일차 진행성 실어증(Primary Progressive Aphasia, PPA) 환자군을 대상으로 한 효율적이고 체계적인 진단과 중재가 이루어지기 위해 기초연구가 많이 필요함에도 불구하고, 신경언어장애 내치매 하위분야에서도 이들에 대한 연구는 부진한 편이다. 본 연구에서는 최근 10년간 국내외 관련 학술지에서 다루어진 PPA 연구논문의 동향을 분석하여 국내 PPA 연구의 향후 방향을 제시하는데 목적을 두었다. 일차적으로 선별된 171편의 논문을 대상으로 연구대상, 연구목적, 연구영역으로 나누고 연도별 연구동향에 대한 분석을 실시하였다. 연구대상은 국내외 모두 ‘실문법형’이 가장 많았고, 연구목적은 PPA의 전반적 특성이나 세 아형별 비교를 다룬 ‘특성연구’가 가장 많이 차지하고 있었다. 연구영역은 국외 연구에서는 의미(언어) 영역을 가장 많이 다루고 있으면서 ‘말’ 과 ‘기타’연구도 골고루 이루어져 있었던 반면, 우리나라는 ‘언어’ 영역을 제외한 ‘말’과 ‘기타’ 영역에 대한 연구가 아직 시행되지 않은 것으로 나타났다. 국외에서는 기존의 3가지 아형으로 나누어지던 PPA 분류에 말실행증(apraxia of speech) 분류가 추가되어 4가지 증상의 구별에 대한 연구가 활발히 모색되고 있는 반면, 국내는 환자 사례 발출이 어려워 이들이 보이는 증상이 의사소통 장애에 상당히 초점이 맞춰져 있음에도 불구하고 시행된 연구가 거의 없었다. 기본적으로 관련연구의 수가 절대적으로 부족하고 연구 영역의 다양성이 부족한 것으로 나타난 만큼, 향후 이에 대한 관심과 연구 확대가 필요하다. The purposes of this study were to determine recent trends of domestic and international studies for primary progressive aphasia (PPA) patient group and to identify areas for future research. For these purposes, 171 studies on communication disorders of a PPA were chosen and sub-categories for research methods and contents were also analyzed. The sub-categories for research methods include types of research subject, research purpose andresearch field. As the research subject, ‘agrammatism’ was most frequently dealt with in both domestic and international studies; as the research purpose, ‘characteristics study’ accounted for the largest portion; and with respect to the research field, the international studies have covered evenly language, speech and others, while the domestic studies have handled only the field of ‘language,’ paying no attention to ‘speech’ and ‘others.’ In the international studies, apraxia of speech was added to the conventional three sub-types of PPA classification, so distinction between the four symptoms has been actively studied; however, patient cases were difficult to find in Korea, so the symptoms the patients had, although centering around communication disorders, have been rarely researched in the domestic studies. The results suggested that there have been extremely few studies related and their research field has lacked diversity. We suggested a direction for future research on communication disorders with PPA. \
