        보험진료비 심사일원화의 사회ㆍ경제적 효과

        현재 우리나라의 주요 사회보험인 건강보험, 자동차보험, 산재보험은 진료비심사와 지급주체, 관련법령이 상이하게 되어 있으며, 이로 인해 진료수가체계 및 급여기준이 다원화되어 있으며, 적용 보험 종류에 따라 급여항목에 대한 각기 다른 종별 가산률을 적용하고 있다. 이렇듯, 다원화되어 있는 현재의 진료비 심사제도로 인해 동일질병ㆍ상해에 대해 각 보험의 서로 다른 기준적용으로 진료비, 입원기간, 입원율에 차이가 발생하게 된다. 본 연구는 건강보험, 자동차보험, 산업재해보상보험의 의료비심사를 현행 건강보험수가 기준에 적용했을 경우의 심사결과를 예측해봄으로써 다원화되어 있는 현재의 제도하에서의 결과치와 보험진료비심사를 일원화 했을 경우의 결과치를 비교 분석함으로써 보험진료비심사일원화를 위한 정책 수립에 도움이 되고자 하였다. Our current major social insurances such as National Health, Automobile, and Work Accident insurance are different from each other in terms of review system, reimbursement, and related regulation, which led to pluralization of price system for medical review and of costs, and different additional rates are applied to each benefit item depending on type of insurance. Thus, due to pluralization of current medical review system, differences of medical cost, days of hospital care, and hospitalization rate are generated by applying different standards for each insurance. This paper suggests review result when current National Health insurance review system applied to Health, Automobile, and Work Accident insurance to compare and analyze the result from integrated insurance medical review system in order to establish a policy for integrating insurance medical review systems.

        약국의 수익성 결정 요인에 관한 연구

        본 연구는 보건의료 전달체계 내에서 약국의 수익성에 영향을 미치는 요인을 분석하고, 약 국의 운영 및 관리에서의 문제점 파악을 통해 약국서비스의 질적 담보와 안정적인 약국서비스 제공을 위한 정책적 개선방향을 모색하기 위해 수행되었다. 이를 위해 2006년 실시된 약국경영활성화를 위한 실태조사 자료를 이용하여 기본적인 기술통계, ANOVA 및 회귀분석을 설시 하였다. 그 결과 약국의 수익성에는 약사의 생산성, 그리고 비용관리와 약국의 인지도가 영향을 미 치는 것으로 나타났다. 그 중 비용관리의 경우 인건비 비율은 약국의 수익성과 부(-)의 상관성을 가지고 영향을 미치고 있었으며, 단골고객의 비율 역시 약국의 수익성과 부(-)의 영향을 미 치는 것으로 나타났다. This study aimed to understand the problems of the management of pharmacy and to analyze the primary factor affecting the profitability of pharmacies as a part of the non-benefit constituents in health cam system for quality secure and stable pharmacy service. This study refers to the reseamh on the actual condition data for pharmacy management revitalization of 2006, and operated basic stair pica) analysis, ANOVA, and regression by using SPSS to analysis relationships between traits and profitability of the pharmacies. The results of this study are as follows. Expenditure indicatois, inventory level indicators, and customer recognilion indicators are affect negative effect to productivity. Also the opening year and pairon ratio shows negative effect to productivity. Only the pmductivity indicators appeared to affect positive effect to pharmacy`s profitability. When design this study, there was a concern about how the regional traits and customer service of pharmacies affects to pharmacy`s profit, but we couldn`t find statistical significance to be attended to factor analysis and correlational analysis. It interprets as meaning of customer service is not that big with pharmacy`s management technique. As a result which factors and how affect in direct or indirect each other were identified: expenditure, inventory level, customer recognition and productivity affect pharmacy`s profitability.

        의약분업이후 약국 경영수지 및 관련 요인 분석

        The purpose of this study is to analyze the factors affecting financial performance of community pharmacies after the implementation of the new drug policy in Korea: separation of drug prescription and dispensing (SPD). The online survey questionnaires were sent to the active pharmacists of the total 20,633 community pharmacies in Korea and 1,147 pharmacists responded to the survey (the response rate was 5.5%). The questionnaire asked simple financial data in order to compare their financial performances before and after the policy. With the SPSS package (version 12.0), two levels of data analysis were used: 1) descriptive statistics to see the financial status of the pharmacies; and 2) multiple regression analysis to find the factors. {or A multiple regression method was used for the data analysis.} The finding illustrated that the average net benefit of the pharmacies was 4,870 thousand won in August 2006, and the major factors affecting the net benefit were the location of pharmacy (geographical proximity to medical institutions), number of prescriptions, drug management services for patients, chain pharmacy membership. The findings of this study suggest that service improvement for patients, revitalization of non-prescription drug sales, professional management will contribute to promote community based pharmacy business.

        노인의 건강증진활동 경험이 건강증진활동 의지 형성에 미치는 영향

        This study's purpose is to find the influence factor of health promotionintention in the young old. And to discuss heath promotion policy for the elderlywith study results. The survey period of this study was from Sept. 13th to Dec. 10th, 2013. 232 complete questionnaires were used for final analysis. Thecollected data were analyzed by SPSS Ver. 19, and reliability analysis, factoranalysis, ANOVA, t-test. And then used Amos Ver. 9 to path analyzed. Theresearch Hypothesis were showed that endogenous variable was influencedsignificantly direct or indirect by other variables such as socio-demographic,satisfaction of facilities and coach, social supports, perceived health promotioneffect. Especially, Social supports presented higher effect(total effect .551) thanexogenous variables. It is need to formulate health policy that is to encouragesocial supports, to improve coach's capability and satisfaction for the young old. 노인의 건강증진을 위한 정책적 노력에도 불구하고 운동실천율은 계속 감소하고 있다. 본 연구에서는 건강증진모형, 건강신념모형 등을 참고한 연구모형을 통해 이전의 경험과 사회적 지지가 노인의건강증진활동 의지에 미치는 영향을 경로분석을 통해 살펴보았다. 건강증진프로그램 참여 노인을 대상으로 설문조사를 실시하여 분석한 결과 이전의 관련행위 들은 인지된 건강상태를 통해 직간접적으로 건강증진활동 의지에 영향을 미쳤으며, 사회적 지지는 가장 큰 효과(총효과 .551)로 직접 영향을미쳤다. 연구결과를 바탕으로 향후 노인의 건강증진을 위해 사회적 지지의 강화와 공공체육시설에 대한 질 관리 및 노인체육활동을 위한 전문가 육성체계 마련 등의 정책을 제안하고자 한다.

        소비자 특성별 PHR을 이용한 평생건강관리서비스 이용행태: 소비자 수요도 조사

        Objective: This study was conducted to investigate public opinion regarding personal health management service using Personal Health Record (PHR) in Korea. Specifically, the relationships between consumer's demographic characteristics, need for health information, consumer's benefit and concern, type of health information, type of management and security of patient information using PHR and attitude to PHR were explored. Methods: A survey was conducted, using questionnaire developed by the study team based on literature review. Structured questionnaires were completed through a telephone survey for 715 consumers. Results: A key finding was the significant difference for customer's attitude towards using PHR by customer's characteristics. Regarding factors affecting attitudes of customers with higher concern about their health, considering authorization level to access their private information and using reliable method to certificate were more likely to prefer PHR system positively. Conclusion: Public opinion is reflected in policy of health information. Meeting consumers' needs sufficiently, the goal of health information service to improve consumers' accessibility to and satisfaction will archive successfully

        의료서비스의 대기시간 요인이 고객만족과 서비스 가치에 미치는 영향

        One of the important complaint factors by the patients using medical institutions is various kinds of waiting time. This research is about the causal relationships among waiting time(application of waiting time, perceived of waiting time, fairness of waiting, explanation of the delay, acceptance possibility), service values, and customer satisfaction. The subjects for this study were 256 out-patients currently using seven general hospitals in the Busan area. The findings of the empirical analysis are as follows: First, the good application of waiting time or fair waiting time procedure didn`t actually affect perceived waiting time. Second, though the application of waiting time didn`t affect acceptance possibility, the fair procedure of waiting time had an affirmative effect on acceptance possibility. Third, even when there was enough explanation about the need to wait, it could shorten perceived waiting time but failed to get sufficient sympathy or agreement from the patients. Fourth, the possibility of acceptance following the sufficient sympathy of patients had an effect on shortening perceived waiting time. Fifth, perceived waiting time lowered customer satisfaction, but had no negative effect on service values. Finally, the ranges of acceptance possibility by the patients were found to increase customer satisfaction and maximize service values. In conclusion, every medical institution needs to focus on, first of all, shortening waiting time for more customer satisfaction and improved service values.
