갑상선침생검으로 확인된 아급성 갑상선염 26예의 임상적 고찰

N/A The clinical features and laboratory findings of 26 patients with histologically-proven subacute thyroiditis were examined from March 1984 to December 1988 at the Masan Koryo Hospital. The results were as follows: 1) All 26 patients with subacute thyroiditis were women, and the age distribution showed the peak incidence to be between the third and fourth decades(average age : 40 years). 2) The seasonal distribution showed the peak incidence to be between summer(34.6%) and autumn(30.8%). 3) The chief complaints were frequent anterior neck pain(69.2%), anterior neck swelling(23.1%) and fever(7.7%). The peak time from onset was 1-2weeks(50.0%). 4) On physical examination, 15 cases were nodular goiter(57.7%), and 11 cases were diffuse goiter(42.3%). All cases except one had moderate or severe tenderness. All 26 cases had a firm or hard consistency. 5) The mean value of ESR was elevated to 40.9 mm/ hr, and 12 cases(46.2%) were of mild or moderate hyperthyroid status. The serum thyroglobulin level was checked in 19 patients, and the mean value was elevated to 169.1 ng/ml. The positive rates for antithyroglobulin and antimicrosomal antibody were 1/24 and 3/24, respectively. 6) The thyroid scans were classified into the follwing groups: 15 cases(57.7%) showed both lobes nearly nonvisualized, 10 cases(38.5%) showed one lobe nearly nonvisualized, and one case(3.8%) showed both lobes slightly diffuse and enlarged. 7) Among the 26 cases, four were checked with radioactive iodine uptake(RAIU). The value was markedly depressed from 1-4% in all cases except one. 8) With aspirin or prednisolone therapy, the pain subsided and the goiters were reduced within several days.
