간호·간병통합서비스 병동간호사의 의사소통능력과 간호근무환경이 노인 간호실천에 미치는 영향

인구의 고령화와 핵가족화로 인해 가족의 간병과 돌봄에 대한 경제적, 신체적 부담이 급속히 늘어나면서 환자 개인의 문제를 넘어 사회적 부담이 가중되었다. 이에 정부는 간호 · 간병통합 서비스 제도를 도입하였으며, 시범사업이 시작된 이후로 점차 확대 운영되고 있다. 노인 환자가 병원에서 차지하는 비율이 높아지면서 간호⋅간병통합서비스 병동 간호사는 노인 문제를 총체적으로 파악하고, 정신적 신체적 측면의 간호를 제공해야 한다. 본 연구는 간호 · 간병서비스 병동 간호사의 의사소통능력, 간호근무환경, 노인 간호실천 정도를 파악하고 의사소통능력과 간호근무환경이 노인 간호실천에 미치는 영향을 알아보기 위한 서술적 조사연구이다. 자료수집은 2021년 07월 16일부터 08월 06일까지 수도권에 소재한 100병상 이상 800병상 이내 규모의 간호 · 간병통합서비스를 시행하는 5개의 병원에서 총 임상경력이 1년 이상인 간호 · 간병통합서비스 병동 간호사 181명을 대상으로 하였다. 도구는 의사소통능력 척도(Global Interpersonal Communication Competence (GICC), 간호근무환경 측정도구(K-PES-NWI), 노인 간호실천을 측정하는 도구를 사용하였고, 수집된 자료는 IBM SPSS/WIN 25.0 통계프로그램을 이용하여 분석하였다. 연구결과는 다음과 같다. 간호 · 간병통합서비스 병동에서 돌봄을 하는 간호사의 평균 연령은 31.74±7.51세, 미혼자가 127명(70.2%)로 대부분을 차지했고, 일반 간호사가 142명(78.5%), 총 임상경력은 평균 8.03±6.67년, 간호 · 간병 통합서비스 병동 경력은 평균 2.03±1.28년 이었다. 월 평균 급여는 300만원 이상 350만원 미만이 105명(58.0%), 근무형태는 3교대가 149명(82.3%), 노인 간호 선호여부는 ‘보통이다’라고 답한 군이 110명(60.8%)으로 가장 많았다. 의사소통능력의 점수는 75점 만점에 평균 55.46±5.99점, 간호근무환경의 점수는 116점 만점에 평균 76.29±11.61점이었다. 노인 간호실천 점수는 64점 만점에 평균 53.87±6.08점이었다. 노인 간호실천은 의사소통능력(r=.37, p<.001) 및 간호근무환경(r=.31, p<.001)과 양의 상관관계이며, 간호근무환경과 의사소통능력(r=.24, p=.001)도 양의 상관관계이었다. 본 연구에서 노인 간호실천과 관련된 예측 요인은 의사소통능력(β=.30, p=<.001), 간호근무환경(β=.24, p=.001), 총 임상경력(β=.22, p=.001)인 것으로 나타났으며, 노인 간호실천에 대한 설명력은 21.9%이었다. 간호 · 간병통합서비스 병동 간호사의 간호근무환경을 개선하고 의사소통능력을 키운다면 노인 간호실천이 향상 될 것으로 사료된다. 이에 효율적인 간호근무환경 조성과 간호사 대상의 의사소통능력 향상을 위한 교육 프로그램이 필요하다. The aging of population and nuclear family are causing a major social problem due to economic and physical burden caused by family care of themselves at home. Accordingly the government instituted a pilot project named the comprehensive nursing care service system and it has been gradually expending from the first time. As population of old patients grow, nurses who working in the comprehensive nursing care should understand problems of old patients and provide not just physical care but also mental care. This study is a descriptive research to identify communication ability, nursing work environment and geriatric nursing practice in comprehensive nursing care service word, and to investigate effect of communication ability and nursing work environment on geriatric nursing practice. Data collection was conducted from July 16 to August 06 2021, a total 181 nurses with more than 1 year of clinical experience in a comprehensive nursing care service ward were targeted and those five hospitals where they work more than 100 beds and less than 800 beds in the metropolitan area. Measurement instruments used a Global Interpersonal Communication Competence(GICC), a nursing work environment measurement tool(K-PES-NW1), and a tool to measure geriatric nursing practice, and the collected data were analyzed using the IBM SPSS/WIN 25.0 statistical program. The results are as follows. The average age of nurses who care in the comprehensive nursing care service ward was 31.74±7.51 years, 127 unmarried (70.2%), 142 staff nurses(78.5%), total clinical experience was 8.03±6.67 years, and average comprehensive nursing care service ward experience was 2.03±1.28 years. The average monthly income was 105 people(58.0%) with more than 3 million won and less than 3.5million won, 149 people(82.3%) with three shifts, and 110 people (60.8%) answered ‘Average’ in terms of preference for geriatric nursing. The average score of communication ability was 55.46±5.99 out of 75, the average score of nursing work environment was 76.29±11.61 out of 116, and the average score of geriatric nursing practice was 53.87±6.08 out of 64. Geriatric nursing practice and communication skills(r=.37, p<.001), nursing work environment(r=.31, p<.001) was found to have a positive correlation. The basis for communication skills and nursing work environment(r=.24, p=.001) showed a positive correlation. The factor influencing geriatric nursing practice is communication ability(β=.30, p=<.001), nursing working environment(β=.24, p=.001), total clinical experience(β =.22, p=.001). The explanatory power for geriatric nursing practice was 21.9%. If the nursing work environment of nurses in the comprehensive nursing care service ward is improved and communication skills are trained, geriatric care is expected to develop. Therefore, It is demanded to organize an efficient working space for nurses and an educational program that advance communication for comprehensive nursing care service provider.
