압작전은 초기의 군인봉기가 내전으로 확대되는 중요한 계기가 되었다. 여순사건에서 정부군과 치열한 전투를 치룬 반란군과 좌익세력은 산악지역을 거점으로 한 게릴라 운동으로 방향을 전환하였다. 여순봉기 세력이 지리산을 중심으로 게릴라 정치를 지속하자, 정부는 이들을 상대로 새로운 작전개념과 전술을 채택하였다. 이들 작전과 전술의 주요한 내용은 대개 계엄령, 초토화 정책과 방화, 부락소개와 집단부락 건설, 비민분리 정책 등이었다. 일반적으로 대항 게릴라 작전에서 계엄령은 군이 사태를 장악하고 주민을 통제하는 수단으로 활용되어 왔다. 여순사건에서 계엄령은 국가가 위기상황에서 발동하는 최고 비상조치라는 일반적 의미를 떠나 군사작전의 효율성과 편의성을 극대화하는 기능을 갖고 있다. 여순사건 진압과정에서 발생한 민간인 대량학살은 넓은 의미에서 방화나 소각, 무차별 사살 등을 포함한 초토화 정책의 결과였다. 국군이 지리산을 중심으로 게릴라를 토벌하는 작전에서 방화 또는 소각은 주요한 전술이었다. 부락소개는 대항 게릴라 작전에서 전통적으로 활용되는 전술로서 게릴라와 민간인을 공간적 사회적으로 분리하는 이른바 비민분리 정책의 중심이었다. 일반적으로 대항 게릴라 작전에서 전투원과 비전투원의 구별, 이른바 비민분리정책은 가장 핵심적인 정책이었다. 정부와 국군은 여순사건 이후 산악지역의 게릴라 소탕작전에서도 비민분리 정책을 일정하게 추진하였지만, 많은 한계를 안고 있었다. 군경당국은 게릴라 토벌작전의 군사적 효율성만을 고려하여 비민을 분리, 구별하지 하고 일체화시켜 작전대상으로 삼았다. 군경은 게릴라와 민간인 모두를 작전 목표로서 사살의 대상으로 삼는 민간인 희생화 전략을 채택하였고, 이는 민간인 대량학살이 발생하는 배경이 되었다. 전투원에 비해 민간인의 희생비율이 과도하게 높은 비대칭적 구조는 우발적 요인이라기보다는 민간인을 작전대상에 포함시킨 계획적 결과였다.

정부수립 전후 시기 국민형성의 동종화와 정치학살의 담론 발전

The political violence in the period of establishment of South Korean government mainly focused on the left wings. Korean government and right wings announced ‘emergency state’ system and began ‘left sweeping project’. With this mass mobilization system, the government developed this project on the base of mutual accusation and surveillance, common responsibility system. This shows the typical character of emergency state(or inspection state) whose target was the complete sweeping of left wings. Korean Government by right wings in this period pursued the demolition of political survival against left wings through the political violence in order not to challenge their power. However, this left sweeping project brought about the excess of political violence, and turned into the making of internal enemy, that is to say red making, over the simple elimination of lefts. Furthermore, this was connected to a kind of revenge culture which meant the efforts to solve the grudge, desire or hatred in the personal level, and the instrument of control for social order and power sustenance through the making common enemy as reds in social level. The use of political violence in this period was also the process for nation making. In the light of nation making, the organizing of national guidance alliance(NGA) was a sort of nation making strategy in order to make leftists of South Korea become ‘right friendly’ or ‘neutralized’ nation. This work was mostly practiced by the formality of confession politics with violence. The confession politics was no less stigmatization to reds than nation making through the regulation and publicization of past leftist's activity. The purpose of left sweeping was the intensification of rightist's political power and maximization of purification by removal of rebellious elements. Generally, the fear of pollution by leftists was directly connected to the madness of purification. Therefore, this perception was only an expression of nation homogenization or ideological pure racism in the process of nation formation. Through this process, Korean nationalism showed the possibility of change which turned to the political culture like annihilation of the differences or mass killing. Even though the policy to leftist of Korean government in this period had pursued more adaptation, control or separation to the system than massive annihilation, it had also included the possibility of politicide. As a matter of fact, this policy produced many kinds of discourses like non-nationalization/dehumanization, demonization, and some part of this culture was realized politicide as a annihilation policy after Korean war.
